    Conceptualizing pathways linking women's empowerment and prematurity in developing countries.

    BackgroundGlobally, prematurity is the leading cause of death in children under the age of 5. Many efforts have focused on clinical approaches to improve the survival of premature babies. There is a need, however, to explore psychosocial, sociocultural, economic, and other factors as potential mechanisms to reduce the burden of prematurity. Women's empowerment may be a catalyst for moving the needle in this direction. The goal of this paper is to examine links between women's empowerment and prematurity in developing settings. We propose a conceptual model that shows pathways by which women's empowerment can affect prematurity and review and summarize the literature supporting the relationships we posit. We also suggest future directions for research on women's empowerment and prematurity.MethodsThe key words we used for empowerment in the search were "empowerment," "women's status," "autonomy," and "decision-making," and for prematurity we used "preterm," "premature," and "prematurity." We did not use date, language, and regional restrictions. The search was done in PubMed, Population Information Online (POPLINE), and Web of Science. We selected intervening factors-factors that could potentially mediate the relationship between empowerment and prematurity-based on reviews of the risk factors and interventions to address prematurity and the determinants of those factors.ResultsThere is limited evidence supporting a direct link between women's empowerment and prematurity. However, there is evidence linking several dimensions of empowerment to factors known to be associated with prematurity and outcomes for premature babies. Our review of the literature shows that women's empowerment may reduce prematurity by (1) preventing early marriage and promoting family planning, which will delay age at first pregnancy and increase interpregnancy intervals; (2) improving women's nutritional status; (3) reducing domestic violence and other stressors to improve psychological health; and (4) improving access to and receipt of recommended health services during pregnancy and delivery to help prevent prematurity and improve survival of premature babies.ConclusionsWomen's empowerment is an important distal factor that affects prematurity through several intervening factors. Improving women's empowerment will help prevent prematurity and improve survival of preterm babies. Research to empirically show the links between women's empowerment and prematurity is however needed

    Axiomatic Interpretability for Multiclass Additive Models

    Generalized additive models (GAMs) are favored in many regression and binary classification problems because they are able to fit complex, nonlinear functions while still remaining interpretable. In the first part of this paper, we generalize a state-of-the-art GAM learning algorithm based on boosted trees to the multiclass setting, and show that this multiclass algorithm outperforms existing GAM learning algorithms and sometimes matches the performance of full complexity models such as gradient boosted trees. In the second part, we turn our attention to the interpretability of GAMs in the multiclass setting. Surprisingly, the natural interpretability of GAMs breaks down when there are more than two classes. Naive interpretation of multiclass GAMs can lead to false conclusions. Inspired by binary GAMs, we identify two axioms that any additive model must satisfy in order to not be visually misleading. We then develop a technique called Additive Post-Processing for Interpretability (API), that provably transforms a pre-trained additive model to satisfy the interpretability axioms without sacrificing accuracy. The technique works not just on models trained with our learning algorithm, but on any multiclass additive model, including multiclass linear and logistic regression.     Number Processing in Infants and Children Born Very Preterm

    Individuals born very preterm (<32 weeks; VP) have notably poorer attainment in mathematics than their term-born peers. Only a handful of studies have investigated basic numerical skills in VP children and the underlying mechanisms associated with problems with mathematics in this population are still not fully comprehended. Basic processes underlying numerical cognition can go awry very early in development and there is a lack of knowledge of early trajectories of acquisition of numerical skills in infants born prematurely. This thesis reports on a series of studies investigating number processing in very preterm infants and children. These make use of a combination of tools, such as neurodevelopmental assessments, eye-tracking, event-related-potentials, neuropsychological evaluations and experimental tasks. Specifically, cross-sectional studies investigated numerical sensitivity in VP infants aged six and twelve months. Behavioural and electrophysiological measures assessing a range of domain-general and domain-specific skills associated with mathematics performance were also investigated in VP school-aged children. The results showed that, during the first year of post-natal life, VP infants do not exhibit differential developmental trajectories in the basic ability to discriminate numerosities compared to infants born at full term, although they required a longer time to discriminate the new number of elements. Later in development, school-aged VP children demonstrated difficulties in processing basic numerical information. Electrophysiological data demonstrated that this might be associated with deficits in sensory and attention resources and not necessarily in how VP children encode number-related information. Difficulties in processing numerical information, however, have only a marginal impact on their performance in mathematics. We tentatively conclude that difficulties in mathematics in individuals born very prematurely are largely associated with domain-general skills

    Severe birth prematurity : a case study of a boy’s psychoeducational support needs

