    Laparoscopic-assisted percutaneous internal ring ligation in children

    Aim To evaluate the feasibility and safety of laparoscopic ligation of internal ring in congenital inguinal hernia in children.Patients and methods Laparoscopic percutaneous ligation of internal inguinal ring has been performed on 97 children with 133 hernias. The age ranged between 6 months and 11.5 years. The procedure was carried out under general anesthesia, through skin puncture opposite to the lateral edge of the internal ring. The needle was passed under direct vision to encircle the internal ring without touching the vas or testicular vessels. The heel of the needle is backed through the subcutaneous tissue and out of the original stab puncture. The contralateral side was examined and the internal ring was closed if it was patent.Results All children were subjected to a follow-up schedule for 1 year to evaluate both efficacy and reliability of the technique. The mean operative time in our series was 15 (± 3) and 20 (± 5) min for bilateral cases, without anesthesia time. Of 133 hernias repaired, two hernias recurred at 1 and 2 months postoperatively. Conversion rate was 0% in our hands. Five (3.8%) cases developed minimal extraperitoneal hemorrhage during surgery. Six (4.5%) cases developed mild hydrocele, which was successfully treated conservatively.Conclusion Laparoscopic percutaneous ligation of internal inguinal ring repair of congenital hernia is proved to be a safe, simple, and easy procedure and can be performed routinely. The procedure has better cosmetic effect because we use a single umbilical port only.     Design optimization of three dimensional geometry of wind tunnel contraction

    AbstractThe aim of the present study is to redesign three dimensional geometry of existing open circuit wind tunnel contraction. The present work achieves the recommended contraction ratio, maximum uniformity at the working section mid-plane, without separation, no Gortler vortices in the contraction, and minimizing the boundary layer thickness at entrance to the working section. Using CFD along with optimization tools can shorten the design optimization cycle time. Moreover CFD allows insight into the minute flow details which otherwise are not captured using flow bench tests. The design exploration algorithm is used to optimize the profile of the contraction in an automated manner. The optimization is based on using screening method to choose the best design set and verified by the CFD solver. The new contraction, compared to the old design contraction is confirmed using CFD. The new design is manufactured in full scale. The optimized contraction is investigated computationally and experimentally

    Safety Pin Suture for Management of Atonic Postpartum Hemorrhage

    Objective. To assess the efficacy of a new suture technique in controlling severe resistant uterine atonic postpartum hemorrhage. Patients and Methods. This is a retrospective observational study that included thirteen women with uterine atony and postpartum bleeding that did not react to usual medical management. All these women underwent compressing vertical suture technique in which the anterior and posterior walls of the uterus were attached so as to compress the uterus. The suture is transfixed at the uterine fundus, thus eliminating the risk of sutures sliding off at the uterine fundus (safety pin suture). Results. safety pin uterine compression suture was a sufficient procedure to stop the bleeding immediately in 92.2% of the women. None of the women developed complications related to the procedure. Conclusion. A new safety pin suture is a simple and effective procedure to control bleeding in patients with treatment-resistant, life-threatening atonic postpartum hemorrhage with the advantage of eliminating the risk of the sutures sliding off at the uterine fundus
