    The relationship between acoustic indices of speech motor control variability and other measures of speech performance in dysarthria

    Previous studies suggested that variability indices based on information extracted from the acoustic signal are potentially useful in assessing dysarthric speech. Because of the ease of data collection, this method is especially applicable in the clinical setting. This study assessed the relationship between variability indices of sentence repetitions obtained by Functional Data Analysis with intelligibility ratings and maximum performance tasks in groups of speakers with hypokinetic dysarthria and ataxic dysarthria. The results showed significant correlations between selected parameters, which varied with dysarthria type. For the speakers with ataxic dysarthria, the variability measure mainly reflected differences in intelligibility, while for the group with hypokinetic dysarthria, there was a stronger relationship between variability indices and DDK performance. Lack of stronger correlations between variability measures and intelligibility ratings and maximum performance tasks are possibly due to heterogeneity of severity across and within speaker groups. This study provides further evidence that variability measures such as the FDA might be sensitive to speech performance of speakers with dysarthria, and can potentially differentiate between dysarthria types

    Non-speech oro-motor exercise use in acquired dysarthria management : regimes and rationales

    Non-speech oro-motor exercises (NSOMExs) are described in speech and language therapy (SLT) manuals, and are thought to be much used in acquired dysarthria intervention, though there is no robust evidence of an influence on speech outcome. Opinions differ as to whether, and for which dysarthria presentations, NSOMExs are appropriate. The investigation sought to collect development phase data, in accordance with the Medical Research Council evaluation of complex interventions. The aims were to establish the extent of NSOMExs use in acquired disorders, the exercise regimes in use for dysarthria, with which dysarthric populations, and the anticipated clinical outcomes. A further aim was to determine the influencing rationales where NSOMExs were or were not used in dysarthria intervention. SLTs throughout Scotland, Wales and Northern Ireland, working with adult acquired dysarthria, were identified by their service heads. They received postal questionnaires comprising 21 closed and two open questions, covering respondent biographics, use of NSOMExs, anticipated clinical outcomes, and practice influencing rationales. One hundred and ninety one (56% response) completed questionnaires were returned. Eighty-one per cent of respondents used NSOMExs in dysarthria. There was no association with years of SLT experience. Those who used and those who did not use NSOMExs provided similar influencing rationales, including evidence from their own practice, and Higher Education Institute (HEI) teaching. More experienced SLTs were more likely than those more recently qualified to be guided by results from their own practice. Input from the attended HEI was more influential for those less experienced than for those more experienced. Clinical outcome aims were not confined to speech, but included also improvements in movement, sensory awareness, appearance, emotional status, dysphagia and drooling. NSOMExs were used with many neurological disorders, especially stroke, all dysarthria classes, especially flaccid, and all severity levels. Tongue and lip exercises were more frequent than face, jaw and soft palate. The most common regimes were 4-6 repetitions of each exercise, during three practice periods daily, each of 6-10 minutes. Conclusions & Implications: NSOMExs are a frequent component of dysarthria management in the UK devolved government countries. This confirmation, along with the details of SLT practice, provides a foundation for clinical research which will compare outcomes for people with dysarthria, whose management includes and does not include NSOMExs. SLT practice may be guided by evidence that speech outcome is or is not affected by NSOMExs

    The effect of three practice conditions on the consistency of chronic dysarthric speech

    This study investigated whether it is possible for people with chronic dysarthria to adjust their articulation in three practice conditions. A speaker dependent, speech recognition system was used to compare participants' practice attempts with a model of a word made from previous recordings to give a recognition score. This score was used to indicate changes in production of practice words with different conditions. The three conditions were reading of written target words, visual feedback, and an auditory model followed by visual feedback. For eight participants with dysarthria, the ability to alter speech production was shown, together with a differential effect of the three conditions. Copying an auditory target gave significantly better recognition scores than just repeating the word. Visual feedback was no more effective than repetition alone. For four control participants, visual feedback did produce significantly better recognition scores than just repetition of written words, and the presence of an auditory model was Significantly more effective than visual feedback. Possible reasons for differences between conditions are discussed

    Jaw Rotation in Dysarthria Measured With a Single Electromagnetic Articulography Sensor

    Purpose This study evaluated a novel method for characterizing jaw rotation using orientation data from a single electromagnetic articulography sensor. This method was optimized for clinical application, and a preliminary examination of clinical feasibility and value was undertaken. Method The computational adequacy of the single-sensor orientation method was evaluated through comparisons of jaw-rotation histories calculated from dual-sensor positional data for 16 typical talkers. The clinical feasibility and potential value of single-sensor jaw rotation were assessed through comparisons of 7 talkers with dysarthria and 19 typical talkers in connected speech. Results The single-sensor orientation method allowed faster and safer participant preparation, required lower data-acquisition costs, and generated less high-frequency artifact than the dual-sensor positional approach. All talkers with dysarthria, regardless of severity, demonstrated jaw-rotation histories with more numerous changes in movement direction and reduced smoothness compared with typical talkers. Conclusions Results suggest that the single-sensor orientation method for calculating jaw rotation during speech is clinically feasible. Given the preliminary nature of this study and the small participant pool, the clinical value of such measures remains an open question. Further work must address the potential confound of reduced speaking rate on movement smoothness

