    Testing the surface fixation method in gestational diabetes mellitus

    Introduction: To test the surface fixation method contrasting urine samples of women with GDM vs healthy pregnant women. Methods: This was a pilot descriptive study. Three groups were conformed: A) Pregnant women with GDM, B) Women with healthy pregnancies and C) Non-pregnant healthy women. The positiveness of the surface fixation method was contrasted with Odds Ratio. Results: 12 women with GDM, 14 with healthy pregnancies and 9 non-pregnant women were included in the study. The OR for a positive surface fixation test when contrasting GDM vs Healthy pregnancies was of 2.7 while the value when contrasting GDM vs Healthy pregnancies + Non pregnant women was of 3.2 without reaching significant statistical difference in any case. Conclusion: the surface fixation method used with urine samples, suggests the existence of a transient antigen-antibody reaction that contributes to the inefficient insulin secretion

    Reliability of Rapid Diagnostic Tests in Diagnosing Pregnancy-Associated Malaria in North-Eastern Tanzania.

    Accurate diagnosis and prompt treatment of pregnancy-associated malaria (PAM) are key aspects in averting adverse pregnancy outcomes. Microscopy is the gold standard in malaria diagnosis, but it has limited detection and availability. When used appropriately, rapid diagnostic tests (RDTs) could be an ideal diagnostic complement to microscopy, due to their ease of use and adequate sensitivity in detecting even sub-microscopic infections. Polymerase chain reaction (PCR) is even more sensitive, but it is mainly used for research purposes. The accuracy and reliability of RDTs in diagnosing PAM was evaluated using microscopy and PCR. A cohort of pregnant women in north-eastern Tanzania was followed throughout pregnancy for detection of plasmodial infection using venous and placental blood samples evaluated by histidine rich protein 2 (HRP-2) and parasite lactate dehydrogenase (pLDH) based RDTs (Parascreen™) or HRP-2 only (Paracheck Pf® and ParaHIT®f), microscopy and nested Plasmodium species diagnostic PCR. From a cohort of 924 pregnant women who completed the follow up, complete RDT and microscopy data was available for 5,555 blood samples and of these 442 samples were analysed by PCR. Of the 5,555 blood samples, 49 ((proportion and 95% confidence interval) 0.9% [0.7 -1.1]) samples were positive by microscopy and 91 (1.6% [1.3-2.0]) by RDT. Forty-six (50.5% [40.5 - 60.6]) and 45 (49.5% [39.4 - 59.5]) of the RDT positive samples were positive and negative by microscopy, respectively, whereas nineteen (42.2% [29.0 - 56.7]) of the microscopy negative, but RDT positive, samples were positive by PCR. Three (0.05% [0.02 - 0.2]) samples were positive by microscopy but negative by RDT. 351 of the 5,461 samples negative by both RDT and microscopy were tested by PCR and found negative. There was no statistically significant difference between the performances of the different RDTs. Microscopy underestimated the real burden of malaria during pregnancy and RDTs performed better than microscopy in diagnosing PAM. In areas where intermittent preventive treatment during pregnancy may be abandoned due to low and decreasing malaria risk and instead replaced with active case management, screening with RDT is likely to identify most infections in pregnant women and out-performs microscopy as a diagnostic tool

    Policy integration and conflict resolutions among various organizations still remain a major challenge.Moreover, policy inconsistency detection approach with logical reasoning techniques which considers integration requirements from collaboration parties has not been well studied.In this paper, we proposed a model to detect inconsistencies based on role-based access control (RBAC) that considers role hierarchy (RH) and temporal and spatial constraints.A model to prune and collect only the required policies based on access control requirements from different organizations is designed.Policy inconsistency detection should be enhanced with logical-based analysis in order to develop security policy integration.We believe this work could provide manner to filter a large amount of unrelated policies and only return potential collaboration policies for conflict resolution

    Stigma and treatment of eating disorders in Ireland: healthcare professionals' knowledge and attitudes

    Objectives: This study examines aspects of health professionals’ knowledge and attitudes about eating disorders (EDs) , which might impede the effective detection or treatment of EDs in Ireland. Methods: 1,916 health professionals were invited to participate in a web-based survey. Participants were randomly allocated to view one of five vignettes depicting a young person with symptoms consistent with Anorexia Nervosa, Bulimia Nervosa, Binge Eating Disorder, Depression or Type 1 Diabetes.     Inspection and crime prevention : an evolutionary perspective

    In this paper, we analyse inspection games with an evolutionary perspective. In our evolutionary inspection game with a large population, each individual is not a rational payoff maximiser, but periodically updates his strategy if he perceives that other individuals' strategies are more successful than his own, namely strategies are subject to the evolutionary pressure. We develop this game into a few directions. Firstly, social norms are incorporated into the game and we analyse how social norms may influence individuals' propensity to engage in criminal behaviour. Secondly, a forward-looking inspector is considered, namely, the inspector chooses the level of law enforcement whilst taking into account the effect that this choice will have on future crime rates. Finally, the game is extended to the one with continuous strategy spaces

    A Review of Atrial Fibrillation Detection Methods as a Service

    Atrial Fibrillation (AF) is a common heart arrhythmia that often goes undetected, and even if it is detected, managing the condition may be challenging. In this paper, we review how the RR interval and Electrocardiogram (ECG) signals, incorporated into a monitoring system, can be useful to track AF events. Were such an automated system to be implemented, it could be used to help manage AF and thereby reduce patient morbidity and mortality. The main impetus behind the idea of developing a service is that a greater data volume analyzed can lead to better patient outcomes. Based on the literature review, which we present herein, we introduce the methods that can be used to detect AF efficiently and automatically via the RR interval and ECG signals. A cardiovascular disease monitoring service that incorporates one or multiple of these detection methods could extend event observation to all times, and could therefore become useful to establish any AF occurrence. The development of an automated and efficient method that monitors AF in real time would likely become a key component for meeting public health goals regarding the reduction of fatalities caused by the disease. Yet, at present, significant technological and regulatory obstacles remain, which prevent the development of any proposed system. Establishment of the scientific foundation for monitoring is important to provide effective service to patients and healthcare professionals

    The Conflict Notion and its Static Detection: a Formal Survey

    The notion of policy is widely used to enable a flexible control of many systems: access control, privacy, accountability, data base, service, contract , network configuration, and so on. One important feature is to be able to check these policies against contradictions before the enforcement step. This is the problem of the conflict detection which can be done at different steps and with different approaches. This paper presents a review of the principles for conflict detection in related security policy languages. The policy languages, the notions of conflict and the means to detect conflicts are various, hence it is difficult to compare the different principles. We propose an analysis and a comparison of the five static detection principles we found in reviewing more than forty papers of the literature. To make the comparison easier we develop a logical model with four syntactic types of systems covering most of the literature examples. We provide a semantic classification of the conflict notions and thus, we are able to relate the detection principles, the syntactic types and the semantic classification. Our comparison shows the exact link between logical consistency and the conflict notions, and that some detection principles are subject to weaknesses if not used with the right conditions
