Međudjelovanje četiri-razinskog atoma u impulsnom svojstvenom stanju s jednomodnim poljem
We study the interaction between a four-level atom (ladder type) in a momentum eigenstate with a single mode cavity field in the presence of non-linearities of both the field and the intensity-dependent coupling. The constants of motion and the wave function for the atomic system have been obtained. Special attention is given to discuss some statistical aspects of the considered atomic system such as momentum increment, momentum diffusion and high-order squeezing. The influence of the Kerr-like medium and the intensity dependent coupling on the momentum increment and the high-order squeezing are investigated numerically. It is found that addition of these parameters has an important effect on both the momentum increment and the squeezing phenomenon.Proučavamo međudjelovanje četiri-razinskog atoma (poput ljestvi) u impulsnom svojstvenom stanju s jednomodnim poljem u rezonatoru, uz nelinearnosti polja i vezanja ovisnog o intenzitetu. Izveli smo stalnice gibanja valne funkcije atomskog sustava. Posebnu smo pažnju posvetili raspravi o statističkim odlikama razmatranog atomskog sustava, kao što su povećanje i difuzija impulsa i zbijanje višeg reda. Numerički smo istražili utjecaj Kerrovog sredstva i vezanja ovisnog o intenzitetu na povećanje impulsa i zbijanje višeg reda. Našli smo da dodavanje tih parametara ima snažan učinak na povećanje impulsa i pojavu zbijanja
Isogeometric analysis for functionally graded microplates based on modified couple stress theory
Analysis of static bending, free vibration and buckling behaviours of
functionally graded microplates is investigated in this study. The main idea is
to use the isogeometric analysis in associated with novel four-variable refined
plate theory and quasi-3D theory. More importantly, the modified couple stress
theory with only one material length scale parameter is employed to effectively
capture the size-dependent effects within the microplates. Meanwhile, the
quasi-3D theory which is constructed from a novel seventh-order shear
deformation refined plate theory with four unknowns is able to consider both
shear deformations and thickness stretching effect without requiring shear
correction factors. The NURBS-based isogeometric analysis is integrated to
exactly describe the geometry and approximately calculate the unknown fields
with higher-order derivative and continuity requirements. The convergence and
verification show the validity and efficiency of this proposed computational
approach in comparison with those existing in the literature. It is further
applied to study the static bending, free vibration and buckling responses of
rectangular and circular functionally graded microplates with various types of
boundary conditions. A number of investigations are also conducted to
illustrate the effects of the material length scale, material index, and
length-to-thickness ratios on the responses of the microplates.Comment: 57 pages, 14 figures, 18 table
Multiphoton Bloch-Siegert shifts and level-splittings in a three-level system
In previous work we studied the spin-boson model in the multiphoton regime,
using a rotation that provides a separation between terms that contribute most
of the level energies away from resonance, and terms responsible for the level
splittings at the anticrossing. Here, we consider a generalization of the
spin-boson model consisting of a three-level system coupled to an oscillator.
We construct a similar rotation and apply it to the more complicated model. We
find that the rotation provides a useful approximation to the energy levels in
the multiphoton region of the new problem. We find that good results can be
obtained for the level splittings at the anticrossings for resonances involving
the lower two levels in regions away from accidental or low-order resonances of

Mutations in ASXL1 are associated with poor prognosis across the spectrum of malignant myeloid diseases
The ASXL1 gene is one of the most frequently mutated genes in malignant myeloid diseases. The ASXL1 protein belongs to protein complexes involved in the epigenetic regulation of gene expression. ASXL1 mutations are found in myeloproliferative neoplasms (MPN), myelodysplastic syndromes (MDS), chronic myelomonocytic leukemia (CMML) and acute myeloid leukemia (AML). They are generally associated with signs of aggressiveness and poor clinical outcome. Because of this, a systematic determination of ASXL1 mutational status in myeloid malignancies should help in prognosis assessment
Relation of gallbladder function and Helicobacter pylori infection to gastric mucosa inflammation in patients with symptomatic cholecystolithiasis
