Clinical Studies on Transcobalamin and Transcobalamin Subfractions in Health and Diseases.

Serum transcobalamin determinations were made on 148 normal control sera and 233 various pathologic sera of patients with carcinoma, cirrhosis of the liver, leukemia, anemia and other miscellaneous diseases. Serum B12 concentration and unsaturated B12 binding capacity (UB12BC) were determined by charcoal absorption methods of Lau and Gottlieb. The normal ranges of B12, UB12BC, and TB12BC were as follows: B12, 126 to 1260 pg/ml, with a mean of 518 pg/ml, UB12BC, 508-1484 pg/ml, with a mean of 884 pg/ml, and TB12BC, 655-2619 pg/ml, with a mean of 1408 pg/ml, respectively. A significant increase in UB12BC was noted in cases with carcinoma and infectious diseases, whereas an abnormal increase was noted in cases with chronic myelogenous leukemia coinciding with previous reports by others. To investigate the clinical significance of these transcobalamin (TC) elevations, a B12 loading test (CN B12 30μg i.m.) was carried out on a number of selected cases, and changes in serum B12 concentrations, UB12BC, and its subtraction levels were determined at postinjection periods of 30 min, 1, 3, 5, 10 and 24 hours.Fractionation of UB12BC subfractions were carried out by a modification of Retief's DEAE cellulose rapid elution method. The new method consisted mainly of an increase of the amount of exchanger and control of elution speed by the use of a proportioning pump. Free B12, TC II, post TC II, which was arbitrarily named according to the slower elution from the column than TC II and TC I, were well separated by the single elution. In normal cases a CN-B12 loading test resulted in a slight increase in B12 concentration shortly after the injection and an inverse decrease in UB12BC, in which the decrease was mostly due to the decrease of TC II. Serial determinations have disclosed that the decreased level of UB12BC tend to return to preinjection level 5 to 10 hours after the injection. In cases with chronic hepatitis and liver cirrhosis, UB12BC and TC II were apparently higher than normal and after the B12 loading, recovery to the preinjection levels were achieved with only a slight delay as compared with that of normal subjects, indicating that TC II synthesis may not be hindered by the presence of hepatic dysfunction. In CML, on the other hand, UB12BC level showed only an insignificant fluctuation by B12 loading and in one case of CML, in which abnormal elevation of TC II was noted, no apparent change of TC II could he observed by B12 loading test. 看護学生の認知症高齢者との関係 : 印象に残っている場面での気持ちに焦点をあてて

老年看護学実習において,認知症高齢者とのコミュニケーションや関係で印象に残っている場面で,看護学生が感じた気持ちの特徴をみた.その結果,《認知症と患者・家族を理解した》《うれしかった》《患者のために現状を何とかしたい》など肯定的な気持ちが7カテゴリー,《困った》《驚いた》《辛かった》など否定的な気持ちが6カテゴリー,その他の気持ちが4カテゴリーであり,計17カテゴリーとなった.約半数の看護学生は肯定的な気持ちを感じており,その内には《患者のために現状を何とかしたい》と答えた学生もいた.しかし,認知症高齢者との関係で《困った》学生が最も多かった.このことから,今後,認知症の症状や具体的な対応の仕方をイメージできるように実習前に演習を行う.また,臨地実習では学生が困難に感じた場面で,具体的な状況,高齢者の気持ち,対処方法などを考えさせるよう指導し,看護学生にコミュニケーション技術の習得を促す

長崎県対馬地域および長崎市周辺地域におけるカンキツ遺伝資源の調査

We performed exploration on citrus germplasms in Tsushima Islands and the periphery of Nagasaki city in 2007. There are old trees of three kinds of mandarin in Tsushima islands. One of them was C. tachibana Tanaka but others were not identified. Then we collected \u27Yukou\u27 (Citrus sp.) and \u27Nagasaki zabon sandaime\u27 (C. grandis Osbeck). Old trees which estimated over 100 years of \u27Yukou\u27 have been distributed in Sotome area and south part of the Nagasaki city such as Ogomori, Fukahori, Sanwa and Isomichimachi. Collected \u27Yukou\u27 is a selection for propagating in Nagasaki city. \u27Nagasaki zabon sandaime\u27 is an old seedling tree estimated 120 years old.
