La médecine générale perçue par les étudiants de la faculté de médecine de Sousse (Tunisie)
Introduction: Les Ă©tudiants en dĂ©but du cursus suivent la mĂȘme formation hospitalo-universitaire, cependant, ils sont peu nombreux Ă choisir dĂ©libĂ©rĂ©ment la mĂ©decine gĂ©nĂ©rale comme discipline d'exercice mĂ©dical. Notre objectif est de prĂ©ciser auprĂšs des Ă©tudiants en mĂ©decine, les facteurs qui dĂ©terminent le choix de la mĂ©decine gĂ©nĂ©rale comme carriĂšre et leur vision de cette discipline. MĂ©thodes: Etude descriptive transversale menĂ©e auprĂšs d'un Ă©chantillon d'Ă©tudiants inscrits Ă la facultĂ© de mĂ©decine de Sousse (Tunisie) pour l'annĂ©e universitaire 2012-2013. Le recueil des donnĂ©es a Ă©tĂ©Â pratiquĂ© par l'intermĂ©diaire d'un questionnaire conçu pour les fins de ce travail. La saisie et l'analyse des donnĂ©es ont Ă©tĂ© effectuĂ©es par le logiciel SPSS 18.0.RĂ©sultats: Notre Ă©tude a portĂ© sur 388 Ă©tudiants dont 69,5% Ă©taient du genre fĂ©minin. L'Ăąge moyen Ă©tait de 22,1 ± 2,8 ans. Cent Ă©tudiants (25,8%) avaient un parent cadre de santĂ© sans que cela n'ait une influence sur le choix de carriĂšre. A l'entrĂ©e Ă la facultĂ©, seulement 7,1% (n=27) voulaient faire de la mĂ©decine gĂ©nĂ©rale leur carriĂšre. Le changement de choix vers la mĂ©decine gĂ©nĂ©rale est statistiquement diffĂ©rent Ă l'entrĂ©e et Ă la sortie de la facultĂ© chez les internes finissants. Conclusion: Afin d'attirer davantage les Ă©tudiants vers la mĂ©decine gĂ©nĂ©rale, il serait judicieux d'adopter des changements majeurs touchant l'enseignement thĂ©orique et pratique et de modifier les conditions de travail en fonction des attentes des gĂ©nĂ©rations futures.Key words: MĂ©decine gĂ©nĂ©rale, enseignement mĂ©dical, choix de spĂ©cialit

De par son objet et de par la méthodologie employée, l'étude DEQUASUD (DEmarche QUalité et Accompagnement des Soins aux Usagers de Drogues) constitue une premiÚre en France.
S'attachant à évaluer la satisfaction retirée par les usagers de drogues et les médecins d'une prise en charge dans les cabinets de médecine générale du Réseau Micro-Structures d'Alsace (RMS) comparativement à la médecine générale classique, l'étude a aussi tenu compte des dépenses générées par ces deux stratégies d'accompagnement.Cette étude coût-utilité multi-critÚres multi-agents permet ainsi de rendre compte de la supériorité, en terme de coût-utilité, d'une prise en charge psycho-socio-sanitaire proposée par les micro-structures alsaciennes par rapport aux cabinets conventionnels. La pratique dâune spiromĂ©trie systĂ©matique au cabinet de mĂ©decine gĂ©nĂ©rale chez des fumeurs sans projet de sevrage tabagique permet-elle de faire progresser leur motivation ?
