    Phase 1 Trial of Vaccination with Autologous Tumor Cells and Antisense Directed Against the Insulin Growth Factor Type 1 Receptor (IGF-1R AS ODN) in Patients with Recurrent Glioblastoma

    Background: Extending a previous Phase I study, we report the results of a second Phase I autologous tumor cell vaccination trial for patients with recurrent glioblastomas (IND 14379-101, NCT01550523). Methods: Following surgery, subjects were treated by 24 hour implantation in the rectus sheath of ten biodiffusion chambers containing irradiated autologous tumor cells and IGF-1R AS ODN with the objective of stimulating tumor immunity. Patients were monitored for safety, clinical and radiographic as well as immune responses. Results: There were no Grade 3 toxicities related to protocol treatment and overall median survival from initial diagnosis was 91.4 weeks. Two protocol survival cohorts with median survivals of 48.2 and 10 weeks were identified and predicted by our pre-treatment assessments of immune function, corroborated by post-vaccination pro-inflammatory cytokine profiles. Longer survival subjects had imaging findings including transient elevations in cerebral blood volume (rCBV) and sustained elevations of apparent diffusion coefficient (ADC) interpreted as transient hyperemia and cell loss. Conclusions: The vaccine paradigm was well-tolerated with a favorable median survival. Our data support this as a novel treatment paradigm that promotes anti-tumor immunity

    Private states, quantum data hiding and the swapping of perfect secrecy

    We derive a formal connection between quantum data hiding and quantum privacy, confirming the intuition behind the construction of bound entangled states from which secret bits can be extracted. We present three main results. First, we show how to simplify the class of private states and related states via reversible local operation and one-way communication. Second, we obtain a bound on the one-way distillable entanglement of private states in terms of restricted relative entropy measures, which is tight in many cases and shows that protocols for one-way distillation of key out of states with low distillable entanglement lead to the distillation of data hiding states. Third, we consider the problem of extending the distance of quantum key distribution with help of intermediate stations. In analogy to the quantum repeater, this paradigm has been called the quantum key repeater. We show that when extending private states with one-way communication, the resulting rate is bounded by the one-way distillable entanglement.     Handling Non-deterministic Data Availability in Parallel Query Execution.

    The situation of non-deterministic data availability, where it is not known a priori which of two or more processes will respond first, cannot be handled with standard techniques. The consequence is sub-optimal processing because of inefficient resource allocation and unnecessary delays. In this paper we develop an effective solution to the problem by extending the demand-driven evaluation paradigm to the end of using operators with more than just one output stream. We show how inter-process communication and non-deterministic data availability in parallel query processing reduce to cases that can be executed efficiently with the new evaluation paradigm

    Distribution of the Object Oriented Databases. A Viewpoint of the MVDB Model's Methodology and Architecture

    In databases, much work has been done towards extending models with advanced tools such as view technology, schema evolution support, multiple classification, role modeling and viewpoints. Over the past years, most of the research dealing with the object multiple representation and evolution has proposed to enrich the monolithic vision of the classical object approach in which an object belongs to one hierarchy class. In particular, the integration of the viewpoint mechanism to the conventional object-oriented data model gives it flexibility and allows one to improve the modeling power of objects. The viewpoint paradigm refers to the multiple descriptions, the distribution, and the evolution of object. Also, it can be an undeniable contribution for a distributed design of complex databases. The motivation of this paper is to define an object data model integrating viewpoints in databases and to present a federated database architecture integrating multiple viewpoint sources following a local-as-extended-view data integration approach.object-oriented data model, OQL language, LAEV data integration approach, MVDB model, federated databases, Local-As-View Strategy.

    Employment outcomes in people with bipolar disorder : a systematic review

    Objective: Employment outcome in bipolar disorder is an under investigated, but important area. The aim of this study was to identify the long-term employment outcomes of people with bipolar disorder. Method: A systematic review using the Medline, PsychInfo and Web of Science databases. Results: Of 1962 abstracts retrieved, 151 full text papers were read. Data were extracted from 25 papers representing a sample of 4892 people with bipolar disorder and a mean length of follow-up of 4.9 years. Seventeen studies had follow-up periods of up to 4 years and eight follow-up of 5–15 years. Most studies with samples of people with established bipolar disorder suggest approximately 40–60% of people are in employment. Studies using work functioning measures mirrored this result. Bipolar disorder appears to lead to workplace underperformance and 40–50% of people may suffer a slide in their occupational status over time. Employment levels in early bipolar disorder were higher than in more established illness. Conclusion: Bipolar disorder damages employment outcome in the longer term, but up to 60% of people may be in employment. Whilst further studies are necessary, the current evidence provides support for extending the early intervention paradigm to bipolar disorder