    Improved status following behavioural intervention in a case of severe dysarthria with stroke aetiology

    There is little published intervention outcome literature concerning dysarthria acquired from stroke. Single case studies have the potential to provide more detailed specification and interpretation than is generally possible with larger participant numbers and are thus informative for clinicians who may deal with similar cases. Such research also contributes to the future planning of larger scale investigations. Behavioural intervention is described which was carried out with a man with severe dysarthria following stroke, beginning at seven and ending at nine months after stroke. Pre-intervention stability between five and seven months contrasted with significant improvements post-intervention on listener-rated measures of word and reading intelligibility and communication effectiveness in conversation. A range of speech analyses were undertaken (comprising of rate, pause and intonation characteristics in connected speech and phonetic transcription of single word production), with the aim of identifying components of speech which might explain the listeners’ perceptions of improvement. Pre- and post intervention changes could be detected mainly in parameters related to utterance segmentation and intonation. The basis of improvement in dysarthria following intervention is complex, both in terms of the active therapeutic dimensions and also the specific speech alterations which account for changes to intelligibility and effectiveness. Single case results are not necessarily generalisable to other cases and outcomes may be affected by participant factors and therapeutic variables, which are not readily controllable

    Brittany Bernal - Sensorimotor Adaptation of Speech Through a Virtually Shortened Vocal Tract

    The broad objective of this line of research is to understand how auditory feedback manipulations may be used to elicit involuntary changes in speech articulation. We examine speech sensorimotor adaptation to supplement the development of speech rehabilitation applications that benefit from this learning phenomenon. By manipulating the acoustics of one’s auditory feedback, it is possible to elicit involuntary changes in speech articulation. We seek to understand how virtually manipulating participants’ perception of vowel space affects their speech movements by assessing acoustic variables such as formant frequency changes. Participants speak through a digital audio processing device that virtually alters the perceived size of their vocal tract. It is hypothesized that this modification to auditory feedback will facilitate adaptive changes in motor behavior as indicated by acoustic changes resulting from speech articulation. This study will determine how modifying the perception of vocal tract size affects articulatory behavior, indicated by changes in formant frequencies and changes in vowel space area. This work will also determine if and how the size of the virtual vowel space affects the magnitude and direction of sensorimotor adaptation for speech. The ultimate aim is to determine how important it is for the virtual vowel space to mimic the talker’s real vowel space, and whether or not perturbing the size of the perceived vowel space may facilitate or impede involuntary adaptive learning for speech. The system is developed to help Dysarthria of Speech patients communicate better with others in their everyday life. It will provide some background information about motor speech disorders, Dysarthria of Speech and the technical aspects of this application. It will then explain and test the algorithms to recognize impaired speech using audio fingerprinting technology. Finally it will discuss the test results and recommends some future work to improve the current algorithms

    The effectiveness of traditional methods and altered auditory feedback in improving speech rate and intelligibility in speakers with Parkinson's disease

    Communication problems are a frequent symptom for people with Parkinson's disease (PD) which can have a significant impact on their quality-of-life. Deciding on the right management approach can be problematic though, as, with the exception of LSVT (R), very few studies have been published demonstrating the effectiveness of treatment techniques. The aim of this study was to compare traditional rate reduction methods with altered auditory feedback (AAF) with respect to their effectiveness to reduce speech rate and improve intelligibility in speakers with PD. Ten participants underwent both types of treatments in once weekly sessions for 6 weeks. Outcomes measures were speech rate for passage reading as well as intelligibility on both a passage reading and a monologue task. The results showed that, as a group, there was no significant change in either speech rate or intelligibility resulting from either treatment type. However, individual speakers showed improvements in speech performance as a result of each therapy technique. In most cases, these benefits persisted for at least 6 months post-treatment. Possible reasons for the variable response to treatment, as well as issues to consider when planning to use AAF devices in treatment are discussed

    Learning to detect dysarthria from raw speech

    Speech classifiers of paralinguistic traits traditionally learn from diverse hand-crafted low-level features, by selecting the relevant information for the task at hand. We explore an alternative to this selection, by learning jointly the classifier, and the feature extraction. Recent work on speech recognition has shown improved performance over speech features by learning from the waveform. We extend this approach to paralinguistic classification and propose a neural network that can learn a filterbank, a normalization factor and a compression power from the raw speech, jointly with the rest of the architecture. We apply this model to dysarthria detection from sentence-level audio recordings. Starting from a strong attention-based baseline on which mel-filterbanks outperform standard low-level descriptors, we show that learning the filters or the normalization and compression improves over fixed features by 10% absolute accuracy. We also observe a gain over OpenSmile features by learning jointly the feature extraction, the normalization, and the compression factor with the architecture. This constitutes a first attempt at learning jointly all these operations from raw audio for a speech classification task.Comment: 5 pages, 3 figures, submitted to ICASS
