    Basic Clinical Characteristics and Hospital Outcomes of Acute Coronary Syndrome Patients-Sudan

    Background: There are Variation in the presentation of the acute coronary syndrome between countries. The present study aimed to investigate the basic clinical characteristics and in-hospital outcomes among acute coronary syndrome patients in the Sudan.Material and Methods: A cross-sectional comparative study conducted among 202 consecutive acute coronary syndrome patients at a reference coronary care unit in Omdurman Teaching Hospital between July 2014 and August 2015. Participants signed a written informed consent, and then a case report form was filled. Information collected include vascular risk factors, vital signs, echocardiographic findings, and in Hospital complications. The local ethical committee approved the research, and the chi-square test was used to compare ST-segment Elevation (STSEACS) and None ST-Segment Elevation Acute coronary syndrome (NSTSEACS).Results: (out of 202 women (53.75%) in (NSEACS) P =0.009). Prior myocardial infarction, hypertension, diabetes mellitus, and, smoking were evident in 19.8%. 53.%, 30.2%, and 16.6% of acute coronary syndrome respectively, 97% of patients presented with chest pain, 54% presented to the hospital after 24 hours. Hypotension, heart failure, low ejection fraction, and in-hospital complications were more in (STSEACS) than (NSTSEACS), while (NSTEACS) patients received less Thrombolysis and Percutaneous coronary angioplasty P-value < 0.05Conclusions: Acute coronary syndrome patients were younger and had more complications than others in the West. ST-Segment Elevation Myocardial Infarction Patients are more likely to develop in-hospital complications and to receive reperfusion therapy. The limitation of the study is the lack of follow-up information after discharge

    Morphology of Intestinal Goblet Cells of The Dromedary (Camelus dromedarius)

    Morphology of the goblet cells in the intestine of the dromedary (Camelus dromedarius ) was carried out by light and electron microscopes. Histologically, the goblet cells appeared as globular or ovoidshaped with basally located flat nuclei. They were dispersed among the columnar cells lining the epithelia of the villi and crypts of Lieberkuhn, and they increased greatly in number towards the rectum. Goblet cells showed positive reaction to (PAS) stain. Ultrastructurally, the scanning electron microscopy showed villi with rough surface which displayed goblet cells and microvilli in the small intestine. In the large intestine, the surface of the mucosal fold was characteristically covered by goblet cells and epithelial cells. The ultrastructure of the goblet cells showed the cell membrane lacked microvilli and the cytoplasm contained numerous large spherical granules of mucin, rough endoplasmic reticulum and mitochondria.     Long term functioning in early onset psychosis: Two years prospective follow-up study

    <p>Abstract</p> <p>Background</p> <p>There were few studies on the outcome of schizophrenia in developing countries. Whether the outcome is similar to or different from developed world is still a point for research. The main aim of the current study was to know if patients with early onset non affective psychosis can behave and function properly after few years from start of the illness or not. Other aims included investigation of possible predictors and associated factors with remission and outcome.</p> <p>Method</p> <p>The study prospectively investigated a group of 56 patients with onset of psychosis during childhood or adolescence. Diagnosis made according to DSM-IV criteria and included; schizophrenia, psychotic disorder not otherwise specified and acute psychosis. Severity of psychosis was measured by PANSS. Measures of the outcome included; remission criteria of Andreasen et al 2005, the children's global assessment scale and educational level.</p> <p>Results</p> <p>Analysis of data was done for only 37 patients. Thirty patients diagnosed as schizophrenia and 7 with Psychotic disorder not otherwise specified. Mean duration of follow up was 38.4 +/- 16.9 months. At the end of the study, 6 patients (16.2%) had one episode, 23(62.1%) had multiple episodes and 8 (21.6%) continuous course. Nineteen patients (51.4%) achieved full remission, and only 11(29.7%) achieved their average educational level for their age. Twenty seven percent of the sample had good outcome and 24.3% had poor outcome. Factors associated with non remission and poor outcome included gradual onset, low IQ, poor premorbid adjustment, negative symptoms at onset of the illness and poor adherence to drugs. Moreover, there was tendency of negative symptoms at illness start to predict poor outcome.</p> <p>Conclusion</p> <p>Some patients with early onset non affective psychosis can behave and function properly after few years from the start of the illness. Although remission is a difficult target in childhood psychosis, it is still achievable.</p

    Effects of benzylaminopurine and naphthalene acetic acid on proliferation and shoot growth of pineapple (Ananas comosus L. Merr) in vitro

    This study was conducted to evaluate the pineapple regeneration and shoot growth as affected by 6- benzylaminopurine (BAP) at 2.0 mg/l and naphthalene acetic acid (NAA) at 0.2 mg/l in vitro. BAP and NAA at the concentration of 2.0 and 0.2 mg/l were used in this study. BAP at 2.0 mg/l significantly affected the production of shoots per explant, shoot length and weight. Total shoot length was higher in BAP (2 mg/l) than in control (MS medium without hormone) and NAA (0.2mg/l) after 10, 20, 30, 40, 50 and 60 days incubation period. Total shoot length was highest in BAP in all incubation periods. Total shoot weight was higher in BAP (2 mg/l) and lower in NAA (0.2 mg/l) as compared to MS medium without hormone. The results showed that BAP at the concentration of 2 mg/l was effective for pineapple shoot growth and development.Key words: Pineapple regeneration, shoot growth, 6-benzylaminopurine, naphthalene acetic acid

    Gastrointestinal Basidiobolomycosis Mimicking Colon Cancer in a Sudanese Patient

    Basidiobolomycosisis a rare disease caused by the fungus Basidiobolus ranarum. The incidence Rate of Gastrointestinal Basidiobolomycosis is approximately 1 in 45,333,334 or 0.00% in every 5 people in USA), member of the class Zygomycetes found worldwide (1).Basidiobolomycosis is usually a subcutaneous infection but rarely gastrointestinal1. This fungus is found mainly in the soil and on decaying vegetations2. It has been isolated from the banks of tropical rivers in West Africa, and has also been found in association with some insects2. The fungus is known to be present in the gastrointestinal tracts of reptiles, amphibians, and some bat species3. Definitive diagnosis requires culture and serological testing may be helpful. The fungal morphology and the Splendore-Hoeppli phenomenon are characteristic histological features. Basidiobolomycosis is treated with surgical resection and itracanzole 200mg BD for three months or Amphotericin B 5mg /kg iv/24 hrs4.Keywords: Zygomycetes, vegetations, subcutaneous infection.

    Production of actinium-225 from a (n,p) reaction: Feasibility and pre-design studies

    Actinium-225 is used in nuclear medicine for the treatment of malignant tumours. It can be applied to produce Bi-213 in a reusable generator or can be used alone as an agent for radiation therapy, in particular for targeted alpha therapy. However, the availability of Ac-225 for worldwide use, particularly in low- and middle-income countries, is limited. We present a feasibility study employing GATE, an open-source Monte Carlo simulation toolkit, on the production of Ac-225 from a neutron generator. This work suggests that a design consisting of three concentric cylinders, the innermost a Cf-252 neutron source, the middle nickel cylinder acting as a proton-producing target and the outer cylinder a RaCl2 target may provide a feasible design outline for an Ac-225 generator
