Cerebrovascular complications and infective endocarditis. impact of available evidence on clinical outcome
Infective endocarditis (IE) is a life-threatening disease. Its epidemiological profile has substantially changed in recent years although 1-year mortality is still high. Despite advances in medical therapy and surgical technique, there is still uncertainty on the best management and on the timing of surgical intervention. The objective of this review is to produce further insight intothe short- and long-term outcomes of patients with IE, with a focus on those presenting cerebrovascular complications
Transition in a numerical model of contact line dynamics and forced dewetting
We investigate the transition to a Landau-Levich-Derjaguin film in forced
dewetting using a quadtree adaptive solution to the Navier-Stokes equations
with surface tension. We use a discretization of the capillary forces near the
receding contact line that yields an equilibrium for a specified contact angle
called the numerical contact angle. Despite the well-known
contact line singularity, dynamic simulations can proceed without any explicit
additional numerical procedure. We investigate angles from to
and capillary numbers from to where the mesh size
is varied in the range of to of the capillary length
. To interpret the results, we use Cox's theory which involves a
microscopic distance and a microscopic angle . In the numerical
case, the equivalent of is the angle and we find
that Cox's theory also applies. We introduce the scaling factor or gauge
function so that and estimate this gauge function by
comparing our numerics to Cox's theory. The comparison provides a direct
assessment of the agreement of the numerics with Cox's theory and reveals a
critical feature of the numerical treatment of contact line dynamics: agreement
is poor at small angles while it is better at large angles. This scaling factor
is shown to depend only on and the viscosity ratio . In the
case of small , we use the prediction by Eggers [Phys. Rev. Lett.,
vol. 93, pp 094502, 2004] of the critical capillary number for the
Landau-Levich-Derjaguin forced dewetting transition. We generalize this
prediction to large and arbitrary and express the critical
capillary number as a function of and . An analogy can be drawn

Yield-related salinity tolerance traits identified in a nested association mapping (NAM) population of wild barley
Producing sufficient food for nine billion people by 2050 will be constrained by soil salinity, especially in irrigated systems. To improve crop yield, greater understanding of the genetic control of traits contributing to salinity tolerance in the field is needed. Here, we exploit natural variation in exotic germplasm by taking a genome-wide association approach to a new nested association mapping population of barley called HEB-25. The large population (1,336 genotypes) allowed cross-validation of loci, which, along with two years of phenotypic data collected from plants irrigated with fresh and saline water, improved statistical power. We dissect the genetic architecture of flowering time under high salinity and we present genes putatively affecting this trait and salinity tolerance. In addition, we identify a locus on chromosome 2H where, under saline conditions, lines homozygous for the wild allele yielded 30% more than did lines homozygous for the Barke allele. Sleep During Pregnancy: The nuMoM2b Pregnancy and Sleep Duration and Continuity Study
Study Objectives:
To characterize sleep duration, timing and continuity measures in pregnancy and their association with key demographic variables.
Methods:
Multisite prospective cohort study. Women enrolled in the nuMoM2b study (nulliparous women with a singleton gestation) were recruited at the second study visit (16-21 weeks of gestation) to participate in the Sleep Duration and Continuity substudy. Women <18 years of age or with pregestational diabetes or chronic hypertension were excluded from participation. Women wore a wrist activity monitor and completed a sleep log for 7 consecutive days. Time in bed, sleep duration, fragmentation index, sleep efficiency, wake after sleep onset, and sleep midpoint were averaged across valid primary sleep periods for each participant.
Results:
Valid data were available from 782 women with mean age of 27.3 (5.5) years. Median sleep duration was 7.4 hours. Approximately 27.9% of women had a sleep duration of 9 hours. In multivariable models including age, race/ethnicity, body mass index, insurance status, and recent smoking history, sleep duration was significantly associated with race/ethnicity and insurance status, while time in bed was only associated with insurance status. Sleep continuity measures and sleep midpoint were significantly associated with all covariates in the model, with the exception of age for fragmentation index and smoking for wake after sleep onset.
