    Prevalence of Overweight and Obesity among Primary School Children Aged 8-13 Years in Dar es Salaam City, Tanzania

    Background. The understanding of obesity as a growing health problem in Africa and Tanzania in particular is hampered by lack of data as well as sociocultural beliefs in which overweight and obesity are revered. This study sought to determine the prevalence of overweight and obesity among primary school children aged 8-13 years in Dar es Salaam, Tanzania. Method. A cross-sectional analytical research design was used to study overweight and obesity in primary schools in Dar es Salaam, Tanzania. The target population was 150,000 children aged 8-13 years. Stratified random sampling was used to select 1781 children. Weight and height were taken and WHO standards for children were used to determine weight status. Results. Findings showed that the prevalence of overweight and obesity was 15.9% and 6.7%, respectively ( = 1781). However, 6.2% of the children were underweight. There were significant differences in mean BMI between children in private and public schools ( = 0.021), between male and female ( < 0.001), and across age groups of 8-10 and 11-13 years ( < 0.001).     Evidence of negative energy balance using doubly labelled water in elite Kenyan endurance runners prior to competition

    Previous studies have found Kenyan endurance runners to be in negative energy balance during training and prior to competition. The aim of the present study was to assess energy balance in nine elite Kenyan endurance runners during heavy training. Energy intake and expenditure were determined over 7d using weighed dietary intake and doubly labelled water, respectively. Athletes were on average in negative energy balance (mean energy intake 13 241 (SD 1330) kJ/d v. mean energy expenditure 14 611 (SD 1043) kJ/d; P=0·046), although there was no loss in body mass (mean 56·0 (SD 3·4) kg v. 55·7 (SD 3·6) kg; P=0·285). The calculation of underreporting was 13; (range −24 to +9%) and almost entirely accounted for by undereating (9% (range −55 to +39%)) as opposed to a lack of significant underrecording (i.e. total water intake was no different from water loss (mean 4·2 (SD 0·6) l/d v. 4·5 (SD 0·8) l/d; P=0·496)). Fluid intake was modest and consisted mainly of water (0·9 (SD 0·5) l/d) and milky tea (0·9 (SD 0·3) l/d). The diet was high in carbohydrate (67·3 (SD 7·8) %) and sufficient in protein (15·3 (SD 4·0) %) and fat (17·4 (SD 3·9) %). These results confirm previous observations that Kenyan runners are in negative energy balance during periods of intense training. A negative energy balance would result in a reduction in body mass, which, when combined with a high carbohydrate diet, would have the potential in the short term to enhance endurance running performance by reducing the energy cost of runnin

    Comparative study of physical activity patterns among school children in Kenya and Canada: Results from the ISCOLE Project

    Abstract Examination of the timing and patterns of daily activity are crucial in understanding when children accumulate the highest levels of physical activity. The objectives of this study were to examine moderate-to-vigorous physical activity (MVPA) patterns accrued by time of day among Kenyan children, and compare activity patterns in Kenya to those of Canadian children. Physical activity and body weights of participating children were measured by accelerometry and anthropometry, while supplementary self-report data were captured by questionnaires. Data were collected as part of a larger International Study of Childhood Obesity, Lifestyle and Environment (ISCOLE) in Nairobi for ISCOLE-Kenya and in the Ottawa Region for ISCOLE-Canada. A total of 555 Kenyan and 541 Canadian children 9 to 11 years were included in the analyses. In Kenya, boys, under/healthy weight, and children attending public (lower socioeconomic status (SES)) schools were found to have significantly higher MVPA levels compared to girls, overweight/obese, and children attending private (higher SES) schools respectively. MVPA on weekdays was higher than on weekend-days. Activity profiles among Kenyan and Canadian children were very similar; however, Kenyan children had significantly higher MVPA and lower sedentary time on weekend-days. MVPA patterns among urban Kenyan children were largely similar to those of urban Canadian children when assessed by sex, BMI category, and weekday/weekend days. However, in the Kenyan sample, unlike in many higher income countries, lower SES was associated with higher MVPA

    This study reports on the adaptation and test-retest reliability of the Neighborhood Environment Walkability Scale in seven sub-Saharan African countries (NEWS-Africa). The measure will be useful for surveillance of built environment conditions for planning purposes, and to evaluate physical activity and policy interventions in Africa, two effective strategies for controlling deaths from physical inactivity-related non-communicable diseases.     Health-related quality of life and lifestyle behavior clusters in school-aged children from 12 countries

