Möglichkeiten der systemischen Therapie in metastasierten Stadien des Magenkarzinoms
Zusammenfassung: Die Ergebnisse mehrerer, in letzter Zeit publizierter Phase-III-Studien haben die therapeutischen Möglichkeiten in der Behandlung des metastasierten Magenkarzinoms deutlich erweitert. Die Dauerinfusion von 5-Fluorouracil (5-FU) kann ohne Verlust an Wirkung durch Capecitabin ersetzt werden, ebenso wie Cisplatin durch Oxaliplatin. Nach den Ergebnissen der REAL-2-Studie zeigt die Kombination aus Epirubicin, Oxaliplatin und Capecitabin (EOX) eine Verbesserung des Gesamtüberlebens (9,9 vs. 11,2Monate; HR 0,8) im Vergleich zu Epirubicin, Cisplatin und 5-FU (ECF). Die Frage, ob in der First-Line-Therapie eine Dreifachkombination oder eine Zweifachkombination eingesetzt werden sollte, ist allerdings weiterhin umstritten. Die Kombination aus Irinotecan und 5-FU stellt für solche Patienten, bei denen aufgrund von Komorbiditäten eine platinfreie Therapie bevorzugt wird, eine Alternative zur Kombination Cisplatin/5-FU dar. Docetaxel, 5-FU und Cisplatin (DCF) hat sich bezüglich des Überlebens in einer randomisierten Phase-III-Studie als statistisch signifikant überlegen erwiesen, allerdings besteht eine ausgeprägte hämatologische Toxizität, welche die Anwendbarkeit insbesondere bei den häufig älteren Patienten mit einem Magenkarzinom limitiert. Zmiany płucne w przebiegu raka żołądka — dwa przypadki zespołu Barda
Zespół Barda to zespół chorobowy związany z prosowatym rozsiewem raka żołądka do płuc. Stanowi on trudny problem diagnostyczny z uwagi na konieczność różnicowania z wieloma jednostkami chorobowymi manifestującymi się w zdjęciu radiologicznym klatki piersiowej poprzez zmiany rozsiane w miąższu płucnym. Pierwotne ognisko nowotworu w żołądku pozostaje często nieme klinicznie, mimo zaawansowania procesu rozrostowego. Zmiany przerzutowe powodują wiele objawów z zakresu układu oddechowego kierujących proces diagnostyczny w kierunku pierwotnej patologii płucnej. Zespół Barda powinien być zawsze brany pod uwagę w procesie diagnostyczno- różnicowym zmian rozsianych, zwłaszcza z powodu częstości występowania raka żołądka. Rak żołądka, nieoperacyjne metody leczenia
Mimo postępu dokonującego się w onkologii wyniki
leczenia raka żołądka są niezadowalające. Nawet
u chorych z ograniczoną postacią raka, piecioletnie
przeżycia wahają się od 15% do 30%. Leczeniem zasadniczym
jest zabieg chirurgiczny, ale ponad połowa
chorych, ze względu na znaczne miejscowe zaawansowanie
choroby lub obecność przerzutów odległych,
nie kwalifikuje się do tej formy leczenia. W ostatnim
czasie podjęto wiele badań klinicznych dotyczących
leczenia przedoperacyjnego i uzupełniającego. Wyniki
dwóch z nich (Intergroup 0116 i MAGIC), w których
zastosowano leczenie skojarzone obejmujące pooperacyjną,
uzupełniającą radiochemioterapię i okołooperacyjną
chemioterapię, wykazały znamienne wydłużenie
czasu przeżycia i czasu do progresji w grupach
chorych leczonych metodą skojarzoną w porównaniu
z pacjentami leczonymi wyłącznie chirurgicznie. Podobnie,
wyniki niedawno opublikowanej metaanalizy
potwierdziły, że również pooperacyjna chemioterapia
znamiennie wydłuża czas przeżycia chorych w porównaniu
z wyłącznym leczeniem chirurgicznym.
Wyniki badań porównujących paliatywną chemioterapią
z optymalnym leczeniem objawowym u chorych
na zaawansowaną, nieoperacyjną lub przerzutową
postać raka żołądka wskazują na poprawę przeżyć
wśród chorych otrzymujących chemioterapię.
W celu ustalenia optymalnych standardów postępowania
u chorych na raka żołądka, konieczne są dalsze
prospektywne badania kliniczne.

