    The organisational precursors to human automation interaction issues in safety-critical domains: the case of an automated alarm system from the air traffic management domain

    Much has been written about the side effects of automation in complex safety-critical domains, such as air traffic management, aviation, nuclear power generation, and healthcare. Here, human factors and safety researchers have long acknowledged that the potential of automation to increase cost-effectiveness, quality of service and safety, is accompanied by undesired side effects or issues in human automation interaction (HAI). Such HAI issues may introduce the potential for increased confusion, uncertainty, and frustration amongst sharp end operators, i.e. the users of automation. These conditions may result in operators to refuse to use the automation, in impaired ability of operators to control the hazardous processes for which they are responsible, and in new, unintended paths to safety failure. The present thesis develops a qualitative framework of the organisational precursors to HAI issues (OPHAII) that can be found in safety-critical domains. Organisational precursors denote those organisational and managerial conditions that, although distant in time and space from the operational environment, may actually influence the quality of HAI found there. Such precursors have been extensively investigated by organisational safety (OS) scholars in relation to the occurrence of accidents and disasters—although not HAI issues. Thus, the framework’s development is motivated by the intent to explore the theoretical gap lying at the intersection between the OS area and the current perspectives on the problem—the human computer interaction (HCI) and the system lifecycle ones. While considering HAI issues as a design problem or a failure in human factors integration and/or safety assurance respectively, both perspectives, in fact, ignore, the organisational roots of the problem. The OPHAII framework was incrementally developed based on three qualitative studies: two successive, historical, case studies coupled with a third corroboratory expert study. The first two studies explored the organisational precursors to a known HAI issue: the nuisance alert problem relative to an automated alarm system from the air traffic management domain. In particular, the first case study investigated retrospectively the organisational response to the nuisance alert problem in the context of the alarm’s implementation and improvement in the US between 1977 and 2006. The second case study has a more contemporary focus, and examined at the organisational response to the same problem within two European Air Navigation Service Providers between 1990 and 2010. The first two studies produced a preliminary version of the framework. The third study corroborated and refined this version by subjecting it to the criticism from a panel of 11 subject matter experts. The resulting framework identifies three classes of organisational precursors: (i) the organisational assumptions driving automation adoption and improvement; (2) the availability of specific organisational capabilities for handling HAI issues; and (3) the control of implementation quality at the boundary between the service provider and the software manufacturer. These precursors advance current understanding of the organisational factors involved in the (successful and problematic) handling of HAI issues within safety-critical service provider organisations. Its dimensions support the view that HAI issues can be seen as and organisational phenomenon—an organisational problem that can be the target of analysis and improvements complementary to those identified by the HCI and the system lifecycle perspectives

However it can fail to deliver the target benefit as intended by those managers and designers advocating the introduction of the tool. In safety critical domains this problem is of significance not only because the unexpected effects of automation might prevent its widespread usage but also because they might turn out to be a contributor to incident and accidents. Research on failures of automation to deliver the intended benefit has focused mainly on human automation interaction. This paper presents a PhD research plan that aims at characterizing decisions for those involved in development process of automation for safety critical domains, taken under productive pressure, to identify where and when the initial intention the automation is supposed to deliver can be lost along the development process. We tentatively call such decisions as drift and the final objective is to develop principles that will allow to identify and compensate for possible sources of drift in the development of new automation. The research is based on case studies and is currently entering Year 2

    The argali (Ovis ammon antiqua) from the Magliana area (Rome)

    During the Middle Pleistocene, the subspecies was widespread from Georgia to Portugal, though it is scantily recorded in local faunal assemblages of Southern Europe. Its occurrence in a few Late Pleistocene sites needs to be confirmed. In Italy, the subspecies is recorded in the late Galerian fauna of Visogliano (MIS 13 - 10) as well as in the Magliana area, where an incomplete skull was found at the beginning of the last century. Although the precise location where this specimen was found is unknown, on the basis of the results of the geochemical analysis performed on a small amount of sediment, sampled from the filling of the inner cavities of the horn-cores, and considering the stratigraphy of the area, the hypothesis that the skull comes from the deposits of the PG4 sequence (MIS14 partim - MIS13) cannot be ruled out. This hypothetical remark would confirm the occurrence in Italy of the argali in late Galerian faunas. The dimensions of Ovis ammon antiqua from the Magliana area fit well within the range of variability of the larger specimens, likely males, found at La Caune de L’Arago (France) (MIS 14-12), from which the Italian specimen differs in having less twisted and more dorso-ventrally curved horn-cores

    Playing with Literacy

    This is a qualitative research study that examines how the act of play influences the development of literacy skills. The research is formatted as a case study of a single five-year-old child from Western New York. Data were collected over the course of five weeks, with ten total observations of the focal child in his home environment. Data collection methods include interviews, observations of the focal child playing independently, observations of the focal child playing collaboratively, as well as the use of double entry journals and a cell phone for audio recordings. From the data, three major findings were discovered: a) play situations are created through imitation; b) self-directed speech is used during independent play; and c) imaginative play is promoted through social interaction with peers. Based on these major findings, conclusions and implications were made based on the way in which the focal child develops his literacy skills through various play interactions

    VIDEO KINEMATIC EVALUATION OF THE HEART (VI.KI.E.): AN IDEA, A PROJECT, A REALITY

    Introduction: The technological development of the last 20 years pledges the intensity of efforts for implementing novel imaging contactless modalities that accelerate the translation from the research bench to the patient bedside, especially in the cardiac field. In this work, a novel intraoperative cardiac imaging approach, named Video Kinematic Evaluation (Vi.Ki.E.), is presented and explained in detail. This technology is able to monitor, contactless, the cardiac mechanics and deformation in-situ during heart surgery. Cardiac kinematics have been deeply evaluated ranging from the experimental animal approach to the human myocardial pathologies in both left and right ventricles. Methods: Vi.Ki.E. can be defined \u201cas simple as innovative\u201d. It only consists of a high-speed camera placed upon an exposed beating heart in-situ to record cardiac cycles. Afterwards a tracker software is used on the recorded video to follow the epicardial tissue movements. This tracker provides information about trajectories of the epicardium and, thanks to a custom-made algorithm, the technology supplies heart mechanical information such as: Force of contraction or cardiac fatigue, Energy expenditure, Contraction velocity, displacement of the marker and epicardial torsion. This approach has been tested on 21 rats (9 ischemia/reperfusion and/or for validation, 12 for the gender difference study) and on 37 patients who underwent different surgery between 2015 and 2019. In detail 10 patients underwent Coronary Artery Bypass Grafting, 12 underwent Valve Replacement after Tetralogy of Fallot correction surgery, 6 implanted a Left Ventricular Assist Device (1 is moved in the case study section), 6 patients with Hypoplastic Heart Syndrome underwent GLENN or FONTAN surgery, 2 patients underwent Heart Transplantation and finally 1 patient underwent double valve replacement (this patient is moved into case study section). Results: The patients\u2019 results demonstrated that the Vi.Ki.E. technology was able to discriminate, with statistic potency, the kinematic differences before and after the surgery in real-time, suggesting possible clinical implications in the treatment of the patients before the chest closure and/or in the intensive care unit. As it concerns the experimental animals, the results are the basics of the validation technology. Some of them were used as accepted model in comparison with the Vi.Ki.E. results on patients. Conclusions: In conclusion, this study has shown that Vi.Ki.E. is a safe and contactless technology with promising possible clinical application. The ease in the evaluation and the algorithm-based approach makes Video Kinematic Evaluation a widespread technique from cellular level to human cases covering the entire experimental field with in-vivo evaluation and possibly Langendorff/Working Heart approaches
