    Minimising the time required for a patient to receive primary care has always been the concern of the Accidents and Emergency units. Ambulances are usually the first to arrive on the scene and to administer first aid. However, as the time that it takes to transfer the patient to the hospital increases, so does the fatality rate. In this paper, a mobile teleconsultation system is presented, based primarily on third generation mobile links and on Wi-Fi hotspots around a city. This system can be installed inside an ambulance and will permit high-resolution videoconferencing between the moving vehicle and a doctor or a consultant within a base station (usually a hospital). In addition to video and voice, high quality still images and screenshots from medical equipment can also be sent. The test was carried out in Athens, Greece where a 3G system was recently deployed by Vodafone. The results show that the system can perform satisfactory in most conditions and can effectively increase the patient’s quality of service, while having a modest cost

    The importance of this paper lays in its suggestion: minimise the time for the initial treatment of a patient. As soon as an ambulance receives a patient, a videoconferencing session can be initiated between the moving vehicle and a base station in a hospital where a medical consultant resides. The communication link is implemented using 3G networks. Effectively, the use of such a system can decrease fatality on patients being transferred to A&E, as expert opinion can be obtained straight away.     The rapid development of online learning is opening up many new learning opportunities. Yet, with this increased potential come a myriad of risks. Usable security systems are essential as poor usability in security can result in excluding intended users while allowing sensitive data to be released to unacceptable recipients. This chapter presents findings concerned with usability for two security issues: authentication mechanisms and privacy. Usability issues such as memorability, feedback, guidance, context of use and concepts of information ownership are reviewed within various environments. This chapter also reviews the roots of these usability difficulties in the culture clash between the non-user-oriented perspective of security and the information exchange culture of the education domain. Finally an account is provided of how future systems can be developed which maintain security and yet are still usable

    Telemedicine of family-based treatment for adolescent anorexia nervosa: A protocol of a treatment development study.

    BackgroundFamily-based treatment is an efficacious treatment available for adolescents with anorexia nervosa. Yet the implementation of this treatment, at least in the United States, is challenging due to a limited number of trained family-based treatment therapists and the concentration of these therapists in a limited number of urban centers. The use of telemedicine in the delivery of family-based treatment can increase access to this therapy for this patient population.Methods/designThis two-year treatment development study (December 2013-November 2015) follows a two-wave iterative case series design. The study is ongoing and addresses the treatment needs of families in remote, rural, or underrepresented parts of the United States by delivering family-based treatment via telemedicine (video chat). The first six months of the study was dedicated to selecting a cloud-based secure telemedicine portal for use with participants. Recruitment for the first of two consecutive case series (N = 5) began during month seven. After these five patients completed treatment, a systematic review of treatment via feedback from participants and therapists related to the delivery of this model and use of technology was completed. A second wave of recruitment is underway (N = 5). At the end of both waves (N = 10), and after a second review of treatment, we should be able to establish the feasibility and acceptability of family-based treatment delivered via telemedicine for this patient population.DiscussionThis study is the first attempt to deliver family-based treatment for adolescents with anorexia nervosa via telemedicine. If delivering family-based treatment in this format is feasible, it will provide access to an evidence-based treatment for families heretofore unable to participate in specialist treatment for their child's eating disorder

    Demand for satellite-provided domestic communications services up to the year 2000

    Three fixed service telecommunications demand assessment studies were completed for NASA by The Western Union Telegraph Company and the U.S. Telephone and Telegraph Corporation. They provided forecasts of the total U.S. domestic demand, from 1980 to the year 2000, for voice, data, and video services. That portion that is technically and economically suitable for transmission by satellite systems, both large trunking systems and customer premises services (CPS) systems was also estimated. In order to provide a single set of forecasts a NASA synthesis of the above studies was conducted. The services, associated forecast techniques, and data bases employed by both contractors were examined, those elements of each judged to be the most appropriate were selected, and new forecasts were made. The demand for voice, data, and video services was first forecast in fundamental units of call-seconds, bits/year, and channels, respectively. Transmission technology characteristics and capabilities were then forecast, and the fundamental demand converted to an equivalent transmission capacity. The potential demand for satellite-provided services was found to grow by a factor of 6, from 400 to 2400 equivalent 36 MHz satellite transponders over the 20-year period. About 80 percent of this was found to be more appropriate for trunking systems and 20 percent CPS

    Palliative home-based technology from a practitioner's perspective: benefits and disadvantages

    This critical review paper explores the concept of palliative home-based technology from a practitioner's perspective. The aim of the critical review was to scope information available from published and unpublished research on the current state of palliative home-based technology, practitioner-focused perspectives, patient-focused perspectives, quality of life, and the implications for clinical practice. Published and unpublished studies were included. An example of one UK patient-centered home-based technology is explored as an exemplar. The evidence suggests that despite the challenges, there are numerous examples of good practice in relation to palliative home-based technology. Improvements in technology mean that telehealth has much to offer people being cared for at home with palliative needs. However, some of the evaluative evidence is limited, and further rigor is needed when evaluating future technology-based solutions innovations
