Lexical patterns, features and knowledge resources for coreference resolution in clinical notes
Generation of entity coreference chains provides a means to extract linked narrative events from clinical notes, but despite being a well-researched topic in natural language processing, general- purpose coreference tools perform poorly on clinical texts. This paper presents a knowledge-centric and pattern-based approach to resolving coreference across a wide variety of clinical records comprising discharge summaries, progress notes, pathology, radiology and surgical reports from two corpora (Ontology Development and Information Extraction (ODIE) and i2b2/VA). In addition, a method for generating coreference chains using progressively pruned linked lists is demonstrated that reduces the search space and facilitates evaluation by a number of metrics. Independent evaluation results show an F-measure for each corpus of 79.2% and 87.5%, respectively, which offers performance at least as good as human annotators, greatly increased performance over general- purpose tools, and improvement on previously reported clinical coreference systems. Coreference resolution in clinical discharge summaries, progress notes, surgical and pathology reports: a unified lexical approach
We developed a lexical rule-based system that uses a unified approach to resolving coreference across a wide variety of clinical records comprising discharge summaries, progress notes, pathology, radiology and surgical reports from two corpora (Ontology Development and Information Extraction (ODIE) and i2b2/VA) provided for the fifth i2b2/VA shared task. Taking the unweighted mean between 4 coreference metrics, validation of the system against the i2b2/VA corpus attained an overall F-score of 87.7% across all mention classes, with a maximum of 93.1% for coreference of persons, and a minimum of 77.2% for coreference of tests. For the ODIE corpus the overall F-score across all mention classes was 79.4%, with a maximum of 82.0% for coreference of persons and a minimum of 13.1% for coreference of diagnostic reagents. For the ODIE corpus our results are comparable to the mean reported inter-annotator agreement with the gold standard. We discuss the four categories of errors we identified, and how these might be addressed. The system uses a number of reusable modules and techniques that may be of benefit to the research community
An evaluation of the performance of three semantic background knowledge sources in comparative anatomy
In this paper we evaluate the performance and usefulness of three semantic background knowledge sources for predicting synonymous anatomical terms across species boundaries. The reference sources under evaluation are UMLS, FMA-OBO and WordNet, which are applied to the anatomical ontologies of mouse and zebrafish. Our results show that the use of specialized knowledge sources leads to highly accurate predictions, verified through complete manual curation, which can be further improved by combining multiple of said sources. We argue that these three references complement each other in terms of granularity and specificity. From our results we conclude that these references can be used to create reliable ontology mappings with minimal human supervision
Utilizing RxNorm to Support Practical Computing Applications: Capturing Medication History in Live Electronic Health Records
RxNorm was utilized as the basis for direct-capture of medication history
data in a live EHR system deployed in a large, multi-state outpatient
behavioral healthcare provider in the United States serving over 75,000
distinct patients each year across 130 clinical locations. This tool
incorporated auto-complete search functionality for medications and proper
dosage identification assistance. The overarching goal was to understand if and
how standardized terminologies like RxNorm can be used to support practical
computing applications in live EHR systems. We describe the stages of
implementation, approaches used to adapt RxNorm's data structure for the
intended EHR application, and the challenges faced. We evaluate the
implementation using a four-factor framework addressing flexibility, speed,
data integrity, and medication coverage. RxNorm proved to be functional for the
intended application, given appropriate adaptations to address high-speed
input/output (I/O) requirements of a live EHR and the flexibility required for
data entry in multiple potential clinical scenarios. Future research around
search optimization for medication entry, user profiling, and linking RxNorm to
drug classification schemes holds great potential for improving the user
experience and utility of medication data in EHRs.Comment: Appendix (including SQL/DDL Code) available by author request.

A Relation Extraction Approach for Clinical Decision Support
In this paper, we investigate how semantic relations between concepts
extracted from medical documents can be employed to improve the retrieval of
medical literature. Semantic relations explicitly represent relatedness between
concepts and carry high informative power that can be leveraged to improve the
effectiveness of retrieval functionalities of clinical decision support
systems. We present preliminary results and show how relations are able to
provide a sizable increase of the precision for several topics, albeit having
no impact on others. We then discuss some future directions to minimize the

Profiling risk factors for chronic uveitis in juvenile idiopathic arthritis: a new model for EHR-based research.
BackgroundJuvenile idiopathic arthritis is the most common rheumatic disease in children. Chronic uveitis is a common and serious comorbid condition of juvenile idiopathic arthritis, with insidious presentation and potential to cause blindness. Knowledge of clinical associations will improve risk stratification. Based on clinical observation, we hypothesized that allergic conditions are associated with chronic uveitis in juvenile idiopathic arthritis patients.MethodsThis study is a retrospective cohort study using Stanford's clinical data warehouse containing data from Lucile Packard Children's Hospital from 2000-2011 to analyze patient characteristics associated with chronic uveitis in a large juvenile idiopathic arthritis cohort. Clinical notes in patients under 16Â years of age were processed via a validated text analytics pipeline. Bivariate-associated variables were used in a multivariate logistic regression adjusted for age, gender, and race. Previously reported associations were evaluated to validate our methods. The main outcome measure was presence of terms indicating allergy or allergy medications use overrepresented in juvenile idiopathic arthritis patients with chronic uveitis. Residual text features were then used in unsupervised hierarchical clustering to compare clinical text similarity between patients with and without uveitis.ResultsPreviously reported associations with uveitis in juvenile idiopathic arthritis patients (earlier age at arthritis diagnosis, oligoarticular-onset disease, antinuclear antibody status, history of psoriasis) were reproduced in our study. Use of allergy medications and terms describing allergic conditions were independently associated with chronic uveitis. The association with allergy drugs when adjusted for known associations remained significant (OR 2.54, 95% CI 1.22-5.4).ConclusionsThis study shows the potential of using a validated text analytics pipeline on clinical data warehouses to examine practice-based evidence for evaluating hypotheses formed during patient care. Our study reproduces four known associations with uveitis development in juvenile idiopathic arthritis patients, and reports a new association between allergic conditions and chronic uveitis in juvenile idiopathic arthritis patients