Clinical and molecular aspects of gastrointestinal stromal tumors
Nowotwory podścieliskowe przewodu pokarmowego (GIST) to grupa najczęstszych nowotworów pochodzenia
mezenchymalnego w obrębie przewodu pokarmowego. Postępy w poznaniu ich mechanizmów
molekularnych spowodowały opracowanie metod leczenia, które stały się wzorcem leczenia ukierunkowanego
molekularnie w onkologii. Wprowadzenie do terapii metanosulfonianu imatynibu (hamującego
m.in. kinazy KIT/PDGFRA) zrewolucjonizowało terapię zaawansowanych (nieoperacyjnych i/lub
przerzutowych) GIST. Lek ten zarejestrowano również do leczenia uzupełniającego po wycięciu pierwotnego
GIST o znaczącym ryzyku nawrotu. U większości chorych leczonych imatynibem ostatecznie
dochodzi do rozwoju oporności. Postępy wiedzy w ciągu ostatnich lat pozwoliły na częściowe wyjaśnienie
mechanizmów progresji choroby podczas terapii imatynibem, które w większości przypadków wiążą
się z powstawaniem wtórnych mutacji w genach KIT i/lub PDGFRA. Obecnie jedynym lekiem zarejestrowanym
do terapii drugiej linii w GIST jest jabłczan sunitynibu - inhibitor wielokinazowy hamujący między
innymi KIT, PDGFRA/B, receptory naczyniowo-śródbłonkowego czynnika wzrostu (VEGFR), FLT3,
RET i CSF-1R . Trwają również badania nad innymi inhibitorami kinaz tyrozynowych nowej generacji, jak
również lekami działającymi w aternatywnych miejscach szlaków przekazywania sygnału (np. białko
wstrząsu termicznego 90, mTOR, receptor insulinopodobnego czynnika wzrostu).Gastrointestinal stromal tumors (GIST) comprise the entity of the most common mesenchymal neoplasms
of gastrointestinal tract. Advances in knowledge of molecular mechanisms of GISTs led to development
of therapy methods, which are the model of molecular targeted therapy in oncology. Introduction
to clinical practice imatinib mesylate [inhibiting kinases - KIT/PDGFRA (platelet-derived growth
factor receptor-α)] revolutionized therapy of advanced (inoperable and/or metastatic) GISTs. Imatinib
has been recently registered also for adjuvant therapy after resection of primary GIST with significant
risk of recurrence. In majority of patients treated with imatinib the resistance to this therapy developes.
Scientific advances allowed for partial explanation of mechanisms of disease progression during imatinib
therapy. The development of secondary mutations in KIT and/or PDGFRA genes is the most common
mechanism of resistance. The only registered drug for second line therapy in GIST is sunitinib
mesylate - multikinase inhibitor for e.g. KIT, PDGFRA/B, vascular endothelial growth factor receptors
(VEGFR), FLT3, RET i colony stimulating factor 1 receptor (CSF-1R). Many other tyrosine kinase inhibitors and drugs acting on alternative signaling pathways (as heat shock protein 90, mTOR, insuline-like
growth factor receptor) are currently investigated

Inflammatory Serum Proteins Are Severely Altered in Metastatic Gastric Adenocarcinoma Patients from the Chinese Population
<div><p>Background</p><p>Inflammation is one of the major hallmarks of cancer. This study was designed to profile a panel of inflammatory mediators in gastric adenocarcinoma (GA) and to identify their potential differences separately in metastatic and non-metastatic patient subgroups.</p><p>Methods</p><p>Serum samples from 216 GA patients and 333 healthy controls from China were analyzed for six proteins using the Luminex multiplex assay.</p><p>Results</p><p>The serum levels for all the six proteins were significantly elevated in metastatic GA compared to non-metastatic GA. Two acute phase proteins (SAA and CRP) and a CXC chemokine (GRO) were significantly elevated in metastatic GA (p <0.01) but smaller changes were observed in non-metastatic GA compared to healthy controls. OPN is moderately increased in non-metastatic GA (2.05-fold) and more severely elevated in metastatic GA (3.34-fold). Surprisingly, soluble VCAM1 and AGP were significantly lower in both non-metastatic and metastatic GA patients compared to controls. Several individual proteins were shown to possess moderate diagnostic value for non-metastatic GA (AUC = 0.786, 0.833, 0.823 for OPN, sVCAM1 and AGP, respectively) and metastatic GA (AUC = 0.931, 0.720, 0.834 and 0.737 for OPN, sVCAM1, SAA and CRP, respectively). However, protein combinations further improve the diagnostic potential for both non-metastatic GA (best AUC = 0.946) and metastatic GA (best AUC = 0.963). The protein combination with best AUC value for both comparisons is OPN+sVCAM1+AGP+SAA.</p><p>Conclusions</p><p>These results suggest that several serum proteins are directly related to the severity of gastric cancer. Overall, stronger associations are observed with metastatic than non-metastatic GA as the protein changes are greater with the metastatic status. A combination of these serum proteins may serve as non-invasive markers to assess the severity status and stage of gastric cancer.</p></div
Neoadjuvant chemotherapy outcome with taxane-based versus non-taxane protocols in gastric cancer
BACKGROUND: Gastric cancer is the fifth most common cancer worldwide. One of the chemotherapy agents, taxanes is important in increasing patients' survival. The purpose of this study is to assess the efficacy of taxane-based drugs versus non-taxanes in neoadjuvant chemotherapy in non-metastatic gastric adenocarcinoma (GA) in Iranian patients.
