    Molecular diagnostics in tuberculosis

    Molecular diagnostics in tuberculosis has enabled rapid detection of Mycobacterium tuberculosis complex in clinical specimens, identification of mycobacterial species, detection of drug resistance, and typing for epidemiological investigation. In the laboratory diagnosis of tuberculosis, the nucleic acid amplification (NAA) test is rapid and specific but not as sensitive as culture of mycobacteria. The primary determinant of successful NAA testing for tuberculosis depends on the shedding of mycobacterial DNA in secretions from caseating granulomas and its dissemination into sterile body fluids or tissue biopsies. In multibacillary diseases with a high mycobacterial load, a positive Ziehl-Neelsen smear with a positive NAA test is diagnostic of active tuberculosis, whereas a positive Ziehl-Neelsen smear with a negative NAA test in the absence of inhibitors would indicate nontuberculous mycobacterial disease. The role of the NAA test is more important in paucibacillary diseases with low mycobacterial loads. The presence of polymerase chain reaction (PCR) inhibitors, however, especially in extrapulmonary specimens, may produce false-negative results. Although this problem can be overcome to some extent by extra extraction steps, the additional processing invariably leads to the loss of mycobacterial DNA. To circumvent this problem, a brief culture augmentation step is carried out before the NAA test is performed, which can enhance the mycobacterial load while concomitantly diluting inhibitors, thereby maintaining the sensitivity of the test without excessively increasing turnaround time.     Studying ICT supported pedagogical practices

    During the past decade there has been an exponential growth in the use of information and communication technology (ICT) and this has made pervasive impacts both on the society and on our daily lives. It is thus not surprising to find increasing interest, attention and investment put into the use of ICT in education all around the world. In addition to efforts to make use of ICT to improve learning, the emergence of the knowledge economy has also brought about in recent years a much greater emphasis on education and a number of masterplans in ICT in education has been produced in many countries. Such masterplans detailed not only strategies for implementation but more importantly embedded the plans within a broader framework of education reform that aimed to develop studentsā€™ capacities for self-learning, problem-solving, information seeking and analysis, critical thinking and the ability to communicate, collaborate and learn via the internet, abilities that figured much less importantly in the school curricula before. In this context, a new term, ā€œemerging pedagogical practiceā€, was used in SITES (the Second International Information Technology in Education Study, conducted under the auspices of the IEA) (Pelgrum, 1999) to highlight the changing pedagogical goals and practices that has resulted from the use of ICT in education, as opposed to those uses that just aimed at enhancing the effectiveness of ā€œtraditionally important pedagogical practicesā€. However, the concept of ā€œemerging pedagogical practiceā€ as depicted in the SITES Module 1 (M1) report is still rather vague. When schools have access to computers and the Internet to support teaching and learning, how would teachers and students make use of them? What impact has ICT made on classroom practices? What changes, if any, has ICT made on the roles of teachers and students and the interactions between them? Has the introduction of ICT in schools brought about the desired education reforms envisaged in the ICT in education masterplans or are these wishful optimisms? Are there more effective models of ICT implementation in schools, and if so what are their characteristics? In conjunction with the SITES M1 Hong Kong study, we have conducted a study of good practices in the use of ICT for teaching and learning using the case studies approach. The main goal of this extension study was to explore the above questions in the context of good practices as commonly recognized by members of the education community, and to develop ways of disseminating such good practices. This paper describes the conceptual framework and methodology used in this study and reports briefly on some key findings from the study. The research methodology used in this Study is based on a model of pedagogical practice that is couched within a broad curriculum framework where the pedagogical practice is the implemented curriculum. There are two parts to the research. The first part is to investigate and to build models of pedagogical practices involving use of ICT. Here the assumption (substantiated by observations) is that the way ICT is incorporated into classrooms is very much dictated by the teachersā€™ general beliefs and approaches to education. The second component of the methodology deals with the models (strategies) of educational change used in different schools in introducing ICT across the curriculum and to explore if particular models of pedagogical practice is linked with specific school implementation strategies.published_or_final_versio

    The counselling self-estimate inventory (COSE): Does it work in Chinese counsellors?

    Counselling self-efficacy is an important construct for research and evaluation in counsellors' competencies and training effectiveness. Larson et al. developed the Counselling Self-Estimate Inventory (COSE) for counsellors in America and examined its factor structure using exploratory factor analysis. They recommended a five-factor model (microskills, counselling process, difficult client behaviour, cultural competence, and awareness of values) and the use of the COSE for future research. However, little research has investigated the validity of the COSE in the context of counselling Chinese students in schools. In the present study, the factor structure of responses to the Chinese version of the Counselling Self-Estimate Inventory in a sample of 578 Hong Kong secondary school guidance teachers was examined using the EQS approach to confirmatory factor analysis. The results showed that while a five-factor model was fairly able to fit the data, the deletion of items related to the awareness of values factor yielded a better fitting model. The discussion of potential uses and limitations of the C-COSE in the context of preparing and supervising school guidance personnel in student counselling is relevant to counselling psychologists and researchers in Hong Kong and other parts of the world.postprin

    The Therapeutic Effect of Pamidronate on Lethal Avian Influenza A H7N9 Virus Infected Humanized Mice

    A novel avian influenza virus H7N9 infection occurred among human populations since 2013. Although the lack of sustained human-to-human transmission limited the epidemics caused by H7N9, the late presentation of most patients and the emergence of neuraminidase-resistant strains made the development of novel antiviral strategy against H7N9 in urgent demands. In this study, we evaluated the potential of pamidronate, a pharmacological phosphoantigen that can specifically boost human VĪ“2-T-cell, on treating H7N9 virus-infected humanized mice. Our results showed that intraperitoneal injection of pamidronate could potently decrease the morbidity and mortality of H7N9-infected mice through controlling both viral replication and inflammation in affected lungs. More importantly, pamidronate treatment starting from 3 days after infection could still significantly ameliorate the severity of diseases in infected mice and improve their survival chance, whereas orally oseltamivir treatment starting at the same time showed no therapeutic effects. As for the mechanisms underlying pamidronate-based therapy, our in vitro data demonstrated that its antiviral effects were partly mediated by IFN-Ī³ secreted from human VĪ“2-T cells. Meanwhile, human VĪ“2-T cells could directly kill virus-infected host cells in a perforin-, granzyme B- and CD137-dependent manner. As pamidronate has been used for osteoporosis treatment for more than 20 years, pamidronate-based therapy represents for a safe and readily available option for clinical trials to treat H7N9 infection.published_or_final_versio
