    The information sources of the first Spanish Newspapers (1618-1635): the construction of information credibility

    Este trabajo analiza las fuentes de información utilizadas en un corpus numeroso de avisos recogidos en periódicos españoles impresos entre 1618 y 1635. El objetivo de partida es determinar cómo se construye, en términos tanto históricos como retóricos, la credibilidad informativa. Analizaremos la construcción retórica de la credibilidad en el texto mismo de los avisos, concretamente en las fuentes de información que estos declaran u ocultan; para comprobar si existe una intención deliberada de encubrir las fuentes de información, estudiaremos la frecuencia con que aparece la atribución on deep background. La aproximación metodológica que orienta este estudio es la que sostiene el Historical News Discourse, que aplica las conclusiones y métodos de análisis del Análisis Crítico del Discurso a los periódicos del pasado, y contextualiza y explica sus resultados en relación a los planteamientos de la Historia del Periodismo y de la Comunicación (Conboy, Brownlees, Broersma y otros). Defendemos en este trabajo que el análisis del discurso de los primeros impresos informativos resulta fundamental para conocer cómo se consolida en España la profesión periodística, gracias a una nueva forma de auctoritas basada en la competencia discursiva. El gacetero o periodista aparece como una nueva modalidad de escritor, cuya credibilidad y reputación dependen de su capacidad para acceder, organizar y declarar –de acuerdo a patrones retóricos reconocibles para sus lectores– las fuentes de información a las que tiene acceso.This paper analyses the information sources used in a large corpus of news items published in Spanish printed newspapers between 1618 and 1635. The initial aim is to determine how information credibility was constructed in both historical and rhetorical terms. To this end, the rhetorical construction of credibility in the news stories are analysed by focusing on the information sources that these reveal or conceal. And in order to determine whether or not these sources were deliberately concealed, the frequency with which ‘on deep background’ attribution appears is examined. The methodological approach on which this study is based is Historical News Discourse, which applies the conclusions and methods of critical discourse analysis to newspapers of the past, in addition to contextualising and explaining the results in terms of journalism history and communication approaches (Conboy, Brownlees and Broersma, among others). In this work, we contend that the discourse analysis of the first printed newspapers is essential for gaining further insights into how the journalistic profession consolidated its position in Spain thanks to a new form of auctoritas based on discourse competence. The gazetteer or journalist emerged as a new kind of writer, whose credibility and reputation depended on his ability to access, organise and reveal –according to rhetorical patterns recognisable to readers– the information sources available to him

    Background: Ventilator-associated pneumonia (VAP) is one of the most common and severe hospital-adquired infections, and multidrugresistant gram-negative bacilli (MDR-GNB) constitute the main etiology in many countries. Inappropriate empiric antimicrobial treatment is associated with increased mortality. In this context, the empirical treatment of choice for VAP is unknown. Colistin, is now the antimicrobial with greatest in vitro activity against MDR-GNB. Methods/Design: The MagicBullet clinical trial is an investigator-driven clinical study, funded by the Seventh Framework Program of the European Commission. This is designed as a phase IV, randomized, controlled, open label, non-inferiority and international trial to assess the safety and efficacy of colistin versus meropenem in late onset VAP. The study is conducted in a total of 32 centers in three European countries (Spain, Italy and Greece) with specific high incidences of infections caused by MDR-GNB. Patients older than 18 years who develop VAP with both clinical and radiological signs, and are on mechanical ventilation for more than 96 hours, or less than 96 hours but with previous antibiotic treatment plus one week of hospitalization, are candidates for inclusion in the study. A total sample size of 496 patients will be randomized according to a severity clinical score (at the time of VAP diagnosis in a 1:1 ratio to receive either colistin 4.5 MU as a loading dose, followed by 3 MU every eight hours (experimental arm), or meropenem 2 g every eight hours (control arm), both combined with levofloxacin. Mortality from any cause at 28 days will be considered as the main outcome. Clinical and microbiological cure will be evaluated at 72 hours, eight days, the finalization of antibiotic treatment, and 28 days of follow-up. The efficacy evaluation will be performed in every patient who receives at least one study treatment drug, and with etiologic diagnosis of VAP, intention-to-treat population and per protocol analysis will be performed

    Understanding User Behavioral Intention to Adopt a Search Engine that Promotes Sustainable Water Management

    An increase in users’ online searches, the social concern for an efficient management of resources such as water, and the appearance of more and more digital platforms for sustainable purposes to conduct online searches lead us to reflect more on the users’ behavioral intention with respect to search engines that support sustainable projects like water management projects. Another issue to consider is the factors that determine the adoption of such search engines. In the present study, we aim to identify the factors that determine the intention to adopt a search engine, such as Lilo, that favors sustainable water management. To this end, a model based on the Theory of Planned Behavior (TPB) is proposed. The methodology used is the Structural Equation Modeling (SEM) analysis with the Analysis of Moment Structures (AMOS). The results demonstrate that individuals who intend to use a search engine are influenced by hedonic motivations, which drive their feeling of contentment with the search. Similarly, the success of search engines is found to be closely related to the ability a search engine grants to its users to generate a social or environmental impact, rather than users’ trust in what they do or in their results. However, according to our results, habit is also an important factor that has both a direct and an indirect impact on users’ behavioral intention to adopt different search engines

    A systematic review and meta-analysis of the diagnostic accuracy of the Phototest for cognitive impairment and dementia

