Patient-reported outcome survey post primary cleft lip and palate surgery in Hospital Kuala Lumpur using oral health impact profile (COHIP) questionnaire
Introduction: Patient-reported outcome survey is one of the modality to
assess the outcome of surgical intervention from patient’s perspective. Cleft lip and
palate is one of the most common congenital abnormality encountered in plastic
surgery field. The primary surgical repair for both cleft lip and palate were among the
commonest procedure done in treating cleft lip and palate patients. The outcome of
the surgery includes the aesthetic results, speech, functionality self image and quality of life. Child Oral Health Impact Profile questionnaire is one of the reliable and valid assessment tools in the form of questionnaire to evaluate patient’s reported outcome post primary cleft lip and palate surgery.
Objectives: The aims of this study is to evaluate the patients reported outcome
post primary cleft lip and palate surgery in Hospital Kuala Lumpur using Child Oral
Health Impact Profile (COHIP) questionnaire. Specifically is to describe patient’s
perspective regarding their oral health, functional well-being, social/emotional wellbeing, school environment and self image post cleft lip and palate repair using the COHIP score. Second specific objective is to compare the mean difference of COHIP score between early and late timing of surgery. Third specific objective is to compare mean difference of COHIP score between 3 categories of surgeons.
Methodology: A cross-sectional study was carried out on patients with cleft
lip and palate in Hospital Kuala Lumpur, Malaysia. All patients details with orofacial
cleft operated between 1999 and 2005 that have been treated at the Department of
Plastic and Reconstructive Surgery, Hospital Kuala Lumpur, Malaysia were retrieved
from the operation theatre logbook to see the eligible patients in the age group of 8 to 14 years old to participate in the study. From the details collected, the patients that
fulfilled the criteria were invited to participate in the study via a phone call. The
appointment date was given to the patients. On the day of the appointment the patients and their parents/guardians were given a participant information sheet, contains the details of the study conducted. The researcher provided information and answered any question regarding the study. The consent form for parents/guardians and the child participant assent form were distributed after both parents/guardians and the patients agree to participate in the study. The COHIP Questionnaire was distributed to the participant after the researcher wrote down all the demographic details. The patient was given 30 minutes to complete the questionnaire.
Results: A total of 82 patients participated in the study. Of these, 41 (50.0%)
were male and 41 (50.0%) were female with 1:1 ratio of male to female. Their age
range is between 8 years old to 14 years old, with a mean age of 11.3 years old. The
majority of the subjects had left unilateral cleft lip and palate n=44 (53.7%). About
one-third of the subjects had bilateral cleft lip and palate n=25 (30.5%) and the
remainder had right unilateral cleft lip and palate n=13 (15.9%). Majority of the
subjects were operated by the specialist n=39 (47.6%), whereby consultants operatedon 35 subjects (42.7%) and trainee operated on 8 subjects (9.8%). Early primary cleft palate repair less than 1 year old of age were done to 52 subjects (63.4%). Late primary cleft palate repair more than 1 year old of age were done to 30 subjects (36.6%). There was a good outcome of the oral health, functional well-being, social emotional well-being, school environment and self image in primary repair cleft lip and palate with the mean overall Child Oral Health Impact Profile (COHIP) score of 102.2. There were significant differences of the overall COHIP and the oral health well-being, school environment and treatment expectation subscales score between the consultants, specialists and trainee. There was no significant difference of overall COHIP and all the subscales score between early and late timing of surgery.
Conclusions: Results showed the overall satisfaction post primary cleft lip
and palate repair was satisfactory and thus supported the continuation of current
management protocol in HKL for the management of cleft lip and palate patients

Earthquake early warning system
As the climate undergoes changes, the number of natural disasters increase and the severity of these occurrences increases over time. As the population of human race increases, natural protective terrains and barriers were destroyed for urbanization due to the exploitation of vulnerable areas on the earth. Earthquakes are unpredictable and as an initiative to adapt to climate changes and as a strategy to reduce earthquake disaster risk, early warning systems (EWS) has been internationally recognized to play a role in providing alerts to the public so that safety precaution procedures and protocols can be implemented in response to earthquake occurrences..
Parametric modelling of Malaysian teeth template using computer aided design
This study explored a new method and process of design denture using CAD technology to develop a template of a complete denture. Computer aided design were used as a tool of the design process. Occlusion curve was set up as variable as to follow patient size. The maxilla and mandible teeth arrangement were treated as a template of Malaysian user. The accuracy of design is the main aspect of concern that match the patients' data so that the outcome product would be suitable with maximum comfort for the patient. The product of design template will be matched with the conventional method to compare the tolerance between both. The new design template helps to reduce the time consumption of conventional carving method. The final output of 3D geometry teeth templates design will represent the patient details
Simulation based topology optimization assessment with manufacturing constraints
In Automotive and Aerospace industries, Topology Optimization (TO) is being used for the last few decades to produce lightweight structures. On the other hand, TO produces very complex geometrical features (i.e. irregular shape and hidden cavities along the thickness) that is sometimes difficult to be manufactured even with Additive Manufacturing (AM) and Casting techniques. In this paper suitable design and manufacturing constraint (MC) are applied during TO process that act as an Optimization Tool (OT) and improves geometrical features of the mechanical structures for easy manufacturing. Three mechanical structures with different geometries and boundary conditions have been considered for analysis purpose. Topology Optimization based on linear static analysis has been performed using OptiStruct (HyperWorks) solver. Finally, results of analysis conclude that the proposed OT produces lightweight structures with very simple geometries that can easily be manufactured with the help of AM or Casting techniques
Internal Quality Management Model in Islamic Boarding School-Based Madrasah
This research aims to (1) analyze the need for an internal quality management model and (2) design an internal quality management model in Islamic boarding school-based madrasas. This research uses the Research and Development (R&D) method using ADDIE (Analysis, Design, Develop, Implement, Evaluate) procedures and steps. The research results show that the Pesantren-Based Madrasah Quality Assurance Team must receive full support from the Islamic boarding school leadership. The Ministry of Religion's version of the Madrasah Self-Evaluation Instrument (EDM) must be developed to accommodate educational evaluations in Islamic boarding schools. Islamic boarding school-based madrasa's internal quality management model consists of four stages. First, the planning stage which consists of (1) mission alignment, (2) SWOT analysis, (3) EDM gap analysis, and (4) preparation of a quality plan. Second, the quality fulfillment stage consists of (1) determining the person responsible for the program, (2) funding allocation, and (3) reporting on quality implementation. Third, the quality evaluation stage consists of (1) formative evaluation, (2) summative evaluation, (3) preparation of evaluation instruments, (4) data collection, and (5) analysis and preparation of recommendations. Fourth is the quality follow-up stage, which consists of (1) follow-up improvement and (2) follow-up standardization of changes
Are Malaysians ready for Rabbit Meat? An exploratory to assess readiness of Malaysians to accept rabbit meat as an alternative protein source
This study explores the readiness of Malaysians to accept rabbit meat as an alternative protein source. As meat prices rise, identifying alternative options like rabbit meat becomes crucial. Rabbit meat offers nutritional value and is easy to breed, making it a potential substitute. Through a quantitative survey, the study reveals that many Malaysians acknowledge rabbit meat's health benefits and consider it a viable food source. However, concerns related to empathy for animals and the association of rabbits with pets influence resistance to its consumption. With these challenges, future interventions and education campaigns can promote wider acceptance of rabbit meat as an alternative protein source in Malaysia.
