    A Rare Case: Improved Heart Failure with Anti-Complement Therapy in Complement-Dependent Hemolytic Uremic Syndrome

    MakaleWOS:000925209300014xtrarenal involvement occurs in approximately 20% of patients with complement-mediated hemolytic-uremic syndrome. The involvement is usually of the nervous system, and cardiac involvement occurs in 3%-10% of patients. Cardiac manifestations vary, including myocardial infarction, cardiomyopathy, and acute decompensated heart failure. Among these patients, thrombotic microangiopathy-related cardiac dysfunction is mainly due to the continuous activation of the complement system, which leads to endothelial damage and thrombosis in the coronary microvessels. We wanted to highlight the importance of cardiac evaluation at the time of diagnosis or during follow-up in thrombotic micro-angiopathy patients by presenting a case of heart failure with low ejection fraction in a 24-year-old young patient in whom we detected complement-mediated hemolytic-uremic syndrome, a secondary mutation of complement factor H receptor. It is still an unknown issue because of the rarity of cardiac involvement in complement-mediated hemolytic-uremic syndrome patients. Primary myocardial involvement is increasingly recognized as a possible concomitant feature of hemolytic-uremic syndrome. Failure to perform a detailed cardiac evaluation both at diagnosis and during follow-up in complement-mediated hemolytic-uremic syndrome patients can lead to fatal outcomes. Anti-complement therapy can also lead to good cardiac outcomes in these patients

    Serum total L-carnitine levels in non-obese women with polycystic ovary syndrome

    BACKGROUND: Carnitine plays essential roles in energy production, oxidative stress and glucose metabolism. This study was planned to determine serum total l-carnitine levels in non-obese women with polycystic ovary syndrome (PCOS). METHODS: There were 27 non-obese women with PCOS and 30 healthy, age- and body mass index (BMI) matched controls were evaluated in this controlled clinical study. Serum lipid sub-fractions, fasting glucose, insulin and other hormones (gonadotrophins, androgens) and total l-carnitine levels were measured. Homeostasis model assessment (HOMA-IR) was used to estimate insulin resistance. RESULTS: The women with PCOS had significantly higher serum dehydroepiandrosterone sulfate, total testosterone, free androgen index (FAI), luteinizing hormone (LH), low-density lipoprotein (LDL) cholesterol, non-high density lipoprotein (HDL) cholesterol, fasting insulin levels and HOMA-IR measurement and LH/FSH ratios than healthy women. However, total l-carnitine and sex hormone-binding globulin (SHBG) levels were significantly lower in women with PCOS. l-Carnitine level was negatively correlated with FAI, but positively correlated with SHBG. Multiple regression analysis revealed that SHBG was a strong predictor of serum total l-carnitine level. CONCLUSIONS: Decreased total l-carnitine levels may be associated with hyperandrogenism and/or insulin resistance in non-obese women with PCOS. Long-term studies are needed to evaluate carnitine metabolism in PCOS, especially with regard to the molecular basis. © The Author 2008.     The positioning performance of low-cost GNSS receivers in the Precise Point Positioning method

    Satellite-based positioning, which started being developed in the mid-1960s for military purposes, is now used in almost every area. For the studies single and/or double frequency receivers are used. The cost of a receiver and antenna couple that have capable of high coordinate accuracies ranges from 3000to3000 to 15000. With the production of Original Equipment Manufacturer (OEM) receivers, the cost of satellite-based location determination decreases to approximately one in 10 for the civilian user compared to the operations performed with geodetic receivers and antennas. However, although these receivers collect data in multi-Global Navigation Satellite System (GNSS) and frequencies, the accuracy of the coordinate values estimated is not as high as geodetic receivers and antennas. Therefore, it is necessary to carry out an accuracy study to obtain information about which studies can be used in. In this study, measurements were made at the UZEL point located on the roof of the Yıldız Technical University Geomatics Engineering Department by using the ZED-F9P-02B OEM multi GNSS receiver and ANN-MB L1/L2 multi-band GNSS patch antenna. The performance of the test results has been examined by comparing the results from CSRS(Canadian Spatial Reference System)-PPP with the coordinates of the UZEL point. As a result of the comparison, the difference between the coordinate determined with collected 3.5 hr data and the coordinates of the UZEL point has been determined as – 1.4 cm, 2.8 cm, and 9.3 cm in the East, North, and Height directions, respectivel

According to the literature, a few cases of accidental displacement of molars into adjacent anatomical spaces, such as the infratemporal fossa, the pterygomandibular space, the maxillary sinus, the buccal space, or the lateral pharyngeal space, during surgical interventions have been reported [3, 5, 6]. However this is the first reported case of maxillary third molar displaced into pterygopalatine fossa.     Dermoid cysts are uncommon, benign congenital lesions of ectodermal origin that can occur in any region of the body. They arise from epithelial rests during embryogenesis. Nearly 7% occur in the head-neck region, and they represent less than 0.01 % of all cavity cysts. They are usually diagnosed during the 2nd and 3rd decade of life, and rarely present in children. A case of dermoid cyst in the sublingual region in a 4-year-old female child was presented. The floor of the mouth is the most common location in the oral cavity and should be considered in the differential diagnosis of any midline lesion. Treatment is surgical excision

Günümüzde yaygınlaşan kitle iletişim araçları yoluyla kaynak ülke hedef ülkeye kendi yaşam tarzını ihraç edebilmekte hatta bunu bir statüko belirleyici bir unsur olarak benimsetebilmektedir. Bu sebeple eski Sovyet Sosyalist Cumhuriyetleri (Rusya), Çin gibi ülkelere Mc Donald's ve Star Bucks gibi fast food işletim sistemi temelli bir yaşam tarzı yardımı ile diğer toplumları da kendi doğrultusunda dönüştürebilmektedir. Benzer durum Türkiye'deki kahve dükkanları için de geçerlidir. Son yıllar da her köşeye açılan kahve dükkanları ülkemize önceleri yabancı zincirler aracılığıyla girmiş olup, geçtiğimiz beş ve altı yılda yerli marka ve zincirlerin devreye girmesiyle tüm yurt sathına yayılmıştır. Önceleri çay ve Türk kahvesi içmeye dayalı içecek alışkanlıkları değişirken,yeni açılan kafelerle birlikte girmesiyle esspresso, latte, macchiato gibi içecekler günlük yaşamımıza girmişlerdir. Bu yeni kahve kültürü beraberinde daha önce ülkemizde hiç bilinmeyen kahve ve kahve ekipmanları hakkında bilgi sahibi ve talep edildiğinde kahve ile ilgili detaylı bilgi verecek kişi diye tanımlayabileceğimiz "Barista" kavramı hayatımıza girmiştir. Bu makalede aşağıdaki sorulara cevap aranmaya çalışılmaktadır. Barista nedir ve ne iş yapar? Dünyada ve Türkiye'de ki mevcut konumu nedir?
