    Using the Chapelhow Framework to Deliver Nursing Care.

    Chapelhow et al. (2005) devised a framework consisting of six enablers which nurses must undertake to provide competent, objective and specialised nursing care. This case study focuses upon two enablers: assessment and communication, with reference to a patient’s journey when admitted to a gastroenterology ward. Assessment and communication are important aspects of providing nursing care and this is demonstrated throughout this case study. Furthermore, they interlink to provide efficient nursing care; however, both are not without limitations. This is further explored throughout this case study

     This article will demonstrate how two of the six Chapelhow et al. (2005) enablers were utilised throughout the care of one individual. The two enablers that will be considered in this article will be communication and professional decision making. This article will use both experience from placement and current research to inform practice to demonstrate how these enablers were used in practice.  Communication and professional decision making enable healthcare professionals to deliver effective and efficient care. This article aims to enable healthcare students to develop their understanding of how these two enablers are used in practice and to raise awareness of their importance

    Modern slavery and human trafficking in the United Kingdom presents a series of new challenges for those working within the healthcare sector (Dalphins, 2016). Healthcare professionals have unique access to those involved in modern slavery, with over 88 percent of survivors reporting that they had accessed medical care on numerous occasions throughout their ordeal (Polaris Project, 2016). The variation in victim demographic, the differing forms of exploitation experienced and the traumatic nature of their ordeal can greatly affect the health of modern slavery victims (Adams, 2012). The enormity of the issue must be acknowledged and pre-emptive action taken to overcome barriers to satisfactory health and ultimately end the cycle of exploitation

    ABSTRACT  Diagnosis of chronic diseases have an impact on patient’s quality of life and the healthcare system. Telemedicine facilitates the delivery of medical intervention at home to improve health outcomes and reduces cost to the NHS. Adoption of telemedicine for routine management of chronic diseases impacts the healthcare patients and healthcare professionals, however, there are several challenges affecting the adoption of telemedicine into routine healthcare delivery. Lack of universal regulations and quality assurance protocol can impact the widescale implementation of telemedicine into routine healthcare. Therefore, further research, support and training is required for successful adoption for routine management of chronic diseases

One of the aspects of the nursing assessment procedure is that a set of customised results are agreed, which can be achieved through established teamwork and efficient communication. The Chapelhow Framework was established around six enablers: assessment, communication, risk management, managing uncertainty, record keeping and documentation, professional judgement and decision making. These enablers help healthcare professionals including student nurses to develop their skills to the best of their ability to deliver holistic and person-centred care.     Linking nursing theory to nursing practice using the Chapelhow Framework: A case study

    During observation of and involvement in ongoing care of a patient at risk of developing pressure ulcers and malnutrition, I was able to relate Chapelhow nursing theory to practice using two elements of its framework, assessment and risk management. I researched the efficacy of the tools used during assessment, the quantitative Waterlow Score tool and qualitative nursing clinical judgement tool. I also had the opportunity to assist in implementing and updating care plans tailored for the patient’s specific needs, which were used as part of risk management of the patient’s current condition, and to prevent further deterioration and improve life expectancy
