Acute lung injury in paediatric intensive care: course and outcome
Introduction: Acute lung injury (ALI) and acute respiratory distress syndrome (ARDS) carry a high morbidity and mortality (10-90%). ALI is characterised by non-cardiogenic pulmonary oedema and refractory hypoxaemia of multifactorial aetiology [1]. There is limited data about outcome particularly in children. Methods This retrospective cohort study of 85 randomly selected patients with respiratory failure recruited from a prospectively collected database represents 7.1% of 1187 admissions. They include those treated with High Frequency Oscillation Ventilation (HFOV). The patients were admitted between 1 November 1998 and 31 October 2000. Results: Of the 85, 49 developed acute lung injury and 47 had ARDS. There were 26 males and 23 females with a median age and weight of 7.7 months (range 1 day-12.8 years) and 8 kg (range 0.8-40 kg). There were 7 deaths giving a crude mortality of 14.3%, all of which fulfilled the Consensus I [1] criteria for ARDS. Pulmonary occlusion pressures were not routinely measured. The A-a gradient and PaO2/FiO2 ratio (median + [95% CI]) were 37.46 [31.82-43.1] kPa and 19.12 [15.26-22.98] kPa respectively. The non-survivors had a significantly lower PaO2/FiO2 ratio (13 [6.07-19.93] kPa) compared to survivors (23.85 [19.57-28.13] kPa) (P = 0.03) and had a higher A-a gradient (51.05 [35.68-66.42] kPa) compared to survivors (36.07 [30.2-41.94]) kPa though not significant (P = 0.06). Twenty-nine patients (59.2%) were oscillated (Sensormedics 3100A) including all 7 non-survivors. There was no difference in ventilation requirements for CMV prior to oscillation. Seventeen of the 49 (34.7%) were treated with Nitric Oxide including 5 out of 7 non-survivors (71.4%). The median (95% CI) number of failed organs was 3 (1.96-4.04) for non-survivors compared to 1 (0.62-1.62) for survivors (P = 0.03). There were 27 patients with isolated respiratory failure all of whom survived. Six (85.7%) of the non-survivors also required cardiovascular support.Conclusion: A crude mortality of 14.3% compares favourably to published data. The A-a gradient and PaO2/FiO2 ratio may be of help in morbidity scoring in paediatric ARDS. Use of Nitric Oxide and HFOV is associated with increased mortality, which probably relates to the severity of disease. Multiple organ failure particularly respiratory and cardiac disease is associated with increased mortality. ARDS with isolated respiratory failure carries a good prognosis in children
The Decidability of Verification under Promising 2.0
In PLDI'20, Lee et al. introduced the \emph{promising } semantics PS 2.0 of
the C++ concurrency that captures most of the common program transformations
while satisfying the DRF guarantee. The reachability problem for finite-state
programs under PS 2.0 with only release-acquire accesses is already known to be
undecidable. Therefore, we address, in this paper, the reachability problem for
programs running under PS 2.0 with relaxed accesses together with promises. We
show that this problem is undecidable even in the case where the input program
has finite state. Given this undecidability result, we consider the fragment of
PS 2.0 with only relaxed accesses allowing bounded number of promises. We show
that under this restriction, the reachability is decidable, albeit very
expensive: it is non-primitive recursive. Given this high complexity with
bounded number of promises and the undecidability result for the RA fragment of
PS 2.0, we consider a bounded version of the reachability problem. To this end,
we bound both the number of promises and the "view-switches", i.e, the number
of times the processes may switch their local views of the global memory. We
provide a code-to-code translation from an input program under PS 2.0, with
relaxed and release-acquire memory accesses along with promises, to a program
under SC. This leads to a reduction of the bounded reachability problem under
PS 2.0 to the bounded context-switching problem under SC. We have implemented a
prototype tool and tested it on a set of benchmarks, demonstrating that many
bugs in programs can be found using a small bound
Trust in Robots
Robots are increasingly becoming prevalent in our daily lives within our living or working spaces. We hope that robots will take up tedious, mundane or dirty chores and make our lives more comfortable, easy and enjoyable by providing companionship and care. However, robots may pose a threat to human privacy, safety and autonomy; therefore, it is necessary to have constant control over the developing technology to ensure the benevolent intentions and safety of autonomous systems. Building trust in (autonomous) robotic systems is thus necessary. The title of this book highlights this challenge: âTrust in robotsâTrusting robotsâ. Herein, various notions and research areas associated with robots are unified. The theme âTrust in robotsâ addresses the development of technology that is trustworthy for users; âTrusting robotsâ focuses on building a trusting relationship with robots, furthering previous research. These themes and topics are at the core of the PhD program âTrust Robotsâ at TU Wien, Austria