    The recently developed Phototest is a simple, easy and very brief test for detecting cognitive impairment or dementia. Objective: To evaluate the diagnostic accuracy of the Phototest for detecting cognitive impairment or dementia. METHODS: We used a manually created database to search for studies evaluating the Phototest diagnostic yield and performed an initial meta-analysis to determine sensitivity (Sn) and specificity (Sp) of diagnostic parameters. We also performed a second meta-analysis of individual participant data. RESULTS: In total, 6 studies were included in the meta-analysis. For dementia, Sn was 0.85 (95% CI, 0.82-0.88) and Sp 0.87 (95% CI, 0.85-0.99); for cognitive impairment, Sn was 0.80 (95% CI, 0.77-0.92) and Sp 0.88 (95% CI, 0.86-0.90). In the individual data meta-analysis, 1565 subjects were included, where best cut-off points for dementia and for cognitive impairment were 26/27 (Sn=0.89 (95% CI 0.85-0.91), Sp=0.84 (95% CI, 0.82-0.91)) and 28/29 (Sn=0.79 (95% CI, 0.76-0.81), Sp=0.88 (95% CI, 0.86-0.90)), respectively. CONCLUSION: Phototest has good diagnostic accuracy for dementia and cognitive impairment. It is brief, simple and can be used in illiterate persons. This makes it suitable for use in primary care settings and/or in subjects with low educational level.Phototest é um teste simples, fácil e muito rápido para detecção de comprometimento cognitivo e demência recentemente desenvolvido. Objetivo: Avaliar a acurácia diagnostica do Phototest para detecção de comprometimento cognitivo e demência. MÉTODOS: Nós usamos um banco de dados manualmente criado para estudos que avaliassem a capacidade diagnóstica do Phototest e realizamos uma meta-análise para determinar a sensibilidade (Sn) e especificidade (Ep) dos parâmetros diagnósticos. Nós também realizamos uma segunda meta-análise dos dados individuais dos participantes. RESULTADOS: Um total de seis estudos foram incluídos na meta-análise. Para demência a Sn foi 0.85 (95% CI, 0,82-0,88) e Ep 0,87 (95% CI, 0,85-0,99); para comprometimento cognitivo a Sn foi 0,80 (95% CI, 0,77-0,92) e Sp 0,88 (95% CI, 0,86-0,90). Na meta-análise de dados individuais, 1565 foram incluídos, os melhores escores de corte para demência e para comprometimento cognitivo foram 26/27 (Sn=0,89 (95% CI 0,85-0,91), Ep=0,84 (95% CI, 0,82-0,91)) e 28/29 (Sn=0,79 (95% CI, 0,76-0,81), Ep=0,88 (95% CI, 0,86-0,90)), respectivamente. CONCLUSÃO: Photest tem boa acurácia diagnostica para demência e comprometimento cognitivo. É breve, simples e pode ser usado em pessoas analfabetas.     Health-related quality of life and determinant factors in celiac disease. A population-based analysis of adult patients in Spain

    Background: Celiac disease (CD) has a negative impact on the health-related quality of life (HRQL) of affected patients. Although HRQL and its determinants have been examined in Spanish CD patients specifically recruited in hospital settings, these aspects of CD have not been assessed among the general Spanish population. Methods: An observational, transversal study of a non-randomized, representative sample of adult celiac patients throughout all of Spain's Autonomous Regions. Subjects were recruited through celiac patient associations. A Spanish version of the self-administered Celiac Disease-Quality of Life (CD-QOL) questionnaire was used. Determinant factors of HRQL were assessed with the aid of multivariate analysis to control for confounding factors. Results: We analyzed the responses provided by 1,230 patients, 1,092 (89.2%) of whom were women. The overall mean value for the CD-QOL index was 56.3 ± 18.27 points. The dimension that obtained the most points was dysphoria, with 81.3 ± 19.56 points, followed by limitations with 52.3 ± 23.43 points; health problems, with 51.6 ± 26.08 points, and inadequate treatment, with 36.1 ± 21.18 points. Patient age and sex, along with time to diagnosis, and length of time on a gluten-free diet were all independent determinant factors of certain dimensions of HRQL: women aged 31 to 40 expressed poorer HRQL while time to diagnosis and length of time on a gluten-free diet were determinant factors for better HRQL scores. Conclusions: The HRQL of adult Spanish celiac subjects is moderate, improving with the length of time patients remain on a gluten-free diet

    This report provides an overview of Greater Mexico City and its food system. The city’s history, demographic characteristics, geography and economy are first discussed. The city’s urban food system and urban food security are then examined with a particular focus on formal and informal food retail, food expenditure patterns, and policies to combat hunger and food insecurity. Meeting the daily food demands of Mexico City’s over 20 million inhabitants requires the agricultural production of Mexico’s rural areas, its fishing industry and food imports. Food products arrive in the city from around the country in a combination of traditional and highly sophisticated modern systems of food supply and distribution. Structural changes in recent decades have led to modifications in the systems of supply, distribution and food consumption with vertically integrated companies now controlling aspects of the food chain. The system of supply and marketing of food products is also characterized by competition between public markets, large wholesale and retail companies, and neighbourhood convenience stores. While levels of household food insecurity (undernutrition) are lower than in other global cities of the South, Mexico City faces an epidemic of overnutrition, obesity and non-communicable diseases

    Thermal applications are drawing increasing attention in the solar energy research field, due to their high performance in energy storage density and energy conversion efficiency. In these applications, solar collectors and thermal energy storage systems are the two core components. This paper focuses on the latest developments and advances in solar thermal applications, providing a review of solar collectors and thermal energy storage systems. Various types of solar collectors are reviewed and discussed, including both non-concentrating collectors (low temperature applications) and concentrating collectors (high temperature applications). These are studied in terms of optical optimisation, heat loss reduction, heat recuperation enhancement and different sun-tracking mechanisms. Various types of thermal energy storage systems are also reviewed and discussed, including sensible heat storage, latent heat storage, chemical storage and cascaded storage. They are studied in terms of design criteria, material selection and different heat transfer enhancement technologies.
