A cross-sectional study of vascular risk factors in a rural South African population : data from the Southern African Stroke Prevention Initiative (SASPI)
Background: Rural sub-Saharan Africa is at an early stage of economic and health transition. It is
predicted that the 21st century will see a serious added economic burden from non-communicable disease
including vascular disease in low-income countries as they progress through the transition. The stage of
vascular disease in a population is thought to result from the prevalence of vascular risk factors. Already
hypertension and stroke are common in adults in sub-Saharan Africa. Using a multidisciplinary approach
we aimed to assess the prevalence of several vascular risk factors in Agincourt, a rural demographic
surveillance site in South Africa.
Methods: We performed a cross sectional random sample survey of adults aged over 35 in Agincourt
(population ≈ 70 000). Participants were visited at home by a trained nurse who administered a
questionnaire, carried out clinical measurements and took a blood sample. From this we assessed
participants' history of vascular risk, blood pressure using an OMRON 705 CP monitor, waist
circumference, body mass index (BMI), ankle brachial index (ABI), and total and HDL cholesterol.
Results: 402 people (24% men) participated. There was a high prevalence of smoking in men, but the
number of cigarettes smoked was small. There was a striking difference in mean BMI between men and
women (22.8 kg/m2 versus 27.2 kg/m2), but levels of blood pressure were very similar. 43% of participants
had a blood pressure greater than 140/90 or were on anti-hypertensive treatment and 37% of participants
identified with measured high blood pressure were on pharmacological treatment. 12% of participants had
an ABI of < 0.9, sugesting the presence of sub-clinical atheroma. 25.6% of participants had a total
cholesterol level > 5 mmol/l.
Conclusion: We found a high prevalence of hypertension, obesity in women, and a suggestion of
subclinical atheroma despite relatively favourable cholesterol levels in a rural South African population.
South Africa is facing the challenge of an emerging epidemic of vascular disease. Research to establish the
social determinates of these risk factors and interventions to reduce both individual and population risk
are required
An international comparative study of blood pressure in populations of European vs. African descent
Background: The consistent finding of higher prevalence of hypertension in US blacks compared
to whites has led to speculation that African-origin populations are particularly susceptible to this
condition. Large surveys now provide new information on this issue.
Methods: Using a standardized analysis strategy we examined prevalence estimates for 8 white
and 3 black populations (N = 85,000 participants).
Results: The range in hypertension prevalence was from 27 to 55% for whites and 14 to 44% for
blacks.
Conclusions: These data demonstrate that not only is there a wide variation in hypertension
prevalence among both racial groups, the rates among blacks are not unusually high when viewed
internationally. These data suggest that the impact of environmental factors among both
populations may have been under-appreciated
The epidemiology of chronic kidney disease (CKD) in rural East Africa: A population-based study.
BackgroundChronic kidney disease (CKD) may be common among individuals living in sub-Saharan Africa due to the confluence of CKD risk factors and genetic predisposition.MethodsWe ascertained the prevalence of CKD and its risk factors among a sample of 3,686 participants of a population-based HIV trial in rural Uganda and Kenya. Prevalent CKD was defined as a serum creatinine-based estimated glomerular filtration rate <60 mL/min/1.73m2 or proteinuria (urine dipstick ≥1+). We used inverse-weighting to estimate the population prevalence of CKD, and multivariable log-link Poisson models to assess the associations of potential risk factors with CKD.ResultsThe estimated CKD prevalence was 6.8% (95% CI 5.7-8.1%) overall and varied by region, being 12.5% (10.1-15.4%) in eastern Uganda, 3.9% (2.2-6.8%) in southwestern Uganda and 3.7% (2.7-5.1%) in western Kenya. Risk factors associated with greater CKD prevalence included age ≥60 years (adjusted prevalence ratio [aPR] 3.5 [95% CI 1.9-6.5] compared with age 18-29 years), HIV infection (aPR 1.6 [1.1-2.2]), and residence in eastern Uganda (aPR 3.9 [2.6-5.9]). However, two-thirds of individuals with CKD did not have HIV, diabetes, or hypertension as risk factors. Furthermore, we noted many individuals who did not have proteinuria had dipstick positive leukocyturia or hematuria.ConclusionThe prevalence of CKD is appreciable in rural East Africa and there are considerable regional differences. Conventional risk factors appear to only explain a minority of cases, and leukocyturia and hematuria were common, highlighting the need for further research into understanding the nature of CKD in sub-Saharan Africa
Google matrix of the world trade network
Using the United Nations Commodity Trade Statistics Database
[http://comtrade.un.org/db/] we construct the Google matrix of the world trade
network and analyze its properties for various trade commodities for all
countries and all available years from 1962 to 2009. The trade flows on this
network are classified with the help of PageRank and CheiRank algorithms
developed for the World Wide Web and other large scale directed networks. For
the world trade this ranking treats all countries on equal democratic grounds
independent of country richness. Still this method puts at the top a group of
industrially developed countries for trade in {\it all commodities}. Our study
establishes the existence of two solid state like domains of rich and poor
countries which remain stable in time, while the majority of countries are
shown to be in a gas like phase with strong rank fluctuations. A simple random
matrix model provides a good description of statistical distribution of
countries in two-dimensional rank plane. The comparison with usual ranking by

Metrics to evaluate research performance in academic institutions: A critique of ERA 2010 as applied in forestry and the indirect H2 index as a possible alternative
Excellence for Research in Australia (ERA) is an attempt by the Australian
Research Council to rate Australian universities on a 5-point scale within 180
Fields of Research using metrics and peer evaluation by an evaluation
committee. Some of the bibliometric data contributing to this ranking suffer
statistical issues associated with skewed distributions. Other data are
standardised year-by-year, placing undue emphasis on the most recent
publications which may not yet have reliable citation patterns. The
bibliometric data offered to the evaluation committees is extensive, but lacks
effective syntheses such as the h-index and its variants. The indirect H2 index
is objective, can be computed automatically and efficiently, is resistant to
manipulation, and a good indicator of impact to assist the ERA evaluation

A community programme to reduce salt intake and blood pressure in Ghana
Background
In Africa hypertension is common and stroke is increasing. Detection, treatment and control of high blood pressure (BP) is limited. BP can be lowered by reducing salt intake. In Africa salt is added to the food by the consumer, as processed food is rare. A population-wide approach with programmes based on health education and promotion is thus possible.