    Abstract: This study conducted research related to severe premature birth and the neurodevelopmental disorders that may be associated with it. Severely premature born children, born before the gestational age of 28 weeks, are at an increased risk of developmental delays. These delays may manifest as physical difficulties, learning disabilities, attention deficits, hyperactivity, and behavioural and social problems. Learners with barriers to learning present challenges for teachers due to these developmental delays and comorbid disorders associated with premature birth. The challenges are further intensified by large class sizes, and limited resources. Teachers are generally ill-equipped to meet the demands of the learners in an inclusive situation, in diverse classrooms. It is difficult to differentiate the content and the pace of the curriculum. This study explored the psycho-educational support needs of Lebo (pseudonym), who was born severely premature.     Predictability of catastrophic events: material rupture, earthquakes, turbulence, financial crashes and human birth

    We propose that catastrophic events are "outliers" with statistically different properties than the rest of the population and result from mechanisms involving amplifying critical cascades.     An automatic method to generate domain-specific investigator networks using PubMed abstracts

    <p>Abstract</p> <p>Background</p> <p>Collaboration among investigators has become critical to scientific research. This includes ad hoc collaboration established through personal contacts as well as formal consortia established by funding agencies. Continued growth in online resources for scientific research and communication has promoted the development of highly networked research communities. Extending these networks globally requires identifying additional investigators in a given domain, profiling their research interests, and collecting current contact information. We present a novel strategy for building investigator networks dynamically and producing detailed investigator profiles using data available in PubMed abstracts.</p> <p>Results</p> <p>We developed a novel strategy to obtain detailed investigator information by automatically parsing the affiliation string in PubMed records. We illustrated the results by using a published literature database in human genome epidemiology (HuGE Pub Lit) as a test case. Our parsing strategy extracted country information from 92.1% of the affiliation strings in a random sample of PubMed records and in 97.0% of HuGE records, with accuracies of 94.0% and 91.0%, respectively. Institution information was parsed from 91.3% of the general PubMed records (accuracy 86.8%) and from 94.2% of HuGE PubMed records (accuracy 87.0). We demonstrated the application of our approach to dynamic creation of investigator networks by creating a prototype information system containing a large database of PubMed abstracts relevant to human genome epidemiology (HuGE Pub Lit), indexed using PubMed medical subject headings converted to Unified Medical Language System concepts. Our method was able to identify 70–90% of the investigators/collaborators in three different human genetics fields; it also successfully identified 9 of 10 genetics investigators within the PREBIC network, an existing preterm birth research network.</p> <p>Conclusion</p> <p>We successfully created a web-based prototype capable of creating domain-specific investigator networks based on an application that accurately generates detailed investigator profiles from PubMed abstracts combined with robust standard vocabularies. This approach could be used for other biomedical fields to efficiently establish domain-specific investigator networks.</p

    Etiology of persistent mathematics difficulties from childhood to adolescence following very preterm birth

    Children born very preterm (VP; 0.05), with the exception of writing large numbers and conceptual understanding of arithmetic. In a previous study, 83 of the VP adolescents and 49 of the term-born adolescents were assessed at age 8–10years using measures of the same skills. Amongst these, the between-group difference in mathematics achievement remained stable over time. This study extends findings of a persistent deficit in mathematics achievement among VP children over the primary and secondary school years, and provides evidence of a deficit in factual, procedural and conceptual mathematics skills and in higher order mathematical operations among VP adolescents. We provide further evidence that VP children’s mathematics difficulties are driven by deficits in domain-general rather than domain-specific cognitive skills

    One of Australia’s greatest challenges is the elimination of the gap between the developmental outcomes of Indigenous and non-Indigenous children in the early years of life. This paper reviews existing research and presents strategies to improve early childhood development among Indigenous Australians.     Clinical information extraction for preterm birth risk prediction

    This paper contributes to the pursuit of leveraging unstructured medical notes to structured clinical decision making. In particular, we present a pipeline for clinical information extraction from medical notes related to preterm birth, and discuss the main challenges as well as its potential for clinical practice. A large collection of medical notes, created by staff during hospitalizations of patients who were at risk of delivering preterm, was gathered and analyzed. Based on an annotated collection of notes, we trained and evaluated information extraction components to discover clinical entities such as symptoms, events, anatomical sites and procedures, as well as attributes linked to these clinical entities. In a retrospective study, we show that these are highly informative for clinical decision support models that are trained to predict whether delivery is likely to occur within specific time windows, in combination with structured information from electronic health records