    OBJECTIVE: To evaluate the relationship between children's lifestyles and health-related quality of life and to explore whether this relationship varies among children from different world regions.STUDY DESIGN: This study used cross-sectional data from the International Study of Childhood Obesity, Lifestyle and the Environment. Children (9-11 years) were recruited from sites in 12 nations (n = 5759). Clustering input variables were 24-hour accelerometry and self-reported diet and screen time. Health-related quality of life was self-reported with KIDSCREEN-10. Cluster analyses (using compositional analysis techniques) were performed on a site-wise basis. Lifestyle behavior cluster characteristics were compared between sites. The relationship between cluster membership and health-related quality of life was assessed with the use of linear models.RESULTS: Lifestyle behavior clusters were similar across the 12 sites, with clusters commonly characterized by (1) high physical activity (actives); (2) high sedentary behavior (sitters); (3) high screen time/unhealthy eating pattern (junk-food screenies); and (4) low screen time/healthy eating pattern and moderate physical activity/sedentary behavior (all-rounders). Health-related quality of life was greatest in the all-rounders cluster.CONCLUSIONS: Children from different world regions clustered into groups of similar lifestyle behaviors. Cluster membership was related to differing health-related quality of life, with children from the all-rounders cluster consistently reporting greatest health-related quality of life at sites around the world. Findings support the importance of a healthy combination of lifestyle behaviors in childhood: low screen time, healthy eating pattern, and balanced daily activity behaviors (physical activity and sedentary behavior).TRIAL REGISTRATION: ClinicalTrials.gov: NCT01722500.</p

    Relationships between parental education and overweight with childhood overweight and physical activity in 9-11 year old children : results from a 12-country study

    Background: Globally, the high prevalence of overweight and low levels of physical activity among children has serious implications for morbidity and premature mortality in adulthood. Various parental factors are associated with childhood overweight and physical activity. The objective of this paper was to investigate relationships between parental education or overweight, and (i) child overweight, (ii) child physical activity, and (iii) explore household coexistence of overweight, in a large international sample. Methods: Data were collected from 4752 children (9-11 years) as part of the International Study of Childhood Obesity, Lifestyle and the Environment in 12 countries around the world. Physical activity of participating children was assessed by accelerometry, and body weight directly measured. Questionnaires were used to collect parents' education level, weight, and height. Results: Maternal and paternal overweight were positively associated with child overweight. Higher household coexistence of parent-child overweight was observed among overweight children compared to the total sample. There was a positive relationship between maternal education and child overweight in Colombia 1.90 (1.23-2.94) [odds ratio (confidence interval)] and Kenya 4.80 (2.21-10.43), and a negative relationship between paternal education and child overweight in Brazil 0.55 (0.33-0.92) and the USA 0.54 (0.33-0.88). Maternal education was negatively associated with children meeting physical activity guidelines in Colombia 0.53 (0.33-0.85), Kenya 0.35 (0.19-0.63), and Portugal 0.54 (0.31-0.96). Conclusions: Results are aligned with previous studies showing positive associations between parental and child overweight in all countries, and positive relationships between parental education and child overweight or negative associations between parental education and child physical activity in lower economic status countries. Relationships between maternal and paternal education and child weight status and physical activity appear to be related to the developmental stage of different countries. Given these varied relationships, it is crucial to further explore familial factors when investigating child overweight and physical activity

    A model for presenting accelerometer paradata in large studies : ISCOLE

    Background: We present a model for reporting accelerometer paradata (process-related data produced from survey administration) collected in the International Study of Childhood Obesity Lifestyle and the Environment (ISCOLE), a multi-national investigation of >7000 children (averaging 10.5 years of age) sampled from 12 different developed and developing countries and five continents. Methods: ISCOLE employed a 24-hr waist worn 7-day protocol using the ActiGraph GT3X+. Checklists, flow charts, and systematic data queries documented accelerometer paradata from enrollment to data collection and treatment. Paradata included counts of consented and eligible participants, accelerometers distributed for initial and additional monitoring (site specific decisions in the face of initial monitoring failure), inadequate data (e.g., lost/malfunction, insufficient wear time), and averages for waking wear time, valid days of data, participants with valid data (>= 4 valid days of data, including 1 weekend day), and minutes with implausibly high values (>= 20,000 activity counts/min). Results: Of 7806 consented participants, 7372 were deemed eligible to participate, 7314 accelerometers were distributed for initial monitoring and another 106 for additional monitoring. 414 accelerometer data files were inadequate (primarily due to insufficient wear time). Only 29 accelerometers were lost during the implementation of ISCOLE worldwide. The final locked data file consisted of 6553 participant files (90.0% relative to number of participants who completed monitoring) with valid waking wear time, averaging 6.5 valid days and 888.4 minutes/day (14.8 hours). We documented 4762 minutes with implausibly high activity count values from 695 unique participants (9.4% of eligible participants and Conclusions: Detailed accelerometer paradata is useful for standardizing communication, facilitating study management, improving the representative qualities of surveys, tracking study endpoint attainment, comparing studies, and ultimately anticipating and controlling costs.Peer reviewe
