    The timing of maternal depressive symptoms and child cognitive development: a longitudinal study.

    Background: Maternal depression is known to be associated with impairments in child cognitive development, although the effect of timing of exposure to maternal depression is unclear. Methods: Data collected for the Avon Longitudinal Study of Parents and Children, a longitudinal study beginning in pregnancy, included self-report measures of maternal depression the Edinburgh Postnatal Depression Scale, completed on 6 occasions up to 3 years of age, and IQ of the index child (WISC) measured at aged 8 years. We used these data to assign women to 8 groups according to whether depression occurred in the antenatal, postnatal, preschool period, any combination of these times, or not at all. We compared a model comprising all patterns of depression (saturated model) with models nested within this to test whether there is a relationship between depression and child cognitive development and, if so, whether there is a sensitive period. We then investigated the relationship with child IQ for each model, following adjustment for confounders. Results: Six thousand seven hundred and thirty-five of 13,615 children from singleton births (49.5%, of eligible core sample) attended a research clinic at 8 years and completed a WISC with a score ≥ 70. A total of 5,029 mothers of these children had completed mood assessments over the 3 time periods. In unadjusted analyses, all three sensitive period models were as good as the saturated model, as was an accumulation model. Of the sensitive period models, only that for antenatal exposure was a consistently better fit than the accumulation model. After multiple imputation for missing data (to n = 6,735), there was no effect of postnatal depression on child IQ independent of depression at other times [-0.19 IQ points, 95% confidence interval (CI) -1.5 to 1.1 points]. There was an effect of antenatal depression (-3.19 IQ points, 95% CI: -4.33 to -2.06) which attenuated following adjustment (-0.64 IQ points, 95% CI: -1.68 to 0.40). Conclusions: The postnatal period is not a sensitive one for the effect of maternal depression on child cognitive development. © 2011 The Authors. Journal of Child Psychology and Psychiatry

    Largemouth bass (Micropterus salmoides Lacépède): results of farming trials

    In the last years in the United States, the demand of largemouth bass (Micropterus salmoides) larger than the size usually produced for sport fishing purposes has increased. The growth phase is carried on in ponds, after having evaluated different strains (Williamson and Carmichael, 1990) using feeds for salmonids (Tidwell et al., 1996) supplemented with amino acids (Coyle et al., 2000). Also in Italy, the market currently requires largemouth bass of 300-500 g. This situation has induced one farmer to cooperate with our research centre and carried out trials to verify the possibility of rearing this fish, from the fingerling stage up to the size suitable for food, using the same farming techniques as those currently being applied in the United States (Davis and Lock, 1997)

    The challenge of perioperative pain management in opioid-tolerant patients

    The increasing number of opioid users among chronic pain patients, and opioid abusers among the general population, makes perioperative pain management challenging for health care professionals. Anesthesiologists, surgeons, and nurses should be familiar with some pharmacological phenomena which are typical of opioid users and abusers, such as tolerance, physical dependence, hyperalgesia, and addiction. Inadequate pain management is very common in these patients, due to common prejudices and fears. The target of preoperative evaluation is to identify comorbidities and risk factors and recognize signs and symptoms of opioid abuse and opioid withdrawal. Clinicians are encouraged to plan perioperative pain medications and to refer these patients to psychiatrists and addiction specialists for their evaluation. The aim of this review was to give practical suggestions for perioperative management of surgical opioid-tolerant patients, together with schemes of opioid conversion for chronic pain patients assuming oral or transdermal opioids, and patients under maintenance programs with methadone, buprenorphine, or naltrexone

    Natural and synthetic pigments used in the pink-red coloration of salmon flesh: methodology of quali-quantitative assessments and sampling results

    In Europe Atlantic salmon leads the first position in the farmed species contest with more than 763,000t (FES, 2007). In the year 2006, Norway produced alone beyond 603,000t followed by the United Kingdom with 128,000t. In these last years, the demand of organic salmon is gradually increased both in domestic and foreign markets. In these fish the pink-red coloration of flesh is obtained by supplementing the feed with shrimp waste meal. In Italy, Coop Italia since three years has started the commercialization in the context of "prodotti a marchio" of Atlantic salmon farmed following prescriptions reported in terms of specification that prohibits the use of synthetic and GM pigments in the feeds. In collaboration with this supermarket company, the Faculty of Veterinary Medicine of Camerino University carried out the quali-quantitative assessments on the presence of molecules and relative isomers that distinguish the natural carotenoid pigment from the synthetic one. Skinned portion of fillet (10g) were collected from dorsal muscle (retro-cranial, central and caudal region), mixed and homogenized and repeatedly extracted with acetone until they were colourless. The pooled extract were filtered and an aliquot (10ml) was centrifugated (2200xg, 5min). The astaxanthin content in the supernatant was determined by HPLC using a Varian ProStar instrument equipped with UV/vis detector using an external astaxanthin standard at detection wavelength 470nm. Analysis was performed on a Varian Kromasil 100 C 18 250x0.3 mm according to Bjerkeng et al. (1997). Identification and determination of stereoisomers were carried out by means of a Sumichiral a-phenylglicine 250x4.6mm column following Abu-Lafi and Turujman method (1999). The analysis performed on all the organic fish have demonstrated the exclusive content of natural pigment. In this group (b.w.=3.9-4.4kg), C-Card for salmonids ranged between 26±1 in 2004 and 21.5±1.3 in 2005 and 21.5±2.1 in 2006. Astaxanthin and isomers decreased from 5.6±0.3mg/kg in 2004 to 2.9±1.1mg/kg in 2006. The low-cost non organic salmon group (b.w.=4.5-4.6kg) resulted pigmented only with synthetic carotenoids and C-card for salmonids ranged between 27.3 in 2004 and 23.7 in 2005 whereas in 2006 it was observed equal to 27. Also in this batch, astaxanthin and isomers decreased passing from 6.5mg/kg in 2004 to 4.9mg/kg in 2006

