    The GRADE Working Group clarifies the construct of certainty of evidence

    Syfte: Syftet med föreliggande studie var att undersöka vilka kommentarer och förslag bröstcancerpatienter i Uppsala/Örebro-regionen har avseende vĂ„rden i samband med bröstcanceroperationen. Metod: Totalt 1573 kvinnor frĂ„n kvalitetsregistret för bröstcancer i Uppsala/Örebro-regionen, som diagnostiserades med primĂ€r bröstcancer tillfrĂ„gades att delta i en enkĂ€tstudie. Av dessa besvarade 1050 kvinnor enkĂ€ten. Data insamlades med hjĂ€lp av en enkĂ€t med tre öppna frĂ„gor varav en ska analyseras i denna studie. Cirka 200 kvinnor svarade pĂ„ denna frĂ„ga. Data analyserades med hjĂ€lp av kvalitativ innehĂ„llsanalys. Resultat: Resultatet av studiedeltagarnas svar visade att det finns en stor variation i vĂ„rdtillfredstĂ€llelse, i samband med operationen, bland bröstcancerdrabbade kvinnor i Uppsala/Örebro-regionen. Genom analysen utkristalliserades fem olika kategorier, bemötande, information, samordning, vĂ„rdtid och övergripande kommentarer om vĂ„rden. Förslagen kvinnorna angav var bland annat förbĂ€ttring i bemötandet av patienter, mer utförlig information, tillgĂ„ng till en patientbunden vĂ„rdgivare samt lĂ€ngre vĂ„rdtid. Slutsats: Slutsatsen av undersökningen Ă€r att kvinnorna har bĂ„de positiva och negativa erfarenheter av vĂ„rden kring bröstcanceroperationen. Majoriteten av kommentarerna var dock negativa, framför allt inom kategorierna samordning och bemötande. Detta tyder pĂ„ att det finns omrĂ„den som Ă€r i behov av förbĂ€ttring i vĂ„rden av bröstcancerpatienter.Purpose: The purpose of this study was to examine the comments and proposals of breast cancer patients in Uppsala/Örebro region regarding the care during breast cancer surgery. The women entered proposals including the improvement in treatment of patients, more detailed information, access to a patient-bound caregivers and longer hospital stay. Conclusion: The conclusion of the study is that the experience of the participants was both positive and negative in care around the breast cancer surgery. The majority of the comments were negative, particularly in the categories of coordination and treatment. This suggests that there are areas in need of improvement in the care of breast cancer patients

    Analys av samtal mellan personer med afasi och logopeder/anhöriga : AnvÀndande av kommunikativa resurser i samarbete mot gemensam förstÄelse

    I föreliggande studie undersöktes kommunikativa resurser i samtal mellan tre personer med afasi och deras respektive logoped/anhörig. Syftet var att identifiera och analysera resurser som samtalsdeltagarna gemensamt anvÀnde för att uppnÄ intersubjektivitet. Vidare undersöktes om det, utifrÄn vem personen med afasi samtalade med, fanns nÄgon inverkan pÄ hur de kommunikativa resurserna anvÀndes. Sex dyader spelades in och samtalsanalys anvÀndes för att studera materialet. SamtalsÀmnen valdes fritt av samtalsdeltagarna. Analysen resulterade i identifiering av tre bevarade resurser hos personerna med afasi; upprepningar, gester och skratt. Det gemensamma för dessa resurser var; att de förekom i den naturliga sekventialiteten i samtalet, att deras kommunikativa funktioner uppstod i samarbetet mellan samtalsdeltagarna och att de bidrog till intersubjektiviteten. Resurserna visade Àven pÄ en social kompetens hos deltagarna med afasi som ofta döljs av de sprÄkliga hindren. DÄ de kommunikativa resurserna fick liknande funktioner i de olika dyaderna, Àr resultatet troligen generaliserbart till annan interaktion dÀr personer med afasi deltar. Resursernas kommunikativa funktioner föreföll inte bero pÄ vem samtalspartnern var. Slutsatsen var att sprÄklig kompetens hos personer med afasi framför allt bör ses ur ett interaktivt perspektiv och att ett sÄdant förhÄllningssÀtt Àr önskvÀrt i logopediskt behandlingsarbete.Analysis of Talk-in-Interaction involving People with Aphasia and Speech and Language Pathologists/Spouses: The Use of Communicative Resources in Collaboration toward Intersubjectivity. In the present study, communicative resources in conversations involving three persons with aphasia and their speech and language pathologists/spouses were investigated. The main purpose was to identify and analyse collaboratively used resources for achieving intersubjectivity. In addition, possible changes in the use of the communicative resources depending on conversational partner were investigated. The conversations of six dyads were recorded and analysed. Conversational topics were chosen by the participants. Through the analysis three preserved resources for participants with aphasia could be identified; repetitions, gestures and laughter. All resources occurred within the natural sequentiality of the conversation, their communicative functions emerged in collaboration of the participants and contributed to intersubjectivity. The social competence of the persons with aphasia, often concealed by the language impairment, was also revealed. The results may possibly be generalised to other communicative activities where persons with aphasia participate, since similar communicative functions were achieved through the same resources in different dyads. The functions of the resources were not determined by conversational partner. To conclude, it may be argued that linguistic competence of persons with aphasia should mainly be considered in an interactive perspective and that this view is preferred in language therapy