Methods
We carried out a community-based cluster randomised trial of health promotion in 1,013 participants from 12 villages (628 women, 481 rural dwellers); mean age 55 years to reduce salt intake and BP. Average BP was 125/74 mmHg and urinary sodium (UNa) 101 mmol/day. A health promotion intervention was provided over 6 months to all villages. Assessments were made at 3 and 6 months. Primary end-points were urinary sodium excretion and BP levels.
Results
There was a significant positive relationship between salt intake and both systolic (2.17 mmHg [95% CI 0.44 to 3.91] per 50 mmol of UNa per day, p < 0.001) and diastolic BP (1.10 mmHg [0.08 to 1.94], p < 0.001) at baseline. At six months the intervention group showed a reduction in systolic (2.54 mmHg [-1.45 to 6.54]) and diastolic (3.95 mmHg [0.78 to 7.11], p = 0.015) BP when compared to control. There was no significant change in UNa. Smaller villages showed greater reductions in UNa than larger villages (p = 0.042). Irrespective of randomisation, there was a consistent and significant relationship between change in UNa and change in systolic BP, when adjusted for confounders. A difference in 24-hour UNa of 50 mmol was associated with a lower systolic BP of 2.12 mmHg (1.03 to 3.21) at 3 months and 1.34 mmHg (0.08 to 2.60) at 6 months (both p < 0.001).
Conclusion
In West Africa the lower the salt intake, the lower the BP. It would appear that a reduction in the average salt intake in the whole community may lead to a small but significant reduction in population systolic BP
The structure of the Arts & Humanities Citation Index: A mapping on the basis of aggregated citations among 1,157 journals
Using the Arts & Humanities Citation Index (A&HCI) 2008, we apply mapping
techniques previously developed for mapping journal structures in the Science
and Social Science Citation Indices. Citation relations among the 110,718
records were aggregated at the level of 1,157 journals specific to the A&HCI,
and the journal structures are questioned on whether a cognitive structure can
be reconstructed and visualized. Both cosine-normalization (bottom up) and
factor analysis (top down) suggest a division into approximately twelve
subsets. The relations among these subsets are explored using various
visualization techniques. However, we were not able to retrieve this structure
using the ISI Subject Categories, including the 25 categories which are
specific to the A&HCI. We discuss options for validation such as against the
categories of the Humanities Indicators of the American Academy of Arts and
Sciences, the panel structure of the European Reference Index for the
Humanities (ERIH), and compare our results with the curriculum organization of
the Humanities Section of the College of Letters and Sciences of UCLA as an
example of institutional organization
Prospects for the development of odour baits to control the tsetse flies Glossina tachinoides and G. palpalis s.l.
Field studies were done of the responses of Glossina palpalis palpalis in Côte d'Ivoire, and G. p. gambiensis and G. tachinoides in Burkina Faso, to odours from humans, cattle and pigs. Responses were measured either by baiting (1.) biconical traps or (2.) electrocuting black targets with natural host odours. The catch of G. tachinoides from traps was significantly enhanced (~5×) by odour from cattle but not humans. In contrast, catches from electric targets showed inconsistent results. For G. p. gambiensis both human and cattle odour increased (>2×) the trap catch significantly but not the catch from electric targets. For G. p. palpalis, odours from pigs and humans increased (~5×) the numbers of tsetse attracted to the vicinity of the odour source but had little effect on landing or trap-entry. For G. tachinoides a blend of POCA (P = 3-n-propylphenol; O = 1-octen-3-ol; C = 4-methylphenol; A = acetone) alone or synthetic cattle odour (acetone, 1-octen-3-ol, 4-methylphenol and 3-n-propylphenol with carbon dioxide) consistently caught more tsetse than natural cattle odour. For G. p. gambiensis, POCA consistently increased catches from both traps and targets. For G. p. palpalis, doses of carbon dioxide similar to those produced by a host resulted in similar increases in attraction. Baiting traps with super-normal (~500 mg/h) doses of acetone also consistently produced significant but slight (~1.6×) increases in catches of male flies. The results suggest that odour-baited traps and insecticide-treated targets could assist the AU-Pan African Tsetse and Trypanosomiasis Eradication Campaign (PATTEC) in its current efforts to monitor and control Palpalis group tsetse in West Africa. For all three species, only ~50% of the flies attracted to the vicinity of the trap were actually caught by it, suggesting that better traps might be developed by an analysis of the visual responses and identification of any semiochemicals involved in short-range interaction
Integrating gender into index-based agricultural insurance: a focus on South Africa
Index insurance is an agricultural risk management tool that can provide a safety net for smallholder farmers experiencing climate risk. While uptake and scale-out of index insurance may be slow among smallholders, we can learn from experiences that demonstrate where crop insurance can protect smallholders’ livelihoods from climate risk. Integrating gender into climate risk management is necessary to ensure that the benefits of index insurance are experienced by both men and women. A dedicated intention to integrate gender may be required. Taking South Africa as a case study, the potential for gender-sensitive index insurance scale-out among smallholders is investigated