Conclusions:
Our results demonstrate the relationship between sleep and important demographic characteristics during pregnancy
A strategic framework for improving food security in Arab countries
In 2007 and the first half of 2008, a sharp rise in agricultural commodity and food prices triggered grave concerns about food security, malnutrition and increased poverty. While the threat of a prolonged food-price shock receded in the second half of 2008, many factors underlying the price volatility are likely to persist, and will require careful management if future food-price shocks are to be avoided. This paper suggests three strategies that, together, could reduce vulnerability to price shocks: (1) strengthen safety nets, improve access to family planning services, and promote education; (2) enhance domestic food production and improve rural livelihoods through increased investment in research and development to increase productivity; and (3) reduce exposure to market volatility through more efficient supply chains and better use of financial instruments to hedge risk. Onset of human preterm and term birth is related to unique inflammatory transcriptome profiles at the maternal fetal interface.
BackgroundPreterm birth is a main determinant of neonatal mortality and morbidity and a major contributor to the overall mortality and burden of disease. However, research of the preterm birth is hindered by the imprecise definition of the clinical phenotype and complexity of the molecular phenotype due to multiple pregnancy tissue types and molecular processes that may contribute to the preterm birth. Here we comprehensively evaluate the mRNA transcriptome that characterizes preterm and term labor in tissues comprising the pregnancy using precisely phenotyped samples. The four complementary phenotypes together provide comprehensive insight into preterm and term parturition.MethodsSamples of maternal blood, chorion, amnion, placenta, decidua, fetal blood, and myometrium from the uterine fundus and lower segment (n = 183) were obtained during cesarean delivery from women with four complementary phenotypes: delivering preterm with (PL) and without labor (PNL), term with (TL) and without labor (TNL). Enrolled were 35 pregnant women with four precisely and prospectively defined phenotypes: PL (n = 8), PNL (n = 10), TL (n = 7) and TNL (n = 10). Gene expression data were analyzed using shrunken centroid analysis to identify a minimal set of genes that uniquely characterizes each of the four phenotypes. Expression profiles of 73 genes and non-coding RNA sequences uniquely identified each of the four phenotypes. The shrunken centroid analysis and 10 times 10-fold cross-validation was also used to minimize false positive finings and overfitting. Identified were the pathways and molecular processes associated with and the cis-regulatory elements in gene's 5' promoter or 3'-UTR regions of the set of genes which expression uniquely characterized the four phenotypes.ResultsThe largest differences in gene expression among the four groups occurred at maternal fetal interface in decidua, chorion and amnion. The gene expression profiles showed suppression of chemokines expression in TNL, withdrawal of this suppression in TL, activation of multiple pathways of inflammation in PL, and an immune rejection profile in PNL. The genes constituting expression signatures showed over-representation of three putative regulatory elements in their 5'and 3' UTR regions.ConclusionsThe results suggest that pregnancy is maintained by downregulation of chemokines at the maternal-fetal interface. Withdrawal of this downregulation results in the term birth and its overriding by the activation of multiple pathways of the immune system in the preterm birth. Complications of the pregnancy associated with impairment of placental function, which necessitated premature delivery of the fetus in the absence of labor, show gene expression patterns associated with immune rejection
Spectral Theory of Sparse Non-Hermitian Random Matrices
Sparse non-Hermitian random matrices arise in the study of disordered
physical systems with asymmetric local interactions, and have applications
ranging from neural networks to ecosystem dynamics. The spectral
characteristics of these matrices provide crucial information on system
stability and susceptibility, however, their study is greatly complicated by
the twin challenges of a lack of symmetry and a sparse interaction structure.
In this review we provide a concise and systematic introduction to the main
tools and results in this field. We show how the spectra of sparse
non-Hermitian matrices can be computed via an analogy with infinite dimensional
operators obeying certain recursion relations. With reference to three
illustrative examples -- adjacency matrices of regular oriented graphs,
adjacency matrices of oriented Erd\H{o}s-R\'{e}nyi graphs, and adjacency
matrices of weighted oriented Erd\H{o}s-R\'{e}nyi graphs -- we demonstrate the
use of these methods to obtain both analytic and numerical results for the
spectrum, the spectral distribution, the location of outlier eigenvalues, and
the statistical properties of eigenvectors.Comment: 60 pages, 10 figure
Prescription and Other Medication Use in Pregnancy
OBJECTIVE:
To characterize prescription and other medication use in a geographically and ethnically diverse cohort of women in their first pregnancy.