Knowledge Capturing in Design Briefing Process for Requirement Elicitation and Validation
Knowledge capturing and reusing are major processes of knowledge management that deal with the elicitation of valuable knowledge via some techniques and methods for use in actual and further studies, projects, services, or products. The construction industry, as well, adopts and uses some of these concepts to improve various construction processes and stages. From pre-design to building delivery knowledge management principles and briefing frameworks have been implemented across project stakeholders: client, design teams, construction teams, consultants, and facility management teams. At pre-design and design stages, understanding the clientâs needs and usersâ knowledge are crucial for identifying and articulating the expected requirements and objectives. Due to underperforming results and missed goals and objectives, many projects finish with highly dissatisfied clients and loss of contracts for some organizations. Knowledge capturing has beneficial effects via its principles and methods on requirement elicitation and validation at the briefing stage between user, client and designer. This paper presents the importance and usage of knowledge capturing and reusing in briefing process at pre-design and design stages especially the involvement of client and user, and explores the techniques and technologies that are usable in briefing process for requirement elicitation
An Investigation on Benefit-Cost Analysis of Greenhouse Structures in Antalya
Significant population increase across the world, loss of cultivable land and increasing demand for food put pressure on agriculture. To meet the demand, greenhouses are built, which are, light structures with transparent cladding material in order to provide controlled microclimatic environment proper for plant production. Conceptually, greenhouses are similar with manufacturing buildings where a controlled environment for manufacturing and production have been provided and proper spaces for standardized production processes have been enabled. Parallel with the trends in the world, particularly in southern regions, greenhouse structures have been increasingly constructed and operated in Turkey. A significant number of greenhouses are located at Antalya. The satellite images demonstrated that for over last three decades, there has been a continuous invasion of greenhouses on all cultivable land. There are various researches and attempts for the improvement of greenhouse design and for increasing food production by decreasing required energy consumption. However, the majority of greenhouses in Turkey are very rudimentary structures where capital required for investment is low, but maintenance requirements are high when compared with new generation greenhouse structures. The angle of arrival estimation of multiple sources plays a vital role in the field of array signal
processing as MIMO systems can be employed at both the transmitter and the receiver end
and the system capacity, reliability and throughput can be significantly increased by using array
signal processing. Almost all applications require accurate direction of arrival (DOA) estimation
to localize the sources of the signals. Another important parameter of localization systems is
the array geometry and sensor design which can be application specific and is used to
estimate the DOA.
In this work, various array geometries and arrival estimation algorithms are studied and then a
new scheme for multiple source estimation is proposed and evaluated based on the
performance of subspace and non-subspace decomposition methods. The proposed scheme
has shown to outperform the conventional Multiple Signal Classification (MUSIC) estimation
and Bartlett estimation techniques. The new scheme has a better performance advantage at
low and high signal to noise ratio values (SNRs).
The research work also studies different array geometries for both single and multiple incident
sources and proposes a geometry which is cost effective and efficient for 3, 4, and 5 antenna
array elements. This research also considers the shape of the ground plane and its effects on
the angle of arrival estimation and in addition it shows how the mutual couplings between the
elements effect the overall estimation and how this error can be minimised by using a decoupling
matrix.
At the end, a novel miniaturised multi element reconfigurable antenna to represent the receiver
base station is designed and tested. The antenna radiation patterns in the azimuth angle are
almost omni-directional with linear polarisation. The antenna geometry is uniplanar printed logspiral
with striplines feeding network and biased components to improve the impedance
bandwidth. The antenna provides the benefit of small size, and re-configurability and is very
well suited for the asset tracking applications
