    VENTRICULAR ENLARGEMENT AND WATER INTOXICATION IN SCHIZOPHRENIA

    To determine the extent to which atrophy was related to water intoxication in schizophrenic patients, ventricular enlargement in polydipsic schizophrenic patients with hyponatremia and schizophrenic controls without hyponatremia was measured in a linear way. Ventricular enlargement was found in both schizophrenic patient groups, and larger ventricles were associated with water intoxication. It is suggested that ventricular enlargement may reflect a developmental or degenerative pathological process involving the occurrence of water intoxication in schizophrenia

    Evaluation of Cerebral Lateral Ventricular Enlargement Derived from Magnetic Resonance Imaging: A Candidate Biomarker of Alzheimer Disease Progression in Vivo

    Alzheimer disease (AD) is the most common form of dementia and has grievous mortality rates. Measuring brain volumes from structural magnetic resonance images (MRI) may be useful for illuminating disease progression. The goal of this thesis was to (1) help refine a novel technique used to segment the lateral cerebral ventricles from MRI, (2) validate this tool, and determine group-wise differences between normal elderly controls (NEC) and subjects with mild cognitive impairment (MCI) and AD and (3) determine the number of subjects necessary to detect a 20 percent change from the natural history of ventricular enlargement with respect to genotype. Three dimensional Ti-weighted MRI and cognitive measures were acquired from 504 subjects (NEC n = 152, MCI n = 247 and AD n = 105) participating in the multi-centre Alzheimer\u27s Disease Neuroimaging Initiative. Cerebral ventricular volume was quantified at baseline and after six months. For secondary analyses, all groups were dichotomized for Apolipoprotein E genotype based on the presence of an e4 polymorphism. The AD group had greater ventricular enlargement compared to both subjects with MCI (P = 0.0004) and NEC (P \u3c 0.0001), and subjects with MCI had a greater rate of ventricular enlargement compared to NEC (P =0.0001). MCI subjects that progressed to clinical AD after six months had greater ventricular enlargement than stable MCI subjects (P = 0.0270). Ventricular enlargement was different between apolipoprotein E genotypes within the AD group (P = 0.010). The number of subjects required to demonstrate a 20% change in ventricular enlargement (AD: N=342, MCI: N=1180) was substantially lower than that required to demonstrate a 20% change in cognitive scores (MMSE) (AD: N=7056, MCI: N=7712). Therefore, ventricular enlargement represents a feasible short-term marker of disease progression in subjects with MCI and subjects with AD for multi-centre studie

    Progressive cognitive impairment evolving to dementia parallels parieto-occipital and temporal enlargement in idiopathic chronic hydrocephalus: a retrospective cohort study

    Little is known regarding progressive enlargement of the ventricular system in symptomatic patients or asymptomatic subjects. Before eventual surgical treatment, we evaluated the clinical and radiological features of an extremely rare group of patients with idiopathic chronic hydrocephalus (ICH) and cognitive impairment evolving to dementia (n = 11), and an extremely rare group of asymptomatic or minimally symptomatic adults (AMSA) with ventricular enlargement (n = 10). We quantified changes over time in the ventricular frontal, occipital, and temporal horns by measuring the Evans' index plus a parieto-occipital ratio and a temporal ratio, and their percentage of progression. Cerebral ventricles expanded over very long term in both demented patients with ICH and in AMSA. In AMSA, frontal enlargement predominated, whereas demented patients showed predominant parieto-occipital (p = 0.00) and temporal (p = 0.00) enlargement that progressed faster than in AMSA (p = 0.00). In ICH, progression of cognitive impairment parallels ventricular parieto-occipital and temporal horn enlargement. Limitations of this study are the retrospective nature, the non-uniform use of neuropsychological tests, the reduced sample size due to the extremely stringent enrollment criteria, the inability to determine the precise rate of progression

    A Mechanism for Ventricular Expansion in Communicating Hydrocephalus

    This report investigates a new possible molecular mechanism for the pathogenesis of hydrocephalus. New research by Dr. Miles Johnston has found that the injection of anti beta_1 integrin antibodies into the ventricle of rats causes a drop in parenchymal pressure and causes the cerebral ventricles to enlarge which is characteristic of hydrocephalus. We investigate intramantle pressure gradients as a possible force to enlarge the ventricles and we propose a new poroelastic model incorporating the effect of the antibodies to determine if they are a possible mechanism for hydrocephalus

    GNAO1 encephalopathy: broadening the phenotype and evaluating treatment and outcome

    OBJECTIVE: To describe better the motor phenotype, molecular genetic features, and clinical course of GNAO1-related disease. METHODS: We reviewed clinical information, video recordings, and neuroimaging of a newly identified cohort of 7 patients with de novo missense and splice site GNAO1 mutations, detected by next-generation sequencing techniques. RESULTS: Patients first presented in early childhood (median age of presentation 10 months, range 0-48 months), with a wide range of clinical symptoms ranging from severe motor and cognitive impairment with marked choreoathetosis, self-injurious behavior, and epileptic encephalopathy to a milder phenotype, featuring moderate developmental delay associated with complex stereotypies, mainly facial dyskinesia and mild epilepsy. Hyperkinetic movements were often exacerbated by specific triggers, such as voluntary movement, intercurrent illnesses, emotion, and high ambient temperature, leading to hospital admissions. Most patients were resistant to drug intervention, although tetrabenazine was effective in partially controlling dyskinesia for 2/7 patients. Emergency deep brain stimulation (DBS) was life saving in 1 patient, resulting in immediate clinical benefit with complete cessation of violent hyperkinetic movements. Five patients had well-controlled epilepsy and 1 had drug-resistant seizures. Structural brain abnormalities, including mild cerebral atrophy and corpus callosum dysgenesis, were evident in 5 patients. One patient had a diffuse astrocytoma (WHO grade II), surgically removed at age 16. CONCLUSIONS: Our findings support the causative role of GNAO1 mutations in an expanded spectrum of early-onset epilepsy and movement disorders, frequently exacerbated by specific triggers and at times associated with self-injurious behavior. Tetrabenazine and DBS were the most useful treatments for dyskinesia