METHODS:
In a prospective, longitudinal cohort study of nulliparous women followed through pregnancy from the first trimester, medication use was chronicled longitudinally throughout pregnancy. Structured questions and aids were used to capture all medications taken as well as reasons they were taken. Total counts of all medications taken including number in each category and class were captured. Additionally, reasons the medications were taken were recorded. Trends in medications taken across pregnancy and in the first trimester were determined.
RESULTS:
Of the 9,546 study participants, 9,272 (97.1%) women took at least one medication during pregnancy with 9,139 (95.7%) taking a medication in the first trimester. Polypharmacy, defined as taking at least five medications, occurred in 2,915 (30.5%) women. Excluding vitamins, supplements, and vaccines, 73.4% of women took a medication during pregnancy with 55.1% taking one in the first trimester. The categories of drugs taken in pregnancy and in the first trimester include the following: gastrointestinal or antiemetic agents (34.3%, 19.5%), antibiotics (25.5%, 12.6%), and analgesics (23.7%, 15.6%, which includes 3.6%; 1.4% taking an opioid pain medication).
CONCLUSION:
In this geographically and ethnically diverse cohort of nulliparous pregnant women, medication use was nearly universal and polypharmacy was common
Excited state-specific CASSCF theory for the torsion of ethylene
State-specific complete active space self-consistent field (SS-CASSCF) theory
has emerged as a promising route to accurately predict electronically excited
energy surfaces away from molecular equilibria. However, its accuracy and
practicality for chemical systems of photochemical interest has yet to be fully
determined. We investigate the performance of SS-CASSCF theory for the
low-lying ground and excited states in the double bond rotation of ethylene. We
show that state-specific approximations with a minimal (2e, 2o) active space
provide comparable accuracy to state-averaged calculations with much larger
active spaces, while optimising the orbitals for each excited state
significantly improves the spatial diffusivity of the wave function. However,
the unbalanced post-CASSCF dynamic correlation in valence and Rydberg
excitations, or the use of a non-diffuse basis set, causes excited state
solutions to coalesce and disappear, creating unphysical discontinuities in the
potential energy surface. Our findings highlight the theoretical challenges
that must be overcome to realise practical applications of state-specific
electronic structure theory for computational photochemistry.Comment: 10 pages, 6 figure
Role of early second-trimester uterine artery Doppler screening to predict small-for-gestational-age babies in nulliparous women
Background
Trophoblastic invasion of the uterine spiral arteries substantially increases compliance to accommodate increased blood flow to the placenta. Failure of this process impedes uterine artery blood flow, and this may be detected by uterine artery Doppler flow studies. However, the clinical utility of uterine artery Doppler flow studies in the prediction of adverse pregnancy outcomes in a general population remains largely unknown.
Objective
We sought to determine the utility of early second-trimester uterine artery Doppler studies as a predictor of small-for-gestational-age neonates.
Study Design
Nulliparous women with a viable singleton pregnancy were recruited during their first trimester into an observational prospective cohort study at 8 institutions across the United States. Participants were seen at 3 study visits during pregnancy and again at delivery. Three indices of uterine artery Doppler flow (resistance index, pulsatility index, and diastolic notching) were measured in the right and left uterine arteries between 16 weeks 0 days’ and 22 weeks 6 days’ gestation. Test characteristics for varying thresholds in the prediction of small for gestational age (defined as birthweight <5th percentile for gestational age [Alexander growth curve]) were evaluated.
Results
Uterine artery Doppler indices, birthweight, and gestational age at birth were available for 8024 women. Birthweight <5th percentile for gestational age occurred in 358 (4.5%) births. Typical thresholds for the uterine artery Doppler indices were all associated with birthweight <5th percentile for gestational age (P < .0001 for each), but the positive predictive values for these cutoffs were all <15% and areas under receiver operating characteristic curves ranged from 0.50-0.60. Across the continuous scales for these measures, the areas under receiver operating characteristic curves ranged from 0.56-0.62. Incorporating maternal age, early pregnancy body mass index, race/ethnicity, smoking status prior to pregnancy, chronic hypertension, and pregestational diabetes in the prediction model resulted in only modest improvements in the areas under receiver operating characteristic curves ranging from 0.63-0.66.
Conclusion
In this large prospective cohort, early second-trimester uterine artery Doppler studies were not a clinically useful test for predicting small-for-gestational-age babies
