    Discomfort experienced at the daily life of relatives of people admitted at ICU

    This is a qualitative research that aimed to get to know the discomforts experienced at the daily life of relatives of people admitted at the intensive care unit (ICU). It happened on a general ICU at a public hospital, in Salvador-BA, by the second half of 2009. Nine relatives of people admitted were interviewed. The technique used was the analysis from the Grounded Theory. The results showed that the interaction of the families with the reality of life threat from the relative admitted, had as main discomfort, the discontinuity in their daily life, which was characterized by four categories: Living the distress of a possible loss, difficulties to take care of themselves, facing a separation in the family, suffering with changes in their social and professional lives. These discomforts can be minimized by the healthcare team’s effectiveness to the demands of the family and the support of its social network

    Pollen morphology of 25 species in the family Apocynaceae from the Adolpho Ducke Forest Reserve, Amazonas, Brazil

    The Apocynaceae family consists of approximately 4555 species worldwide, distributed among 415 genera. In Brazil, this family is represented by 90 genera and 850 species. The Adolpho Ducke Forest Reserve, part of the Instituto Nacional de Pesquisas da Amazônia (INPA) since 1963, comprises 100 km2 of non-isolated continuous primary forest, and 40 species and 16 genera of Apocynaceae have been recorded in this area. Pollen grains, collected from flower buds and/or flowers of voucher specimens deposited at the INPA Herbarium collection, were processed using the acetolysis method, and measured, described and photographed by light microscopy and scanning electron microscopy. The study included 25 Apocynaceae species distributed among 14 genera. The pollen grains were porate to colporate, with exine ornamentation varying from psilate or scabrate to microreticulate. Other morphological characteristics were pores with granules at the base of the annulus in Odontadenia, a distinct margo forming arches around the colpus in Geissospermum, and the presence of a thick endocingulum in the equatorial region in species of Tabernaemontana. © 2016 AASP–The Palynological Society

    LAPTM4B facilitates late endosomal ceramide export to control cell death pathways

    Lysosome-associated protein transmembrane-4b (LAPTM4B) associates with poor prognosis in several cancers, but its physiological function is not well understood. Here we use novel ceramide probes to provide evidence that LAPTM4B interacts with ceramide and facilitates its removal from late endosomal organelles (LEs). This lowers LE ceramide in parallel with and independent of acid ceramidase–dependent catabolism. In LAPTM4B-silenced cells, LE sphingolipid accumulation is accompanied by lysosomal membrane destabilization. However, these cells resist ceramide-driven caspase-3 activation and apoptosis induced by chemotherapeutic agents or gene silencing. Conversely, LAPTM4B overexpression reduces LE ceramide and stabilizes lysosomes but sensitizes to drug-induced caspase-3 activation. Together, these data uncover a cellular ceramide export route from LEs and identify LAPTM4B as its regulator. By compartmentalizing ceramide, LAPTM4B controls key sphingolipid-mediated cell death mechanisms and emerges as a candidate for sphingolipid-targeting cancer therapies

    Global Retinoblastoma Presentation and Analysis by National Income Level.

    Early diagnosis of retinoblastoma, the most common intraocular cancer, can save both a child's life and vision. However, anecdotal evidence suggests that many children across the world are diagnosed late. To our knowledge, the clinical presentation of retinoblastoma has never been assessed on a global scale. To report the retinoblastoma stage at diagnosis in patients across the world during a single year, to investigate associations between clinical variables and national income level, and to investigate risk factors for advanced disease at diagnosis. A total of 278 retinoblastoma treatment centers were recruited from June 2017 through December 2018 to participate in a cross-sectional analysis of treatment-naive patients with retinoblastoma who were diagnosed in 2017. Age at presentation, proportion of familial history of retinoblastoma, and tumor stage and metastasis. The cohort included 4351 new patients from 153 countries; the median age at diagnosis was 30.5 (interquartile range, 18.3-45.9) months, and 1976 patients (45.4%) were female. Most patients (n = 3685 [84.7%]) were from low- and middle-income countries (LMICs). Globally, the most common indication for referral was leukocoria (n = 2638 [62.8%]), followed by strabismus (n = 429 [10.2%]) and proptosis (n = 309 [7.4%]). Patients from high-income countries (HICs) were diagnosed at a median age of 14.1 months, with 656 of 666 (98.5%) patients having intraocular retinoblastoma and 2 (0.3%) having metastasis. Patients from low-income countries were diagnosed at a median age of 30.5 months, with 256 of 521 (49.1%) having extraocular retinoblastoma and 94 of 498 (18.9%) having metastasis. Lower national income level was associated with older presentation age, higher proportion of locally advanced disease and distant metastasis, and smaller proportion of familial history of retinoblastoma. Advanced disease at diagnosis was more common in LMICs even after adjusting for age (odds ratio for low-income countries vs upper-middle-income countries and HICs, 17.92 [95% CI, 12.94-24.80], and for lower-middle-income countries vs upper-middle-income countries and HICs, 5.74 [95% CI, 4.30-7.68]). This study is estimated to have included more than half of all new retinoblastoma cases worldwide in 2017. Children from LMICs, where the main global retinoblastoma burden lies, presented at an older age with more advanced disease and demonstrated a smaller proportion of familial history of retinoblastoma, likely because many do not reach a childbearing age. Given that retinoblastoma is curable, these data are concerning and mandate intervention at national and international levels. Further studies are needed to investigate factors, other than age at presentation, that may be associated with advanced disease in LMICs