Background. Inflammatory alterations of the gastric mucosa are commonly caused by Helicobacter pylori (Hp) infection in patients with symptomatic gallstone disease. However, the additional pathogenetic role of an impaired gallbladder function leading to an increased alkaline duodenogastric reflux is controversially discussed. Aim:To investigate the relation of gallbladder function and Hp infection to gastric mucosa inflammation in patients with symptomatic gallstones prior to cholecystectomy. Patients: Seventy-three patients with symptomatic gallstones were studied by endoscopy and Hp testing. Methods: Gastritis classification was performed according to the updated Sydney System and gallbladder function was determined by total lipid concentration of gallbladder bile collected during mainly laparoscopic cholecystectomy. Results: Fifteen patients revealed no, 39 patients mild, and 19 moderate to marked gastritis. No significant differences for bile salts, phospholipids, cholesterol, or total lipids in gallbladder bile were found between these three groups of patients. However, while only 1 out of 54 (< 2%) patients with mild or no gastritis was found histologically positive for Hp, this infection could be detected in 14 (74%) out of 19 patients with moderate to marked gastritis. Conclusion: Moderate to marked gastric mucosa inflammation in gallstone patients is mainly caused by Hp infection, whereas gallbladder function is not related to the degree of gastritis. Thus, an increased alkaline duodenogastric reflux in gallstone patients seems to be of limited pathophysiological relevance. Copyright (c) 2006 S. Karger AG, Basel

A method of active case detection to target reservoirs of asymptomatic malaria and gametocyte carriers in a rural area in Southern Province, Zambia
<p>Abstract</p> <p>Background</p> <p>Asymptomatic reservoirs of malaria parasites are common yet are difficult to detect, posing a problem for malaria control. If control programmes focus on mosquito control and treatment of symptomatic individuals only, malaria can quickly resurge if interventions are scaled back. Foci of parasite populations must be identified and treated. Therefore, an active case detection system that facilitates detection of asymptomatic parasitaemia and gametocyte carriers was developed and tested in the Macha region in southern Zambia.</p> <p>Methods</p> <p>Each week, nurses at participating rural health centres (RHC) communicated the number of rapid diagnostic test (RDT) positive malaria cases to a central research team. During the dry season when malaria transmission was lowest, the research team followed up each positive case reported by the RHC by a visit to the homestead. The coordinates of the location were obtained by GPS and all consenting residents completed a questionnaire and were screened for malaria using thick blood film, RDT, nested-PCR, and RT-PCR for asexual and sexual stage parasites. Persons who tested positive by RDT were treated with artemether/lumefantrine (Coartem<sup>®</sup>). Data were compared with a community-based study of randomly selected households to assess the prevalence of asymptomatic parasitaemia in the same localities in September 2009.</p> <p>Results</p> <p>In total, 186 and 141 participants residing in 23 case and 24 control homesteads, respectively, were screened. In the case homesteads for which a control population was available (10 of the 23), household members of clinically diagnosed cases had a 8.0% prevalence of malaria using PCR compared to 0.7% PCR positive individuals in the control group (p = 0.006). The case and control groups had a gametocyte prevalence of 2.3% and 0%, respectively but the difference was not significant (p = 0.145).</p> <p>Conclusions</p> <p>This pilot project showed that active case detection is feasible and can identify reservoirs of asymptomatic infection. A larger sample size, data over multiple low transmission seasons, and in areas with different transmission dynamics are needed to further validate this approach.</p
Combined mutations of ASXL1, CBL, FLT3, IDH1, IDH2, JAK2, KRAS, NPM1, NRAS, RUNX1, TET2 and WT1 genes in myelodysplastic syndromes and acute myeloid leukemias
<p>Abstract</p> <p>Background</p> <p>Gene mutation is an important mechanism of myeloid leukemogenesis. However, the number and combination of gene mutated in myeloid malignancies is still a matter of investigation.</p> <p>Methods</p> <p>We searched for mutations in the <it>ASXL1, CBL, FLT3, IDH1, IDH2, JAK2, KRAS, NPM1, NRAS, RUNX1, TET2 </it>and <it>WT1 </it>genes in 65 myelodysplastic syndromes (MDSs) and 64 acute myeloid leukemias (AMLs) without balanced translocation or complex karyotype.</p> <p>Results</p> <p>Mutations in <it>ASXL1 </it>and <it>CBL </it>were frequent in refractory anemia with excess of blasts. Mutations in <it>TET2 </it>occurred with similar frequency in MDSs and AMLs and associated equally with either <it>ASXL1 </it>or <it>NPM1 </it>mutations. Mutations of <it>RUNX1 </it>were mutually exclusive with <it>TET2 </it>and combined with <it>ASXL1 </it>but not with <it>NPM1</it>. Mutations in <it>FLT3 (</it>mutation and internal tandem duplication), <it>IDH1</it>, <it>IDH2</it>, <it>NPM1 </it>and <it>WT1 </it>occurred primarily in AMLs.</p> <p>Conclusion</p> <p>Only 14% MDSs but half AMLs had at least two mutations in the genes studied. Based on the observed combinations and exclusions we classified the 12 genes into four classes and propose a highly speculative model that at least a mutation in one of each class is necessary for developing AML with simple or normal karyotype.</p
