    O combate à desertificação nas cumeadas do baixo Guadiana

O presente estudo começa por abordar essa evolução, tentando perceber de que forma a vida rural evoluiu, quais os fatores humanos e físicos que intervieram sobre a mesma – o êxodo rural progressivo e a evolução da tecnologia – e quais as consequências que os mesmos trouxeram ao desenvolvimento do território. Uma vez contextualizada a realidade que afeta as regiões rurais, este estudo irá debruçar-se sobre uma delas - a zona da Serra do Caldeirão, onde está inserido o caso de estudo. Procurar-se-á identificar as transformações físicas e culturais que a mesma sofreu, as suas condições edafo-climáticas e os aspetos socioeconómicos, que conduziram a um processo de desertificação que tornou esta região uma região “altamente suscetível à desertificação”. Tentar-se-á portanto perceber, numa primeira fase, quais as razões que levaram ao processo da desertificação e quais as consequências que a mesma provocou essencialmente nos meios rurais. Seguidamente, numa segunda fase, que será o foco desta pesquisa, iremos procurar conhecer que ações de combate à desertificação foram levadas a cabo, pelos privados e pelas entidades oficiais, e de que modo as recomendações do Plano de Ação Nacional de Combate à Desertificação (PANCD) foram tidas em conta na elaboração de projetos ou em ações-piloto desenvolvidas, designadamente na Área Piloto da Serra do Baixo Guadiana, pelo setor público ou por agentes privados

    Pulmonary versus aortic pressure behavior of a bovine pericardial valve

    Background: The Carpentier Edwards Perimount Magna Ease aortic valvular prosthesis (Edwards Lifesciences, Irvine, Calif) has been among the most frequently and successfully used tissue prosthetic cardiac valves. Furthermore, this prosthesis has been used off-label in the pulmonary position. Until now, there has been a paucity of data regarding the functioning of tissue prosthetic valves under pulmonary conditions. Methods: Using a pulse duplicator, hydrodynamic characteristics of a 21-mm and 25-mm Magna Ease valve were evaluated. Among parameters evaluated were leakage orifice area, closing time (ie, time required to close), and leakage duration. This procedure was performed under different pulmonic pressure conditions (15/5 mm Hg, 28/11 mm Hg, 73/32 mm Hg) and normal aortic pressure (120/80 mm Hg) as a reference. Moving images were obtained using a Phantom MIRO M320S high-speed camera (Vision Research Inc, Wayne, NJ) at 600 frames per second and used to analyze valve area in closed position. Results: Under normal pulmonic conditions (28/11 mm Hg) the leakage orifice area was 0.020 ± 0.012 mm2 for the 21-mm valve and 0.054 ± 0.041 mm2 for the 25-mm valve (P = .03). Hydrodynamic characteristics of the valves differed between pulmonary and aortic testing condition. Valve closing volumes were significantly lower under pulmonary hypotension and normal pulmonary conditions than under normal aortic conditions (P < .05). Conclusions: Under normal pulmonary pressure conditions, the hydrodynamic characteristics of Magna Ease valves are significantly different compared with aortic conditions. Further research is needed to determine whether these results are associated with prosthetic valve failure

    On `maximal' poles of zeta functions, roots of b-functions and monodromy Jordan blocks

    The main objects of study in this paper are the poles of several local zeta functions: the Igusa, topological and motivic zeta function associated to a polynomial or (germ of) holomorphic function in n variables. We are interested in poles of maximal possible order n. In all known cases (curves, non-degenerate polynomials) there is at most one pole of maximal order n which is then given by the log canonical threshold of the function at the corresponding singular point. For an isolated singular point we prove that if the log canonical threshold yields a pole of order n of the corresponding (local) zeta function, then it induces a root of the Bernstein-Sato polynomial of the given function of multiplicity n (proving one of the cases of the strongest form of a conjecture of Igusa-Denef-Loeser). For an arbitrary singular point we show under the same assumption that the monodromy eigenvalue induced by the pole has a Jordan block of size n on the (perverse) complex of nearby cycles.Comment: 8 pages, to be published in Journal of Topolog

    Long-term outcome of patients with transposition of the great arteries and a systemic right ventricle:A systematic review and meta-analysis

    BACKGROUND: Patients with a transposition of the great arteries (TGA) and a systemic right ventricle are at risk of heart failure (HF) development, arrhythmia and early mortality. Prognostic evaluations in clinical studies are hampered by small sample sizes and single-centred approaches. We aimed to investigate yearly rate of outcome and factors affecting it.METHODS: A systematic literature search of four electronic databases (PubMed, EMBASE, Web of Science and Scopus) was conducted from inception to June 2022. Studies reporting the association of a systemic right ventricle with mortality with a minimal follow-up of 2 years during adulthood were selected. Incidence of HF hospitalization and/or arrhythmia were captured as additional endpoints. For each outcome, a summary effect estimate was calculated.RESULTS: From a total of 3891 identified records, 56 studies met the selection criteria. These studies described the follow-up (on average 7.27 years) of 5358 systemic right ventricle patients. The mortality incidence was 1.3 (1-1.7) per 100 patients/year. The incidence of HF hospitalization was 2.6 (1.9-3.7) per 100 patients/year. Predictors of poor outcome were a lower left ventricular (LV) and right ventricular ejection fraction (RVEF) (standardized mean differences (SMD) of -0.43 (-0.77 to -0.09) and - 0.85 (-1.35 to -0.35), respectively), higher plasma concentrations of NT-proBNP (SMD of 1.24 (0.49-1.99)), and NYHA class ≥2 (risk ratio of 2.17 (1.40-3.35)).CONCLUSIONS: TGA patients with a systemic right ventricle have increased incidence of mortality and HF hospitalization. A lower LVEF and RVEF, higher levels of NT-proBNP and NYHA class ≥2 are associated with poor outcome.</p

    Diabetes, Glycemic Control, and New-Onset Heart Failure in Patients With Stable Coronary Artery Disease: Data from the Heart and Soul Study

    OBJECTIVE Diabetes is a predictor of both coronary artery disease (CAD) and heart failure. It is unknown to what extent the association between diabetes and heart failure is influenced by other risk factors for heart failure. RESEARCH DESIGN AND METHODS We evaluated the association of diabetes and A1C with incident heart failure in outpatients with stable CAD and no history of heart failure (average follow-up 4.1 years). RESULTS Of 839 participants, 200 had diabetes (23.8%). Compared with patients who did not have diabetes, those with diabetes had an increased risk of heart failure (hazard ratio [HR] 2.17 [95% CI 1.37-3.44]). Adjustment for risk factors for CAD (age, sex, race, smoking, physical inactivity, obesity, blood pressure, and LDL cholesterol), interim myocardial infarction, and myocardial ischemia did not alter the strength of the association between diabetes and heart failure. After inclusion also of other risk factors for heart failure (left ventricular ejection fraction, diastolic dysfunction, and C-reactive protein) and medication use, diabetes remained an independent predictor of heart failure (HR 3.34 [95% CI 1.65-6.76]; P = 0.001). Each 1% increase in A1C concentration was associated with a 36% increased HR of heart failure hospitalization (HR 1.36 [95% CI 1.17-1.58]). CONCLUSIONS In patients with stable CAD who are free from heart failure at baseline, diabetes and glycemic control are independent risk factors for new-onset heart failure. The mechanisms by which diabetes and hyperglycemia lead to heart failure deserve further study, as the association is independent of baseline functional assessment of ischemia, systolic and diastolic function, and interim myocardial infarction