MATERIALS AND METHODS: In a historical cohort method, 65 patients between 18 and 75 years old who suffered from non-metastatic GA were included. Nineteen and 21 and 25 patients, had undergone DCF (docetaxel, cisplatin, 5fluorouracil) and FLOT (5fluorouracil, leucovorin, oxaliplatin, docetaxel) and FOLFOX6 (oxaliplatin, leucovorin, 5fluorouracil) regimens, respectively, between 2018 and 2021. Survival criteria consisting of progression-free survival (PFS), overall survival (OS), progression rate, and mortality rate were evaluated using the Kaplan–Meier method, in a three-year follow-up period.
RESULTS: The majority of patients were male (72.3%), with a median age of 65 years. Most of the patients had lesions with tumor, node, metastasis (TNM) stage IIIb (27.7%) and poor differentiated pathological grade (49.2%). OS time had a significant correlation with the low TNM stage (P = 0.01), well-differentiated pathological grade (P = 0.005), and FLOT vs. FOLFOX protocol (20.3 vs. 12.2 months, respectively. P =0.04). FLOT regimen had significantly better OS survival vs. DCF regimen (20.3 vs. 15.4 months, respectively, P = 0.03). No significant correlation was observed between survival criteria and other factors like gender, age, past medical history, Karnofsky scale, and tumor location in the stomach. The taxane-based arm (sum of DSF and FLOT) had no superiority over the non-taxane arm in survival criteria.
CONCLUSION: FLOT protocol, as a taxane-based regimen had better survival compared to FOLFOX protocol in neoadjuvant chemotherapy in gastric non-metastatic adenocarcinoma
Outcomes of Various Treatment Sequencing Strategies in Octogenarians with Gastric Adenocarcinoma: A National Cancer Database Analysis
Introduction: Perioperative therapy for gastric adenocarcinoma (GA) has demonstrated survival benefit. Multimodality therapy in the elderly can be challenging due to comorbidities/frailty. We aimed to describe outcomes of various treatment sequencing strategies in octogenarians with non-metastatic GA.
Methods: The National Cancer Database was queried for patients with GA from 2010 to 2015. Patients with stage I to III disease aged 80 to 89 years at diagnosis were included, then stratified into 3 groups: resection only (R), systemic therapy only (ST), and resection with systemic therapy (neoadjuvant and/or adjuvant therapy; R+ST). Overall survival was estimated from time of diagnosis using Kaplan-Meier curves and compared using log-rank tests (LRT). Multivariable Cox proportional hazard models were used to compare overall survival among treatment groups, adjusted for demographic and clinicopathologic data (Figure).
Results: A total of 2,652 octogenarians with stage I to III GA were identified (R, n = 665; ST, n = 1,680; R+ST, n = 307). Systemic therapy only was the predominant treatment strategy in stage II to III disease. Stage I median survival, R: 50 months, ST: 17.6 months, R+ST: 35.9 months (LRT: R vs ST; p \u3c 0.001, R+ST vs ST; p \u3c 0.001, R vs R+ST; p = 0.175). Systemic therapy only was an independent risk factor for mortality in stage I on multivariable Cox proportional hazard model analysis (reference: R). Stage II median survival, R: 18.5 months, ST: 16.3 months, R+ST: 29.2 months (LRT: R vs ST; p = 0.242, R+ST vs ST; p \u3c 0.001, R vs R+ST; p = 0.008). Stage III median survival, R: 12.0 months, ST: 13.6 months, R+ST: 19.7 months (LRT: R vs ST; p = 0.475, R+ST vs ST; p \u3c 0.001, R vs R+ST; p = 0.082). Systemic therapy only was not an independent risk factor for mortality in stage II to III on multivariable Cox proportional hazard model analysis (reference: R).
Conclusions: Although ST represented a risk factor for mortality in octogenarians with stage I, ST-only appears non-inferior to R-only in stage II to III GA. Outlining outcomes of treatment approaches for octogenarians with GA can aid in patient counseling and clinical decision-making. [Figure presented
