    Nursing workload and occurrence of adverse events in intensive care: a systematic review

    Abstract OBJECTIVE To identifyevidences of the influence of nursing workload on the occurrence of adverse events (AE) in adult patients admitted to the intensive care unit (ICU). METHOD A systematic literature review was conducted in the databases MEDLINE, CINAHL, LILACS, SciELO, BDENF, and Cochrane from studies in English, Portuguese, or Spanish, published by 2015. The analyzed AE were infection, pressure ulcer (PU), patient falls, and medication errors. RESULTS Of 594 potential studies, eight comprised the final sample of the review. TheNursing Activities Score (NAS; 37.5%) and the Therapeutic Intervention Scoring System(TISS; 37.5%) were the instruments most frequently used for assessing nursing workload. Six studies (75.0%) identified the influence of work overload in events of infection, PU, and medicationerrors. An investigation found that the NAS was a protective factor for PU. CONCLUSION The nursing workload required by patients in the ICU influenced the occurrence of AE, and nurses must monitor this variable daily to ensure proper sizing of staff and safety of care

    Psychic workloads and strain processes in nursing workers of brazilian university hospitals

    Objective: to identify the work process, the psychic workloads and the strains generated in nursing workers. Method: the study was developed in five Brazilian university hospitals. The sample was composed by 62 nursing workers and the data collection was done by focal group technique, followed by the application of a collective inquire. The data were quantitatively described and systematized according to a thematic analysis. Results: the study population represented 35,37% of the hospital workforce in national setting. The workers mentioned are exposure to several kinds of psychic workload and link them to with others workloads, highlighting the strain processes resulting from this kind of workload, such stress, fatigue, complaints of gastritis and headaches. Conclusions: nursing workers are exposed to several workloads in the hospital environment, especially psychic workloads. This exposure starts strain processes that compromise the health and quality of life, signaling the necessity of interventions in this reality.El objetivo de este estudio fue identificar el proceso de trabajo, las cargas psíquicas y los desgastes generados en trabajadores de enfermería. Se trata de un estudio desarrollado en cinco hospitales universitarios brasileños. La muestra fue compuesta por 62 trabajadores de enfermería; la recolección fue realizada por medio de la técnica de grupo focal, seguida de la aplicación de encuesta colectiva. Los datos fueron descritos numéricamente y sistematizados según análisis temático. La población de estudio representó 35,37% de la fuerza de trabajo hospitalario en el escenario nacional. Los trabajadores refirieron exposición a diversos tipos de cargas psíquicas y las relacionaron con otras cargas de trabajo, destacando los desgastes provenientes de este tipo de carga, como estrés, fatiga, gastritis y cefalea. Los trabajadores de enfermería están expuestos a las diversas cargas de trabajo en el ambiente hospitalario, especialmente a las cargas psíquicas.     “In Sickness and in Health”? Disclosures of Genetic Risks in Dating

    Individuals who have, or are at risk for, various genetic disorders face many challenges concerning disclosures of genetic information in dating situations. We conducted a qualitative interview study of 64 individuals confronting Huntington's disease, breast cancer, or Alpha‐1 antitrypsin deficiency, examining what issues these individuals encountered, and how they viewed and addressed these—including issues of understandings, privacy, and disclosures of genetic information to various groups (e.g., family members). Incidental to the primary research questions addressed, participants also often described a series of dilemmas in dating situations that they and/or family members, friends, and fellow patients faced of whether to date, and if so, whether, what, how, why, and when to disclose their genetic risk or illness. At times, these individuals feared and experienced rejection, and hence delayed, avoided, or opposed disclosure, or disclosed indirectly or inadvertently. These data are reported in this paper and highlight the importance of patients, their loved ones, genetic counselors, and other health care providers being aware of these issues, and appreciating the complex factors involved, which can affect patients’ coping and social support. This paper, the first to explore several key aspects of disclosures of genetic information in dating, thus suggests needs for public and professional education, and future research in this area
