    Analytic lymph node number establishes staging accuracy by occult tumor burden in colorectal cancer.

    BACKGROUND AND OBJECTIVES: Recurrence in lymph node-negative (pN0) colorectal cancer suggests the presence of undetected occult metastases. Occult tumor burden in nodes estimated by GUCY2C RT-qPCR predicts risk of disease recurrence. This study explored the impact of the number of nodes analyzed by RT-qPCR (analytic) on the prognostic utility of occult tumor burden. METHODS: Lymph nodes (range: 2-159) from 282 prospectively enrolled pN0 colorectal cancer patients, followed for a median of 24 months (range: 2-63), were analyzed by GUCY2C RT-qPCR. Prognostic risk categorization defined using occult tumor burden was the primary outcome measure. Association of prognostic variables and risk category were defined by multivariable polytomous and semi-parametric polytomous logistic regression. RESULTS: Occult tumor burden stratified this pN0 cohort into categories of low (60%; recurrence rate (RR) = 2.3% [95% CI 0.1-4.5%]), intermediate (31%; RR = 33.3% [23.7-44.1%]), and high (9%; RR = 68.0% [46.5-85.1%], P \u3c 0.001) risk of recurrence. Beyond race and T stage, the number of analytic nodes was an independent marker of risk category (P \u3c 0.001). When \u3e12 nodes were analyzed, occult tumor burden almost completely resolved prognostic risk classification of pN0 patients. CONCLUSIONS: The prognostic utility of occult tumor burden assessed by GUCY2C RT-qPCR is dependent on the number of analytic lymph nodes

    Distinct cytokine patterns in Occult Hepatitis C and Chronic Hepatitis C Virus Infection

    Background & Aim: 
The immunopathogenesis of chronic hepatitis C virus (HCV) infection is a matter of great controversy. The imbalance of T-helper lymphocyte cell cytokine production was believed to play an important pathogenic role in chronic viral hepatitis. Occult hepatitis C infection is regarded as a new entity that should be considered when diagnosing patients with a liver disease of unknown origin. The aim of this study was to determine serum T-helper 1 and T-helper 2 cytokine production in patients with occult HCV infection and its role in pathogenesis versus chronic viral hepatitis C infection.

Methods: 
Serum levels of cytokines of T-helper 1 (IL-2, IFN-[gamma]) and T-helper 2 (IL-4) were measured in 27 patients with occult HCV infection and 50 patients with chronic hepatitis C infection.

Results: 
The levels of the T-helper 1 cytokines, IL-2 and IFN-[gamma], were highly and significantly increased in patients with chronic HCV infection as compared with occult HCV infection (p<0.001). The T-helper 2 cytokine IL-4 was highly and significantly increased in occult HCV infection as compared with chronic HCV infection (p<0.001). Necroinflammation (P<0.001) fibrosis (P<0.001) and cirrhosis (P =0.03) were significantly increased in chronic HCV than occult HCV. 

Conclusion: 
Patients with occult HCV infection exhibited distinct immunoregulatory cytokine patterns, favoring viral persistence in the liver in spite of its absence from peripheral blood and explaining the less aggressive course of this disease entity than chronic hepatitis C virus infection

    Background: Clinical outcome of patients with head and neck squamous cell carcinoma (SCCHN) depends on several risk factors like the presence of locoregional lymph node or distant metastases, stage, localisation and histologic differentiation of the tumour. Circulating tumour cells in the bone marrow indicate a poor prognosis for patients with various kinds of malignoma. The present study examines the clinical relevance of occult tumour cells in patients suffering from SCCHN. Patients and Methods: Bone marrow aspirates of 176 patients suffering from SCCHN were obtained prior to surgery and stained for the presence of disseminated tumour cells. Antibodies for cytokeratin 19 were used for immunohistochemical detection with APAAP on cytospin slides. Within a clinical follow-up protocol over a period of 60 months, the prognostic relevance of several clinicopathological parameters and occult tumour cells was evaluated. Results: Single CK19-expressing tumour cells could be detected in the bone marrow of 30.7% of the patients. There is a significant correlation between occult tumour cells in the bone marrow and relapse. Uni- and multivariate analysis of all clinical data showed the metastases in the locoregional lymph system and detection of disseminated tumour cells in the bone marrow to be statistically highly significant for clinical prognosis. Conclusion: The detection of minimal residual disease underlines the understanding of SCCHN as a systemic disease. Further examination of such cells will lead to a better understanding of the tumour biology, as well as to improvement of diagnostic and therapeutic strategies

    Aerosol deposition and origin in French mountains estimated with soil inventories of 210Pb and artificial radionuclides

    Radionuclide inventories were measured in soils from different French mountainous areas: Chaüne des Puys (Massif Central), Eastern Corsica, Jura, Montagne Noire, Savoie, Vosges and Rhine Valley. 210Pb soil inventories were used to estimate long-term (>75 yr) deposition of submicron aerosols. Whereas 210Pb total deposition is explained partly by wet deposition, as demonstrated by increase of 210Pb inventory with annual rainfall; a part of 210Pb in the soils of higher altitude is caused by orographic depositions. Using measurements of radionuclides coming from nuclear aerial weapon tests (137Cs and Pu isotopes), we were able to estimate the origin of aerosols deposited in high-altitude sites and to confirm the importance of occult deposition and feeder–seeder mechanism. Using a simple mass balance model, we estimate that occult deposition and feeder–seeder mechanisms account to more than 50% of total deposition of 210Pb and associated submicron aerosols in French altitude sites

    Watching of Occult Film and its Influence on African’s Supernatural Belief System; Specifically Ghana

    Occult practices or issues surrounding occultism have been on the increase in Ghana, especially among the youth. It is believed that our belief system help nurture these practices when experienced through the propagating mediums. These propagating mediums are quite unclear however, suspected ones included access to the internet, occult materials and ‘perhaps’ movies. Hence, this study examined the influence of watching occult film on African’s supernatural belief system; specifically Ghana. The objective of this study was to examine whether occult film increases one’s supernatural belief system as compared to non-occult film; age, sex, and educational level differences exist in supernatural belief system.   Sixty-six subjects were sampled using stratified random sampling technique. Subjects were randomly assigned into either control or experimental group equally. Both control and experimental group subjects responded to a questionnaire (pre-test) and later watched a non-occult film and occult film respectively. They then answered the questionnaire again (post-test). The hypotheses were analysed using Correlated t test, Independent t test and One-Way ANOVA. The findings revealed that occult film significantly increases ones’ supernatural belief system as compared to non-occult film. However, there was no significant difference between ages, sexes, and educational levels on supernatural belief system. This implies that what we watch can influence our behaviour.
