    Integrating patients' views into health technology assessment: Analytic hierarchy process (AHP) as a method to elicit patient preferences

    Background: Patient involvement is widely acknowledged to be a valuable component in health technology assessment (HTA) and healthcare decision making. However, quantitative approaches to ascertain patients' preferences for treatment endpoints are not yet established. The objective of this study is to introduce the analytic hierarchy process (AHP) as a preference elicitation method in HTA. Based on a systematic literature review on the use of AHP in health care in 2009, the German Institute for Quality and Efficiency in Health Care (IQWiG) initiated an AHP study related to its HTA work in 2010. - \ud Methods: The AHP study included two AHP workshops, one with twelve patients and one with seven healthcare professionals. In these workshops, both patients and professionals rated their preferences with respect to the importance of different endpoints of antidepressant treatment by a pairwise comparison of individual endpoints. These comparisons were performed and evaluated by the AHP method and relative weights were generated for each endpoint. - \ud Results: The AHP study indicates that AHP is a well-structured technique whose cognitive demands were well handled by patients and professionals. The two groups rated some of the included endpoints of antidepressant treatment differently. For both groups, however, the same six of the eleven endpoints analyzed accounted for more than 80 percent of the total weight. - \ud Conclusions: AHP can be used in HTA to give a quantitative dimension to patients' preferences for treatment endpoints. Preference elicitation could provide important information at various stages of HTA and challenge opinions on the importance of endpoints

Enerzijds wordt teruggekeken, maar er wordt vooral ook vooruitgeblikt. De levensverwachting neemt in Nederland naar verwachting toe tot 88,1 jaar voor vrouwen en 83,8 jaar voor mannen in 2050. Deze extra jaren gaan echter gepaard met een toename van chronische ouderdomsziekten en beperkingen. Mede hierdoor zal het zorgvolume tot 2030 met gemiddeld 3,4 procent per jaar stijgen. De overheid zal de knelpunten in de financiering en de beschikbaarheid van voldoende zorgpersoneel onder ogen moeten gezien.Not availableVW

    Does AHP help us make a choice? - An experimental evaluation

    In this paper, we use experimental economics methods to test how well Analytic Hierarchy Process (AHP) fares as a choice support system in a real decision problem. AHP provides a ranking that we statistically compare with three additional rankings given by the subjects in the experiment: one at the beginning, one after providing AHP with the necessary pair-wise comparisons and one after learning the ranking provided by AHP. While the rankings vary widely across subjects, we observe that for each individual all four rankings are similar. Hence, subjects are consistent and AHP is, for the most part, able to replicate their rankings. Furthermore, while the rankings are similar, we do find that the AHP ranking helps the decision-makers reformulate their choices by taking into account suggestions made by AHP.Decision analysis, Multiple Criteria Decision Aid, Analytic Hierarchy Process (AHP)

    The past and future of the AHP in health care decision making

    Objective. Health care decision making is a complex process involving many stakeholders and allowing for multiple decision criteria. The Analytic Hierarchy process (AHP) can support these complex decisions that relate to the application and coverage of health care technologies. The objective of this study is to review the past applications of the AHP in supporting health care decision making, and to make recommendations for its future use.\ud Method. We conducted a systematic review of AHP applications in health care, as described in the relevant medical, health-economical, psycho-sociological, managerial, and applied mathematical literature.\ud Results. We found 62 distinctive AHP applications in health care. Of the retrieved applications, 13 % focus on shared decision-making between patient and clinician, 27 % on the development of clinical practice guidelines, 5 % on the development of medical devices and pharmaceuticals, 40 % on management decisions in health care organizations, and 15 % on the development of national health care policy.\ud Conclusions. From the review it is concluded that the AHP is suitable to apply in case of complex health care decision problems, a need to improve decision making in stead of explain decision outcomes, a need to share information among experts or between clinicians and patients, and in case of a limited availability of informed respondents. We foresee the increased use of the AHP in health economical assessment of technology

    The Analytic Hierarchy Process (AHP) is widely used for decision making involving multiple criteria. Elsner and van den Driessche introduced a max-algebraic approach to the single criterion AHP. We extend this to the multi-criteria AHP, by considering multi-objective generalisations of the single objective optimisation problem solved in these earlier papers. We relate the existence of globally optimal solutions to the commutativity properties of the associated matrices; we relate min-max optimal solutions to the generalised spectral radius; and we prove that Pareto optimal solutions are guaranteed to exist.Comment: 1 figur

    On Axiomatization of Inconsistency Indicators for Pairwise Comparisons

    We examine the notion of inconsistency in pairwise comparisons and propose an axiomatization which is independent of any method of approximation or the inconsistency indicator definition (e.g., Analytic Hierarchy Process, AHP). It has been proven that the eigenvalue-based inconsistency (proposed as a part of AHP) is incorrect.Comment: Enhanced text, with 21 pages and 3 figures, proves that arbitrarily inaccurate pairwise matrices are considered acceptable by theories with a inconsistency based on the principal eigenvalue (e.g., AHP).     Projective Market Model Approach to AHP Decision-Making

    In this paper we describe market in projective geometry language and give definition of a matrix of market rate, which is related to the matrix rate of return and the matrix of judgements in the Analytic Hierarchy Process (AHP). We use these observations to extend the AHP model to projective geometry formalism and generalise it to intransitive case. We give financial interpretations of such generalised model and propose its simplification.     Decision support for information systems management : applying analytic hierarchy process

    Decision-making in the field of information systems has become more complex due to a larger number of alternatives, multiple and sometimes conflicting goals, and an increasingly turbulent environment. In this paper we explore the appropriateness of Analytic Hierarchy Process to support I/S decision making. AHP can be applied if the decision problem includes multiple objectives, conflicting criteria, incommensurable units, and aims at selecting an alternative from a known set of alternatives. An AHP analysis is described by using the project selection decision as an example. The strengths and weaknesses of AHP are investigated based on a set of eight criteria for evaluating I/S decision support methods. This evaluation shows that AHP scores well on most criteria. Given this promising performance, other possible applications of AHP within the I/S function are listed.