Introduction: Our aim was to explore whether performing a spirometry at GPâs office changes non motivated smokersâattitude toward smoking cessation.Method: In our interventional, prospective, before-after single-center study, allowed by ethics committee, we included 74 smokers older than 18, with no-intention to quit smoking, whatever they were visiting GP for. We gave them their spirometric results â FEV1/CV, lung age- and commented it. Nine months later, we called them for another assessment.Results: 56% were women with an average-age of 46,55, who smoked 26,26 pack-years. 82% of them had normal FEV1/VC but lung age was pathological among 38% of them. Nine months later, 61,11% increased their motivation to quit smoking. They smoked 10,94 cigarettes per day versus 13,33 at the inclusion (p=0,0254). Motivation increase was not statistically related to age, gender, previous smoking cessations, daily smoking, nicotine dependence or normal FEV1/VC (p>0,75) but significantly related to normal or abnormal lung age status (p0,75) mais significativement liĂ©e Ă la normalitĂ© - ou non- de l'Ăąge pulmonaire (p<0,03).Conclusion : Cette Ă©tude, la premiĂšre Ă s'intĂ©resser Ă l'impact motivationnel de la spiromĂ©trie rĂ©alisĂ©e au cabinet de mĂ©decine gĂ©nĂ©rale chez des fumeurs non-motivĂ©s, valorise notre mission de prĂ©vention, Ă©largit notre champ de compĂ©tences, apporte potentiellement un nouvel outil Ă la lutte anti-tabac et, dans tous les cas, constitue un travail exploratoire avant des Ă©tudes de plus grande envergure

What is the translation of HSCL-25 in Bulgarian. A consensus procedure by Delphi-round and Forward-Backward translation
Introduction: Family physicians (FPs) are the first port of call for depressive patients in developed countries. The multimorbidpatients over 50 years are especially at risk. Symptoms are difficult to identify owing to their interindividual andintercultural variations. Few diagnostic tools are adapted and used by FPs. Family Practice Depression and Multimorbidity(FPDM) study by European General Practice Research Network (EGPRN) aims to find a diagnostic tool for depression inprimary care and to undertake collaborative research throughout Europe. Previous steps of FPDM have found that theHopkins Symptom Checklist in 25-items (HSCL-25) was the most appropriate tool according to the criteria of effectiveness,reproducibility and ergonomics. Objective: This study aimed to translate HSCL-25 in Bulgarian while adapting its content to the cultural and linguistic characteristics ensuring that original meaning was preserved. Method: A Delphi method adapted for a Forward-Backward translation was used. The translation from English to Bulgarian was submitted by Delphi procedure to a panel of Bulgarian experts in primary care. Backward translation was performed with a blind back-translation principle. Results: The inclusion criteria of panel were followed. The Bulgarian translation was confirmed unanimously in one Delphi round. The Backward English translation was produced and agreed by the FPDMâs scientific committee. Discussion: The quality of the panel of experts FPs ensured a validated and reliable Bulgarian translation. The following step will consist in a cultural check to ensure that HSCL-25 is in total agreement with the Backward translation.Introduction : La dĂ©pression est une maladie chronique souvent diagnostiquĂ©e et traitĂ©e en soins primaires. What is the translation of HSCL-25 in Galician; A consensus procedure by Delphi-round and Forward-Backward translation
Introduction: General Practitioners (GPs) are the first port of call for depressive patients in developed countries. The multi-morbid patients over 50 years are more at risk. Inter-individual and intercultural variations may change the symptoms expression. Few diagnostic tools are adapted and used by GPs. Family Practice Depression and Multimorbidity (FPDM) study by European General Practice Research Network (EGPRN) aims to select a depression diagnostic tool in primary care to undertake collaborative research involving GPs and Psychiatrists throughout Europe. The two previous steps of FPDM found that the Hopkins Symptom Checklist in 25-items (HSCL-25) was the most appropriate tool according to effectiveness, reproducibility and ergonomics criteria, versus DSM. Objective: This study aimed to translate HSCL-25 in Galician, keeping its meaning. This translation must be understandable by practitioners and patients, according to Galician cultural and linguistic features. Method: A Delphi method adapted for a Forward-Backward translation was used. The Forward-translation from English to Galician was submitted by Delphi procedure to a panel of primary care experts. The Back-translation was performed following the blind back-translation principle. Results: The inclusion criteria of panel were followed. The Forward Galician translation was accepted after two rounds. English back-translation was produced blind. Discussion: The Forward-Backward translation by Delphi method was effective to translate the HSCL-25 to Galician. The experts panel quality ensured a validated and reliable Galician translation. The following step will consist in a cross-cultural check. Similarity of interpretability between the Original and the Back-translation will be analysed.Introduction : Les mĂ©decins gĂ©nĂ©ralistes europĂ©ens sont le premier recours des patients dĂ©pressifs.
