Limb salvage surgery in chronic osteomyelitis : a case report

Osteomyelitis refers to an infection of the bone characterized by progressive inflammatory destruction caused by infecting microorganism. Open fracture carries the risk of developing osteomyelitis from 3 to 50% with a high amputation rate. Salvage of the limb is always a challenge and needs perseverance. We report the case of a 44-year-old male with chronic osteomyelitis with successful multiple limb salvage surgeries. The curative approach to chronic osteomyelitis has the following goals including arrest of infection, pain reduction and salvage of limb and function. With regard to survival and function, the result obtained in this case is more promising compared to above knee amputation

Tibialis anterior tendon intrasubstance tear associated with anterior talofibular ligament tear: a rare encounter

    Get PDF
Tibialis anterior (TA) tendon injury is an uncommon condition among other foot and ankle tendon pathology and there is insufficient literature on incomplete or intrasubstance TA tendon tear. To date, no cases of TA tendon injury with concurrent anterior talofibular ligament (ATFL) tear has been documented. This is a rare case of a 22-year-old gentleman with ankle injury following sports activity and magnetic resonance imaging (MRI) ankle showed incomplete TA tendon tear with concurrent ATFL tear. We proceeded with TA tendon direct repair and ATFL reconstruction. Patient showed good recovery and at third months, sixth months and 1-year post-surgery, American Orthopaedic Foot and Ankle Score (AOFAS) Ankle-Hindfoot scores were 86%, 90% and 96%, respectively. In TA tendon injury, incomplete tear or longitudinal split is reported mostly due to TA tendinopathy or degeneration. Early diagnosis followed by surgical treatment is shown to give better results, which can be either direct primary repair, tendon transfer or tendon reconstruction

High prevalence of vitamin D insufficiency and its association with BMI-for-age among primary school children in Kuala Lumpur, Malaysia

<p>Abstract</p> <p>Background</p> <p>Deficiencies of micronutrients can affect the growth and development of children. There is increasing evidence of vitamin D deficiency world-wide resulting in nutritional rickets in children and osteoporosis in adulthood. Data on the micronutrient status of children in Malaysia is limited. The aim of this study was to determine the anthropometric and micronutrient status of primary school children in the capital city of Kuala Lumpur.</p> <p>Methods</p> <p>A cross sectional study of primary aged school children was undertaken in 2008. A total of 402 boys and girls aged 7-12 years, attending primary schools in Kuala Lumpur participated in the study. Fasting blood samples were taken to assess vitamin D [as 25(OH)D], vitamin B12, folate, zinc, iron, and ferritin and haemoglobin concentrations. Height-for-age and body mass index for age (BMI-for-age) of the children were computed.</p> <p>Results</p> <p>Most of the children had normal height-for-age (96.5%) while slightly over half (58.0%) had normal BMI-for-age. A total of 17.9% were overweight and 16.4% obese. Prevalence of obesity was significantly higher among the boys (25%) than in the girls (9.5%) (χ2 = 22.949; <it>P </it>< .001). Most children had adequate concentrations of haemoglobin, serum ferritin, zinc, folate and vitamin B12. In contrast, 35.3% of the children had serum 25(OH)D concentrations indicative of vitamin D deficiency(≤37.5 nmol/L) and a further 37.1% had insufficiency concentrations (> 37.5-≤50 nmol/L). Among the boys, a significant inverse association was found between serum vitamin D status and BMI-for-age (χ2 = 5.958; <it>P </it>= .016).</p> <p>Conclusions</p> <p>This study highlights the presence of a high prevalence of sub-optimal vitamin D status among urban primary school children in a tropical country. In light of the growing problem of obesity in Malaysian children, these findings emphasize the important need for appropriate interventions to address both problems of obesity and poor vitamin D status in children.</p

The purpose of this study was to identify the financial performance of Islamic banking whose data were obtained from journals in the period 2017-2021 and how the solution was with a systematic literature review method approach. The results of a systematic literature review show that, 1). The distribution rate of articles in 2017 was 11, in 2018 there were 6, in 2019 there were 7, in 2020 there were 5 and in 2021 there were 2 articles in 31 journals, 2). The research method used by the researchers is a descriptive quantitative approach with a descriptive statistical approach and none uses a qualitative approach or mix methods, 3). The types of performance measurement tools used are mostly financial ratios, 4). The portrait of Islamic banking financial performance is volatile and tends to improve, 5). The solution for Islamic banks is to improve the performance of their financial ratios, and 6). The limitations of the study lie in the number of samples, variables and indicators, period of observation, and analytical technique
