    What does it take to make integrated care work? A ‘cookbook’ for large-scale deployment of coordinated care and telehealth

    The Advancing Care Coordination & Telehealth Deployment (ACT) Programme is the first to explore the organisational and structural processes needed to successfully implement care coordination and telehealth (CC&TH) services on a large scale. A number of insights and conclusions were identified by the ACT programme. These will prove useful and valuable in supporting the large-scale deployment of CC&TH. Targeted at populations of chronic patients and elderly people, these insights and conclusions are a useful benchmark for implementing and exchanging best practices across the EU. Examples are: Perceptions between managers, frontline staff and patients do not always match; Organisational structure does influence the views and experiences of patients: a dedicated contact person is considered both important and helpful; Successful patient adherence happens when staff are engaged; There is a willingness by patients to participate in healthcare programmes; Patients overestimate their level of knowledge and adherence behaviour; The responsibility for adherence must be shared between patients and health care providers; Awareness of the adherence concept is an important factor for adherence promotion; The ability to track the use of resources is a useful feature of a stratification strategy, however, current regional case finding tools are difficult to benchmark and evaluate; Data availability and homogeneity are the biggest challenges when evaluating the performance of the programmes

    Biomedical research in a digital health framework

    This article describes a Digital Health Framework (DHF), benefitting from the lessons learnt during the three-year life span of the FP7 Synergy-COPD project. The DHF aims to embrace the emerging requirements--data and tools--of applying systems medicine into healthcare with a three-tier strategy articulating formal healthcare, informal care and biomedical research. Accordingly, it has been constructed based on three key building blocks, namely, novel integrated care services with the support of information and communication technologies, a personal health folder (PHF) and a biomedical research environment (DHF-research). Details on the functional requirements and necessary components of the DHF-research are extensively presented. Finally, the specifics of the building blocks strategy for deployment of the DHF, as well as the steps toward adoption are analyzed. The proposed architectural solutions and implementation steps constitute a pivotal strategy to foster and enable 4P medicine (Predictive, Preventive, Personalized and Participatory) in practice and should provide a head start to any community and institution currently considering to implement a biomedical research platform

    Information and communication technologies and patient empowerment: A complex skein

    This article aims to explore the relationship between patient empowerment and information and communication technologies (ICTs). Indeed, ICTs are considered important for increasing access to medical information and for patients\u2019 other experiences, thereby nourishing the empowering rhetoric. The paper presents a research study conducted in Italy that focuses on the self-assessments made by online health communities (OHCs) users, subdivided in three categories, according to their level of online activity: Lurkers, occasionally active users and frequently active users. The concept of empowerment was operationalised in five issues: autonomy from doctors, involvement in medical decision-making, competence, self-management and acceptance. The results support the relationship between perceptions of empowerment and the higher level of activity played on OHCs, contributing instead to reject the idea of a generalised benefit. Moreover, the paper aims to enrich the theory of patient empowerment by adding a socio-material perspective. This helps broaden the understanding of the relationship between empowerment and ICTs by highlighting its underlying complex skein

    Significant improvements in cancer research have led to more cancer patients being cured, and many more enabled to live with their cancer. As the disease is now managed as a chronic illness, it requires long-term surveillance and maintenance treatment. This requires a transformation in the nature of healthcare from reactive to preventive, personalized and participatory. To this direction, in this chapter we present relevant approaches developed within five European funded projects and we report on experiences and lessons learnt. More specifically, we describe the eHealth solutions developed, enabling patients to actively participate in their disease management, the results out of those projects towards the P5 vision and more especially on the participatory aspect and we present a set of requirements and guidelines for future technological solutions

    Life is the result of an intricate systemic interaction between many processes occurring at radically different spatial and temporal scales. Every day, worldwide biomedical research and clinical practice produce a huge amount of information on such processes. However, this information being highly fragmented, its integration is largely left to the human actors who find this task increasingly and ever more demanding in a context where the information available continues to increase exponentially. Investments in the Virtual Physiological Human (VPH) research are largely motivated by the need for integration in healthcare. As all health information becomes digital, the complexity of health care will continue to evolve, translating into an ever increasing pressure which will result from a growing demand in parallel to limited budgets. Hence, the best way to achieve the dream of personalised, preventive, and participative medicine at sustainable costs will be through the integration of all available data, information and knowledge
