    TIME COURSE OF CHANGES IN STRADDLE JUMP AND VERTICAL JUMP PERFORMANCE AFTER ACUTE STATIC STRETCHING IN ARTISTIC GYMNASTS

    AbstractThe aim of the present study was to examine the time course of changes of the effects of static stretching on straddle jump, vertical jump height and flexibility after performing dynamic movements in artistic gymnasts. The study was participated by 14 female artistic gymnasts aged between 9 and 14. Vertical jump heights were measured using the New Test 2000 testing device. Flexibility measurement was performed with sit and reach flexibility test. Artistic gymnastics-specific jump movement was evaluated looking at the angle between lower extremities using the Dartfish motion analysis program. Baseline measurements taken before the standard warm-up and the measurements taken after static stretching were assessed. Four sets of gymnastics moves each lasting two minutes were performed. The tests were repeated after each set. Static stretching was found to affect vertical jump performance significantly. Significant differences were found between Jump2&nbsp;and Jump1&nbsp;(p=0.01), Jump2&nbsp;and Jump3&nbsp;(p=0.03), Jump2&nbsp;and Jump4&nbsp;(p=0.01), Jump2&nbsp;and Jumop5&nbsp;(p=0.004), and Jump2&nbsp;and Jump6&nbsp;(p=0.009). In flexibility-related changes, significant differences were found between Reach2&nbsp;and Reach3&nbsp;(p=0.03), and Reach2&nbsp;and Reach6&nbsp;(p=0.006). Static stretching was found to have no significant effect on the lower extremity straddle jump degree. In conclusion, although the static stretching protocol reduced vertical jump performance significantly, it did not affect the artistic gymnastics-specific jump move. The negative effects of static stretching on jump height approached the baseline value approximately 4 minutes later.</p

    Effect of low dose methotrexate on the bone mineral density of patients with plaque psoriasis

    Amaç: Eklem tutulumu olmayan kronik plak tip psoriasis hastalarında kısa süreli düşük doz metotreksat (MTX) tedavisinin kemik mineral yoğunluğu (KMY) üzerine etkisini araştırmak. Gereç ve Yöntemler: Eklem tutulumu olmayan 15 (8 kadın ve 7 erkek) kronik plak tip psoriasis hastası çalışmaya dahil edildi. Tüm hastaların başlangıç ve MTX tedavisinin birinci yılındaki lomber vertebra (L2-4), femur (femur boynu ve total) ve önkol KMY’leri X-ray absorptiyometri ile ölçüldü. MTX tedavisi öncesi ve sonrası KMY’nin istatistiksel karşılaştırması için Wilcoxon test kullanıldı. Bulgular: Kısa süreli düşük doz MTX tedavisi kullanan hastaların lomber ve femur boynu KMY değerlerinde istatistiksel olarak anlamlı düzeyde azalma tespit edildi (her iki bölge için p0.05). Sonuç: Bu çalışma eklem tutulumu olmayan kronik plak tip psoriasis hastalarında kısa süreli düşük doz MTX tedavisinin trabeküler ve kortikal KMY’yi azaltabileceğini göstermektedirObjective: To investigate the effect of short-term low dose methotrexate (MTX) treatment on bone mineral density (BMD) of patients with chronic plaque psoriasis without joint involvement. Material and Methods: Fifteen patients (8 females and 7 males) with chronic plaque psoriasis without joint involvement were enrolled to the study. BMD of lumbar spine (L2-L4), femur (femoral neck and total) and forearm of all patients were measured using dual energy X-ray absorbtiometry, at baseline and after 1 year of MTX treatment. Wilcoxon test was used to detect differences in BMD of the patients before and after MTX treatment. Results: Significant decrease was found in the BMD of lumbar spine and femur neck in the patients taking short-term low dose MTX (p0.05, p>0.05 respectively). Conclusion: This study indicates that short-term low dose MTX treatment may decrease cortical and trabecular BMD in patients with chronic plaque psoriasis without joint involvement