    The Occipital Nerves Applied Strain Test to Support Occipital Neuralgia Diagnosis

    Introduction: Occipital neuralgia (ON) is a disabling cephalalgia form with demanding diagnostic workflow. We report the description and reliability analyses of the occipital nerves applied strain (ONAS) test for occipital neuralgia (ON) early-stage diagnosis in cephalalgia patients. Methods: In a retrospective and observational study, we evaluated, among n = 163 consecutive cephalalgia patients, the sensitivity, specificity, and prior probability [positive (PPV) and negative (NPV) predictive values] of the ONAS test against two reference tests (occipital nerve anesthetic block and the painDETECT questionnaire). Multinomial logistic regression (MLR) and v2 analyses verified the ONAS test outcome’s dependence upon independent variables (gender, age, pain site, block test, and painDETECT outcomes). We assessed inter-rater agreement with Cohen’s kappa statistic. Results: ONAS test showed sensitivity and specificity of 81 and 18%, respectively, against the painDETECT and of 94 and 46%, respectively, against the block test. PPV was >70% against both tests, while NPV was 81% against the block test and 26% against the painDETECT. Interrater agreement Cohen’s kappa was excellent. Significant association (X2 analyses) and relationship (MLR) were found only between ONAS test and pain site but not with the other independent predictors. Conclusions: The ONAS test showed satisfactory reliability among cephalalgia patients; thus, it might be considered a valuable early stage tool for ON diagnosis in these patients.PLAIN LANGUAGE SUMMARY We report the description and reliability features of an occipital neuralgia diagnostic tool. The latter is based on the assertion that applying a strain on putatively compromised occipital nerves prompts abnormal nerve discharges and subjective pain reactions and thus may reveal occipital neuralgia. Among 163 cephalalgia patients, the test showed sensitivity and specificity of 81 and 18%, respectively, against the painDETECT questionnaire and 94 and 46%, respectively, against the occipital nerves’ block test. Interrater agreement was excellent, and significant associations and relationships were found only between the tool and congruent pain site but not with the other independent predictors. This tool may help clinicians’ early detection of occipital neuralgia in cephalalgia patients

    Effects of dietary supplementation with krill meal on pigmentation and quality of flesh of rainbow trout (Oncorhynchus mykiss)

    Effects of administration of krill meal and synthetic astaxanthin during the finisher phase of the fattening cycle of rainbow trout on flesh pigmentation and quality traits were studied. The inclusion of krill meal increased the body weight and size and decreased the peri-visceral fat and visceral weight indices. The astaxanthin diet produced the highest accumulation of total carotenoids in the fillet compared to the krill meal diet: the difference was significant after 15 days of feeding (2.50 vs 2.10 mg/kg) till the end of the trial (5.00 vs 4.80 mg/kg). The same pattern was observed for astaxanthin concentration with the highest values in the fillets of fish fed the astaxanthin diet. Fillet lightness (L*) was not affected by trout diets whereas redness (a*) and yellowness (b*) were significantly higher in fish fed the astaxanthin diet until day 30 of the trial. Hue was not affected by feeding, whereas chroma was significantly higher in the fish fed astaxanthin throughout the trial except on day 45 of sampling. Trout fed the krill meal diet had a paler pink-red colour on the SalmoFan scale than those receiving the astaxanthin diet. No significant differences emerged in proximate composition and cholesterol content of trout in the two groups. The fatty acid profile of the fillets reflected the fatty acids of the diets administered to the trout: eicosapentaenoic, docosahexaenoic and docosapentaenoic acids and total n-3 polyunsaturated fatty acids were significantly higher in the fish fed the krill meal

    Growth performances of sunshine bass (initial m.b.w.=200\ub150 g) were examined in 3 different rearing conditions: concrete square basins (HSB-1); raceways (HSB-2); natural ponds (HSB-3). Fish received the same extruded feed (prot. 48.8%; lip. 17.8%). The trial lasted 16 months in HSB-1 and HSB-2 and 12 months in HSB-3 due to unfavourable welfare status and low growth performances. At harvest (16 months), sunshine bass reached a satisfactory mean weight without significant differences between groups. HSB-2 fish were longer with a smaller circumference, probably due to different hydrodynamics of the raceway. Glucose, cholesterol, triglycerides, protein, lactic dehydrogenase (LDH), aspartate aminotransferase (AST), alanine aminotransferase (ALT), alkaline phosphatase (ALP), creatine kinase (CK), glycogen of plasma or tissue were not affected by rearing conditions as well as the proximate composition and the fatty acid profile of the fillet

    MUDPIT ANALYSIS OF PSEUDOMONAS AERUGINOSA SECRETOME: CONSEQUENCES OF OXYGEN LIMITATION IN CLINICAL AND LABORATORY STRAINS

    This review discusses the efficacy and safety in chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) of intravenous immunoglobulin and compares this treatment with plasma exchange and prednisolone. We searched publications from 1985 onwards for randomised controlled studies examining the effects of intravenous immunoglobulin in patients with this immune-mediated neuromuscular disorder. Six trials, with 170 patients in total, were judged eligible. A significantly higher proportion of patients improved in disability within a month after the start of treatment with intravenous immunoglobulin than with placebo (relative risk 3.17 [95% CI 1.74 to 5.75]). During this period, intravenous immunoglobulin has similar efficacy to plasma exchange and oral prednisolone; therefore which of these treatments should be the first choice is currently uncertain.
