    Development of non-destructive methodology using ATR-FTIR with PCA to differentiate between historical Pacific barkcloth

    Barkcloths, non-woven textiles originating from the Pacific Islands, form part of many museum collections and date back to the 18th and 19th centuries. The ability to determine different plant species which have been used for producing barkcloth is required by art historians to help understand the origin and use of the cloths and by conservators for whom the species type may have an impact on textile durability, deterioration and hence conservation. However, to date the development of a non-destructive, robust analytical technique has been elusive. This article describes the use of Fourier transform infrared spectroscopy with attenuated total reflection (ATR-FTIR) and principal component analysis (PCA) todifferentiation between historic barkcloths. Three distinct groups of historic cloths were identified using PCA of the FTIR region between 1200 and 1600 cm−1 where molecular vibrations associated with tannins and lignins are dominant. Analysis of contemporary cloths only identified Pipturus albidus cloth as different and highlighted the difficulties around producing a representative textile sample to mimic the historic cloths. While the methodology does not itself identify species, the use of historically well-provenanced samples allows cloths showing similarities to group together and is a significant aid to identification

    Educational weight loss interventions in obese and overweight adults with type 2 diabetes : a systematic review and meta‐analysis of randomized controlled trials

    Aim The worldwide prevalence of type 2 diabetes mellitus is increasing, with most individuals with the disease being overweight or obese. Weight loss can reduce disease‐related morbidity and mortality and weight losses of 10–15 kg have been shown to reverse type 2 diabetes. This review aimed to determine the effectiveness of community‐based educational interventions for weight loss in type 2 diabetes. Methods This is a systematic review and meta‐analysis of randomized controlled trials (RCT) in obese or overweight adults, aged 18–75 years, with a diagnosis of type 2 diabetes. Primary outcomes were weight and/or BMI. CINAHL, MEDLINE, Embase, Scopus and the Cochrane Central Register of Controlled Trials (CENTRAL) were searched from inception to June 2019. Trials were classified into specified a priori comparisons according to intervention type. A pooled standardized mean difference (SMD) (from baseline to follow‐up) and 95% confidence intervals (95% CI) between trial groups (difference‐in‐difference) were estimated through random‐effects meta‐analyses using the inverse variance method. Heterogeneity was quantified using I2 and publication bias was explored visually using funnel plots. Results Some 7383 records were screened; 228 full‐text articles were assessed and 49 RCTs (n = 12 461 participants) were included in this review, with 44 being suitable for inclusion into the meta‐analysis. Pooled estimates of education combined with low‐calorie, low‐carbohydrate meal replacements (SMD = –2.48, 95% CI –3.59, –1.49, I2 = 98%) or diets (SMD = –1.25, 95% CI –2.11, –0.39, I2 = 95%) or low‐fat meal replacements (SMD = –1.15, 95%CI –2.05, –1.09, I2 = 85%) appeared most effective. Conclusion Low‐calorie, low‐carbohydrate meal replacements or diets combined with education appear the most promising interventions to achieve the largest weight and BMI reductions in people with type 2 diabetes

    Turning to art as a positive way of living with cancer: A qualitative study of personal motives and contextual influences

    Why do some women turn to creative art-making after a diagnosis of cancer? Eleven women provided qualitative accounts that were analyzed following guidelines for interpretative phenomenological analysis (IPA). Some described taking up artistic leisure activities initially in order to manage emotional distress. Others emphasized their need for positive well-being, taking up art to experience achievement and satisfaction, to regain a positive identity, and to normalize family dynamics in the context of living with cancer. Participants’ turn to art-making was facilitated by biographical and contextual factors, including pre-existing craft skills, long-standing personal values and coping philosophies, family role models for managing adversity, and the supportive encouragement of family and friends. Other research has acknowledged that positive lifestyle change and post-traumatic growth can occur after a cancer diagnosis, and this study reveals a multi-faceted process. The findings suggest a need for further research into the experiences that facilitate positive lifestyle change and subjective well-being among people who are living with cancer

    Healthalachia: Rural Appalachian Youth Creating Solutions for Our Future

    Kentucky’s Appalachian youth experience high rates of adverse childhood experiences. Opioid abuse, poverty and unemployment dominate our news headlines. Youth are suffering negative impacts of these issues and experiencing disproportionate rates of depression, suicidal thoughts, food insecurities, obesity and lack of physical activity. Overdose deaths and incarceration rates among parents with substance abuse disorders negatively impact family units and contribute to increased homelessness, foster care and kinship care among our youth population. Healthalachia was developed and piloted as a student-based, youth empowerment initiative that provides competitive mini-grants and research mentors to groups of high schoolers who aim to identify a health concern at their school and implement a student led improvement plan. This student-based research pilot engaged students from three rural high schools in Kentucky to identify and strategically address a specific health or social problem. Each team was required to have students, school staff and community members who agreed to actively participate in the project. The initiative was a partnership between University of Kentucky Center of Excellence in Rural Health (UK CERH) and the Kentucky Valley Educational Cooperative (KVEC). Student teams were provided with funding for their project and technical support as they implemented their projects

    On the String Consensus Problem and the Manhattan Sequence Consensus Problem

    In the Manhattan Sequence Consensus problem (MSC problem) we are given kk integer sequences, each of length ll, and we are to find an integer sequence xx of length ll (called a consensus sequence), such that the maximum Manhattan distance of xx from each of the input sequences is minimized. For binary sequences Manhattan distance coincides with Hamming distance, hence in this case the string consensus problem (also called string center problem or closest string problem) is a special case of MSC. Our main result is a practically efficient O(l)O(l)-time algorithm solving MSC for k≀5k\le 5 sequences. Practicality of our algorithms has been verified experimentally. It improves upon the quadratic algorithm by Amir et al.\ (SPIRE 2012) for string consensus problem for k=5k=5 binary strings. Similarly as in Amir's algorithm we use a column-based framework. We replace the implied general integer linear programming by its easy special cases, due to combinatorial properties of the MSC for k≀5k\le 5. We also show that for a general parameter kk any instance can be reduced in linear time to a kernel of size k!k!, so the problem is fixed-parameter tractable. Nevertheless, for k≄4k\ge 4 this is still too large for any naive solution to be feasible in practice.Comment: accepted to SPIRE 201

    Kentucky’s Primary Care Workforce: Current Status and Output of New Trainees January 2020 Update

    Primary care is the backbone of American healthcare. Primary care (PC) physicians are especially cost-effective in preventing and managing illness and disability, and prolonging years of quality life. They also contribute significantly to the economy of their communities. Primary care shortages across the Commonwealth are severe. Kentucky ranks 43rd in the USA for its number of PC physicians per 100,000 people. The Commonwealth would have to add 246 primary care physicians annually to reach the US median by the end of 2029. Kentucky produces and retains only about 55 new PC physicians per year, recruiting 55 to 60 more from out of state. This total of 110 – 115 new physicians falls short of the 124 PC physicians that must be added annually to avoid worsening our shortage, and far below the 246 PC physicians that Kentucky would need to add each year to reach the US median in the coming decade