    Isolated Subtle Neurological Abnormalities in Mild Cognitive Impairment Types

    Background: Isolated, subtle neurological abnormalities (ISNA) are commonly seen in aging and have been related to cerebral small vessel disease (SVD) and subcortical atrophy in neurologically and cognitively healthy aging subjects. Objective: To investigate the frequency of ISNA in different mild cognitive impairment (MCI) types and to evaluate for each MCI type, the crosssectional relation between ISNA and white matter hyperintensities (WMH), lacunes, caudate atrophy, and ventricular enlargement. Methods: One thousand two hundred fifty subjects with different MCI types were included in the analysis and underwent brain magnetic resonance imaging. WMHs were assessed through two visual rating scales. Lacunes were also rated. Atrophy of the caudate nuclei and ventricular enlargement were assessed through the bicaudate ratio (BCr) and the lateral ventricles to brain ratio (LVBr), respectively. Apolipoprotein E (APOE) genotypes were also assessed. The routine neurological examination was used to evaluate ISNAs that were clustered as central-based signs, cerebellar-based signs, and primitive reflexes. The items of Part-III of the Unified Parkinson’s Disease Rating Scale were used to evaluate ISNAs that were clustered as mild parkinsonian signs. Associations of ISNAs with imaging findings were determined through logistic regression analysis. Results: The ISNAs increase with the age and are present in all MCI types, particularly in those multiple domains, and carrying the APOE ϵ4 allele, and are associated with WMH, lacunes, BCr, and LVBr. Conclusion: This study demonstrates that cortical and subcortical vascular and atrophic processes contribute to ISNAs. Long prospective population-based studies are needed to disentangle the role of ISNAs in the conversion from MCI to dementia

    Intrabody Gene Therapy Ameliorates Motor, Cognitive, and Neuropathological Symptoms in Multiple Mouse Models of Huntington's Disease

    Huntington's disease (HD) is an autosomal dominant neurodegenerative disease resulting from the expansion of a glutamine repeat in the huntingtin (Htt) protein. Current therapies are directed at managing symptoms such as chorea and psychiatric disturbances. In an effort to develop a therapy directed at disease prevention we investigated the utility of highly specific, anti-Htt intracellular antibodies (intrabodies). We previously showed that V_(L)12.3, an intrabody recognizing the N terminus of Htt, and Happ1, an intrabody recognizing the proline-rich domain of Htt, both reduce mHtt-induced toxicity and aggregation in cell culture and brain slice models of HD. Due to the different mechanisms of action of these two intrabodies, we then tested both in the brains of five mouse models of HD using a chimeric adeno-associated virus 2/1 (AAV2/1) vector with a modified CMV enhancer/chicken β-actin promoter. V_(L)12.3 treatment, while beneficial in a lentiviral model of HD, has no effect on the YAC128 HD model and actually increases severity of phenotype and mortality in the R6/2 HD model. In contrast, Happ1 treatment confers significant beneficial effects in a variety of assays of motor and cognitive deficits. Happ1 also strongly ameliorates the neuropathology found in the lentiviral, R6/2, N171-82Q, YAC128, and BACHD models of HD. Moreover, Happ1 significantly prolongs the life span of N171-82Q mice. These results indicate that increasing the turnover of mHtt using AAV-Happ1 gene therapy represents a highly specific and effective treatment in diverse mouse models of HD

    The Relationship between Brachycephalic Head Features in Modern Persian Cats and Dysmorphologies of the Skull and Internal Hydrocephalus

    Background: Cat breeders observed a frequent occurrence of internal hydrocephalus in Persian cats with extreme brachycephalic head morphology. Objective: To investigate a possible relationship among the grade of brachycephaly, ventricular dilatation, and skull dysmorphologies in Persian cats. Animals: 92 Persian-, 10 Domestic shorthair cats. Methods: The grade of brachycephaly was determined on skull models based on CT datasets. Cranial measurements were examined with regard to a possible correlation with relative ventricular volume, and cranial capacity. Persians with high (peke-face Persians) and lower grades of brachycephaly (doll-face Persians) were investigated for the presence of skull dysmorphologies. Results: The mean cranial index of the peke-face Persians (0.97 ± 0.14) was significantly higher than the mean cranial index of doll-face Persians (0.66 ± 0.04; P < 0.001). Peke-face Persians had a lower relative nasal bone length (0.15 ± 0.04) compared to doll-face (0.29 ± 0.08; P < 0.001). The endocranial volume was significantly lower in doll-face than peke-face Persians (89.6 ± 1.27% versus 91.76 ± 2.07%; P < 0.001). The cranial index was significantly correlated with this variable (Spearman´s r: 0.7; P < 0.0001). Mean ventricle: Brain ratio of the peke-face group (0.159 ± 0.14) was significantly higher compared to doll-face Persians (0.015 ± 0.01; P < 0.001). Conclusion and Clinical Relevance: High grades of brachycephaly are also associated with malformations of the calvarial and facial bones as well as dental malformations. As these dysmorphologies can affect animal welfare, the selection for extreme forms of brachycephaly in Persian cats should be reconsidered
