    Diagnostic accuracy of positron emission tomography tracers for the differentiation of tumor progression from treatment-related changes in high-grade glioma:a systematic review and meta-analysis

    Background: Post-treatment high-grade gliomas are usually monitored with contrast-enhanced MRI, but its diagnostic accuracy is limited as it cannot adequately distinguish between true tumor progression and treatment-related changes. According to recent response assessment in neuro-oncology (RANO) recommendations PET overcomes this limitation. However, it is currently unknown which tracer yields the best results. Therefore, a systematic review and meta-analysis were performed to compare the diagnostic accuracy of the different PET tracers in differentiating tumor progression from treatment-related changes in high-grade glioma patients. Methods: Pubmed, Web of Science and Embase were searched systematically. Study selection, data extraction and quality assessment were performed independently by two authors. Meta-analysis was performed using a bivariate random effects model when ≥ 5 studies were included. Results: 39 studies (11 tracers) were included in the systematic review. 18F-FDG (12 studies, 171 lesions) showed a pooled sensitivity and specificity of 84% (95%CI 72-92) and 84% (69-93), respectively. 18F-FET (7 studies, 172 lesions) demonstrated a sensitivity of 90% (81-95) and specificity of 85% (71-93). 11C-MET (8 studies, 151 lesions) sensitivity was 93% (80-98) and specificity was 82% (68-91). The number of included studies for the other tracers were too low to combine, but sensitivity and specificity ranged between 93-100% and 0-100% for 18F-FLT, 85-100% and 72-100% for 18F-FDOPA and 100% and 70-88% for 11C-CHO, respectively. Conclusion:18F-FET and 11C-MET, both amino-acid tracers, showed a comparable higher sensitivity than 18F-FDG in the differentiation between tumor progression and treatment-related changes in high-grade glioma patients. The evidence for other tracers is limited, thus 18F-FET and 11C-MET are preferred when available. Our results support the incorporation of amino-acid PET tracers for the treatment evaluation of high-grade gliomas

    Mid-infrared diagnostics of starburst galaxies: clumpy, dense structures in star-forming regions in the Antennae (NGC 4038/4039)

    Recently, mid-infrared instruments have become available on several large ground-based telescopes, resulting in data sets with unprecedented spatial resolution at these long wavelengths. In this paper we examine 'ground-based-only' diagnostics, which can be used in the study of star-forming regions in starburst galaxies. By combining output from the stellar population synthesis code Starburst 99 with the photoionization code Mappings, we model stellar clusters and their surrounding interstellar medium, focusing on the evolution of emission lines in the N- and Q-band atmospheric windows (8-13 and 16.5-24.5 micron respectively) and those in the near-infrared. We address the detailed sensitivity of various emission line diagnostics to stellar population age, metallicity, nebular density, and ionization parameter. Using our model results, we analyze observations of two stellar clusters in the overlap region of the Antennae galaxies obtained with VLT Imager and Spectrometer for mid Infrared (VISIR). We find evidence for clumpy, high density, ionized gas. The two clusters are young (younger than 2.5 and 3 Myr respectively), the surrounding interstellar matter is dense (10^4 cm^-3 or larger) and can be characterized by a high ionization parameter (logU > -1.53). Detailed analysis of the mid-infrared spectral features shows that a (near-)homogeneous medium cannot account for the observations, and that complex structure on scales below the resolution limit, containing several young stellar clusters embedded in clumpy gas, is more likely.Comment: 24 pages, 16 figures (3 in color), accepted for publication in Ap

    Halo millisecond pulsars ejected by intermediate mass black holes in globular clusters

    Intermediate mass black holes (IMBHs) are among the most elusive objects in contemporary astrophysics. Both theoretical and observational evidence of their existence is subject of debate. Conversely, both theory and observations confirm the presence of a large population of millisecond pulsars (MSPs) with low mass companions residing in globular cluster (GC) centers. If IMBHs are common in GC centers as well, then dynamical interactions will inevitably break up many of these binaries, causing the ejection of several fast MSPs in the Galactic halo. Such population of fast halo MSPs, hard to produce with 'standard' MSP generation mechanisms, would provide a strong, albeit indirect, evidence of the presence of a substantial population of IMBHs in GCs. In this paper we study in detail the dynamical formation and evolution of such fast MSPs population, highlighting the relevant observational properties and assessing detection prospects with forthcoming radio surveys.Comment: 13 pages, 7 figures, 2 tables, accepted for publication in MNRA

    CT-measured skeletal muscle mass used to assess frailty in patients with head and neck cancer

    BACKGROUND: Skeletal muscle depletion or sarcopenia is related to multiple adverse clinical outcome. However, frailty questionnaires are currently applied in the daily practice to identify patients who are potentially (un)suitable for treatment but are time consuming and straining for patients and the clinician. Screening for sarcopenia in patients with head and neck cancer (HNC) could be a promising fast biomarker for frailty. Our objective was to quantify sarcopenia with pre-treatment low skeletal muscle mass from routinely obtained neck computed tomography scans at level of third cervical vertebra in patients diagnosed with HNC and evaluate its association with frailty. METHODS: A total of 112 HNC patients with Stages III and IV disease were included from a prospective databiobank. The amount of skeletal muscle mass was retrospectively defined using the skeletal muscle index (SMI). Correlation analysis between SMI and continuous frailty data and the observer agreement were analysed with Pearson's r correlation coefficients. Sarcopenia was present when SMI felt below previously published non-gender specific thresholds (<43.2 cm2 /m2 ). Frailty was evaluated by Geriatrics 8 (G8), Groningen Frailty Indicator, Timed Up and Go test, and Malnutrition Universal Screening Tool. A univariate and multivariate logistic regression analysis was performed for all patients and men separately to obtain odds ratios (ORs) and 95% confidence intervals (95% CIs). RESULTS: The cohort included 82 men (73%) and 30 women (27%), with a total mean age of 63 (±9) years. The observer agreement for cross-sectional measurements was excellent for both intra-observer variability (r = 0.99, P < 0.001) and inter-observer variability (r = 0.98, P < 0.001). SMI correlated best with G8 frailty score (r = 0.38, P < 0.001) and did not differ per gender. Sarcopenia was present in 54 (48%) patients, whereof 25 (46%) men and 29 (54%) women. Prevalence of frailty was between 5% and 54% depending on the used screening tool. The multivariate regression analysis for all patients and men separately isolated the G8 questionnaire as the only independent variable associated with sarcopenia (OR 0.76, 95% CI 0.66-0.89, P < 0.001 and OR 0.76, 95% CI 0.66-0.88, P < 0.001, respectively). CONCLUSIONS: This is the first study that demonstrates that sarcopenia is independently associated with frailty based on the G8 questionnaire in HNC patients. These results suggest that in the future, screening for sarcopenia on routinely obtained neck computed tomography scans may replace time consuming frailty questionnaires and help to select the (un)suitable patients for therapy, which is highly clinically relevant
