    Simulation of coalescence, break up and mass transfer in bubble columns by using the Conditional Quadrature Method of Moments in OpenFOAM

    The evaluation of the mass transfer rates and the fluid-dynamics aspects of bubble columns are strongly affected by the intrinsic poly-dispersity of the gas phase, namely the different dispersed bubbles are usually distributed over a certain range of size and chemical composition values. In our previous work, gas-liquid systems were investigated by coupling Computational Fluid Dynamics with mono-variate population balance models (PBM) solved by using the quadrature method of moments (QMOM). Since mass transfer rates depend not only on bubble size, but also on bubble composition, the problem was subsequently extended to the solution of multi-variate PBM (Buffo et al. 2013). In this work, the conditional quadrature method of moments (CQMOM) is implemented in the open-source code OpenFOAM for describing bubble coalescence, breakage and mass transfer of a realistic partially aerated rectangular bubble column, experimentally investigated by Diaz et al.(2008).     Metabolic, inflammatory and haemostatic effects of a low-dose continuous combined HRT in women with type 2 diabetes: potentially safer with respect to vascular risk?

    BACKGROUND Conventional hormone replacement therapy (HRT) containing conjugated equine oestrogen (CEE) and medroxyprogesterone acetate (MPA) increases triglyceride, C- reactive protein (CRP) and coagulation Factor VII concentrations, potentially explaining their increased coronary heart disease (CHD) and stroke risk. OBJECTIVE To assess the metabolic effects of a continuous combined HRT containing 1 mg oestradiol and 0.5 mg norethisterone or matching placebo. DESIGN Double-blind, randomized placebo-controlled trial. PATIENTS Fifty women with type 2 diabetes. MEASUREMENTS Classical and novel risk factors for vascular disease. RESULTS Triglyceride concentration was not altered (P = 0.31, change in active arm relative to placebo) and low-density lipoprotein (LDL) cholesterol concentration declined 13% (P = 0.018). IL-6 concentration (mean difference -1.42 pg/ml, 95% CI: -2.55 to - 0.29 IU/dl, P = 0.015), Factor VII (-32 IU/dl, -43 to -21 IU/l, P lt 0.001) and tissue plasminogen activator antigen (by 13%, P = 0.005) concentrations fell, but CRP was not significantly altered (P = 0.62). Fasting glucose (P = 0.026) also declined significantly, but there are no significant effects on HBA1c, Factor IX or APC resistance. CONCLUSIONS HRT containing 1 mg oestradiol and 0.5 mg norethisterone may avoid the adverse metabolic effects potentially implicated in the elevated CHD and stroke risk induced by conventional higher dose HRT. This type of preparation may therefore be more suitable than conventional HRT for women at elevated CHD risk such as those with type 2 diabetes. Large randomized controlled trials of such low dose preparations, powered for cardiovascular end points, are now needed

    The role of Computer Aided Process Engineering in physiology and clinical medicine

    This paper discusses the potential role for Computer Aided Process Engineering (CAPE) in developing engineering analysis and design approaches to biological systems across multiple levels—cell signalling networks, gene, protein and metabolic networks, cellular systems, through to physiological systems. The 21st Century challenge in the Life Sciences is to bring together widely dispersed models and knowledge in order to enable a system-wide understanding of these complex systems. This systems level understanding should have broad clinical benefits. Computer Aided Process Engineering can bring systems approaches to (i) improving understanding of these complex chemical and physical (particularly molecular transport in complex flow regimes) interactions at multiple scales in living systems, (ii) analysis of these models to help to identify critical missing information and to explore the consequences on major output variables resulting from disturbances to the system, and (iii) ‘design’ potential interventions in in vivo systems which can have significant beneficial, or potentially harmful, effects which need to be understood. This paper develops these three themes drawing on recent projects at UCL. The first project has modeled the effects of blood flow on endothelial cells lining arteries, taking into account cell shape change resulting in changes in the cell skeleton which cause consequent chemical changes. A second is a project which is building an in silico model of the human liver, tieing together models from the molecular level to the liver. The composite model models glucose regulation in the liver and associated organs. Both projects involve molecular transport, chemical reactions, and complex multiscale systems, tackled by approaches from CAPE. Chemical Engineers solve multiple scale problems in manufacturing processes – from molecular scale through unit operations scale to plant-wide and enterprise wide systems – so have an appropriate skill set for tackling problems in physiology and clinical medicine, in collaboration with life and clinical scientists

    Statistical adjustment for a measure of healthy lifestyle doesn't yield the truth about hormone therapy

    The Women's Health Initiative randomized clinical trial of hormone therapy found no benefit of hormones in preventive cardiovascular disease, a finding in striking contrast with a large body of observational research. Understanding whether better methodology and/or statistical adjustment might have prevented the erroneous conclusions of observational research is important. This is a re-analysis of data from a case-control study examining the relationship of postmenopausal hormone therapy and the risks of myocardial infarction (MI) and ischemic stroke in which we reported no overall increase or decrease in the risk of either event. Variables measuring health behavior/lifestyle that are not likely to be causally with the risks of MI and stroke (e.g., sunscreen use) were included in multivariate analysis along with traditional confounders (age, hypertension, diabetes, smoking, body mass index, ethnicity, education, prior coronary heart disease for MI and prior stroke/TIA for stroke) to determine whether adjustment for the health behavior/lifestyle variables could reproduce or bring the results closer to the findings in a large and definitive randomized clinical trial of hormone therapy, the Women's Health Initiative. For both MI and stroke, measures of health behavior/lifestyle were associated with odds ratios (ORs) less than 1.0. Adjustment for traditional cardiovascular disease confounders did not alter the magnitude of the ORs for MI or stroke. Addition of a subset of these variables selected using stepwise regression to the final MI or stroke models along with the traditional cardiovascular disease confounders moved the ORs for estrogen and estrogen/progestin use closer to values observed in the Women Health Initiative clinical trial, but did not reliably reproduce the clinical trial results for these two endpoints.Comment: Published in at http://dx.doi.org/10.1214/193940307000000437 the IMS Collections (http://www.imstat.org/publications/imscollections.htm) by the Institute of Mathematical Statistics (http://www.imstat.org

    The production of metal artefacts in Southern Etruria (Central Italy): case studies from copper to Iron Age

    An analytical study is presented, aimed to determine the elemental composition of copper-based artefacts dated back from Copper Age to Early Iron Age (mid-fourth millennium to the VIIIth century B.C.), found on the Tyrrhenian side of the peninsula, corresponding to the Lazio region. The objects belong to different archaeological contexts and had various functions. They were analysed by the X-ray fluorescence technique. The results highlight the experimental character of Copper Age metallurgy, which will later evolve in the established use of copper-tin alloys. Regarding the Bronze Age, despite the typological and functional heterogeneity of the artefacts and the wide chronological range, the alloys are relatively homogeneous in composition, with regular changes that appear related to chronology, according to what is already known for the Italian peninsula. Such changes are supposedly due to variations in the availability of tin, which was not locally mined. Early Iron Age metallurgy is represented by the Selvicciola Hoard solely, which restricts the possibility of generalizing the conclusions. A striking feature of the alloys is the great compositional difference between the complete and the fragmented artefacts. The formers are made of tin bronze, whereas in the latter tin is replaced by antimony and/or lead. The use of such unusual alloys is unlikely due to lack of metallurgical knowledge. Considering the urbanized communities that arose in the Middle-Tyrrhenian area during the Early Iron Age, we suppose that such variability in a single context might be related to a production system capable of using alloys of different quality and value to satisfy a diversified demand