    Psoriasis kronik seyirli, tekrarlayıcı, inflamatuvar ve hiperproliferatif bir deri hastalığıdır. Osteoporoz kemik mineral yoğunluğu (KMY) ve kemik matriksinin birlikte kaybı ile karakterize bir hastalıktır. Azalmış KMY osteoporotik kırıklar için major bir risk faktörüdür. Bu çalışmanın amacı, eklem tutulumu olmayan orta ve şiddetli plak tip psoriasisli hastalarda KMY'nu (Z skoru kullanılarak) araştırmaktır. Eklem tutulumu olmayan plak tip psoriasis tanısı konulan 43 hasta (12 erkek, 31 kadın ve yaş ortalaması 38.3) ve 30 sağlıklı kişi (9 erkek, 21 kadın ve yaş ortalaması 38.6) çalışmaya kabul edildi. Tüm olguların ve sağlıklı kişilerin lomber (L2-4), femur ve sol önkol KMY'ları X-ray absorptiyometri ile ölçüldü. Hastaların L2-4, femur ve sol önkol ortalama KMY değerleri ile sağlıklı kişilerin L2-4, femur ve sol önkol ortalama KMY değerleri arasında anlamlı fark tespit edilmedi (her bölge için p>0.05). Her iki grup 40 yaş altı ve üstü olarak ikişer alt gruba ayrıldı ve alt gruplar arasında yapılan karşılaştırmalarda anlamlı fark saptanmadı (p>0.05). Ayrıca menapozda olan hastaların KMY ile menapozda olan sağlıklı kişilerin KMY arasında da anlamlı fark tespit edilmedi (p>0.05). Sonuç olarak, bu çalışma eklem tutulumu olmayan orta ve şiddetli plak tip psoriasisli hastalarda kortikal ve trabeküler kemik dansitesinin etkilenmediğini göstermektedir.Psoriasis is a chronic, recurrent, inflammatory and hyperproliferative skin disease. Osteoporosis is characterized by decrease of bone mineral density (BMD) and bone matrix. Reduced BMD is a major risk factor for osteoporotic fracture. The aim of this study is to investigate BMD (using Z score) in patients with moderate and severe plaque psoriasis without arthropathy. Forty-three patients (12 males, 31 females and mean age 38.2) diagnosed as plaque psoriasis without joint involvement and 30 healthy subjects (9 males, 21 females and mean age 38.6) were included to this study. BMD was measured with dual X-ray absorptiometry at the lumbar spine (L2-4), femur and left forearm of all patients and healthy subjects. There were no significant differences in the mean BMD of L2-4, femur and left forearm between patients and healthy subjects (for each region p>0.05). Both groups were divided into two subgroups according to the age of the subjects as a younger and older than 40-year-old. No significant differences were detected in comparisons between these subgroups (p>0.05). Furthermore, we have not found significant differences in BMD between patients and healthy subjects which were postmenopausal (p>0.05). As a result, this study indicoted that cortical or trabecular bone density was not affected in patients with moderate and severe plaque psoriasis without joint involvement

    Primary spinal extranodal Hodgkin's disease at two levels

    About 90% of Hodgkin's disease cases originate from lymph nodes whereas 10% from extranodal regions. Patients rarely present with spinal cord compression due to epidural Hodgkin's disease. Primary spinal extradural Hodgkin's disease which does not have any other organ involvement in the body is even rarer. A 39-year-old male patient who complained of lumbar pain had normal findings upon neurological examination. Radiological examination revealed a mass on the epidural space at level L3 and the involvement of the vertebral corpus accompanied by the involvement of C6 vertebral body. Primary focus could not be identified despite further investigation. The patient underwent L3 laminectomy and posterior decompression and biopsy was obtained from the lesion extending to epidural space. The pathological result was reported as lymphocyte dominant type Hodgkin's disease. Flow cytometry was performed to the lesion, also. The patient was evaluated as Stage 4A according to Ann Arbor classification. Postoperative radiotherapy was applied to lumbar and cervical region. In the literature we have not come across any case of primary spinal extranodal Hodgkin's disease with involvement at two levels. In conclusion, although it might be extremely rare, primary spinal extranodal Hodgkin's disease with involvement at two levels might be observed. © 2004 Elsevier B.V. All rights reserved

    Infectious diseases and clinical microbiology consultations in the emergency department: A cross-sectional study at a tertiary-care hospital

    Background: Although there is limited data about the role of infectious diseases and clinical microbiology(IDCM) consultations in the Emergency Department (ED), they have a key role in deciding on hospitalizationand appropriate use of antibiotics.Objectives: To evaluate demographic and clinical characteristics of patients who visited the ED of our hospitaland underwent an IDCM consultation.Materials and methods: In this cross-sectional study, were viewed the medical records of adult patientswho visited the ED of our hospital between May and August 2021 and needed IDCM consultation. The demographic data, the date and time of admission and consultation, the departments that were consultedbefore IDCM, laboratory results, diagnosis, and outcome were recorded.Results: Out of 42,116 ED visits, 1,007 (2.4%) IDCM consultations were requested. The median time between admission and IDCM consultation was 239 min (150.0–373.5). Before 56.9% of IDCM consultations,pre-consultations were requested from other departments, and the time interval was significantly longer.The median age of patients was 68 years (51–77years). Infections were confirmed by the IDCM physicianin 79.6% of the consultations. The most diagnosed infections were urinary tract infections (32.4%), skin-softtissue infections (16.9%) and lower respiratory tract infections (10.3%), whereas 9.3% of the consultationsresulted in hospitalization to the infection ward, 25.1% to other wards, and 5% to the intensive care unit (ICU).Conclusions: Two of 3 consultations resulted in hospitalization in other wards, and this shows that IDCMconsultations are beneficial for managing patients with infectious diseases hospitalized in other departments.Communication between IDCM specialists and ED colleagues is important, especially in the managementof elderly patients who require a multidisciplinary approach.</p
