    Oral granuloma with chronic graft-vs-host disease

    BACKGROUND Oral mucositis is often observed with graft-versus-host disease (GVHD); however, the occurrence of oral granuloma is rare. The rapid increase in granulomatous lesions should be distinguished from malignant tumors in patients with GVHD because malignant diseases can develop in those patients. This case is the youngest pediatric patient with granuloma associated with GVHD. CASE SUMMARY The patient was a 1-year and 5-mo-old girl who presented to our department for the management of oral nodules. At the age of 5 mo, she was diagnosed with primary immunodeficiency disease, cord blood transplant was performed at 11 mo and bone marrow transplant at 1 year of age. After transplantation, GVHD and oral mucositis developed, and tacrolimus was administered. Interestingly, nodules appeared on the lower lip and buccal mucosa, which spontaneously disappeared. Then, a new nodule appeared on the left lateral border of the tongue. Resection was performed and the histopathological diagnosis was granuloma. The origin of these nodules were considered to be the fibroblasts activated under inflammation caused by GVHD because the calcineurin inhibitor tacrolimus acted on their proliferation. CONCLUSION It is very important to distinguish oral granulomatous lesions from malignancies if GVHD is present at the base and if immunosuppressive agents and steroids are being administered

    Mineralogy and crystallography of some Itokawa particles returned by the Hayabusa asteroidal sample return mission

    We studied seven Itokawa particles provided by the Japan Aerospace Exploration Agency (JAXA) as first International Announcement of Opportunity (AO) study mainly using electron and synchrotron radiation X-ray beam techniques. All the analyzed particles were collected from the first-touchdown site and composed of olivine and plagioclase with traces of Ca phosphate and chromite, and do not contain pyroxenes. Optical microscopy of these particles shows minor undulatory extinction of olivine and plagioclase, suggesting minor shock metamorphism (shock stage: S2). The electron microprobe analysis shows that olivine is Fo(70-73) and plagioclase is An(13-10)Or(5-7). The synchrotron radiation X-ray diffraction (SR-XRD) analysis of olivine crystals gives cell dimensions of a = 4.708 to 4.779 angstrom, b = 10.271 to 10.289 angstrom, c = 6.017 to 6.024 angstrom, corresponding to the Fo content of Fo(similar to 70) by Vegard's law. This composition matches the result obtained by the electron microprobe analysis. The olivine compositions of the analyzed particles are consistent with those of LL chondrites. The cell dimensions of two plagioclase crystals (a = 8.180 to 8.194 angstrom, b = 12.53 to 12.893 angstrom, c = 7.125 to 7.23 angstrom, a = 92.6 degrees to 93.00 degrees, beta = 116.36 degrees to 116.75 degrees, gamma = 90.03 degrees to 90.17 degrees) indicate that their equilibration temperatures are 800 degrees C +/- 10 degrees C. This temperature is near the peak metamorphic temperature recorded by equilibrated ordinary chondrites. The size of plagioclase crystals and the homogeneity of olivine compositions indicate that their petrologic type is >= 5. We also analyzed plagioclase by SR iron X-ray absorption near-edge structure (SR-XANES) and found that its Fe3+/(Fe2+ + Fe3+) ratio is approximately 0.5. Such high Fe3+ abundance indicates the formation under a relatively oxidizing environment. Thus, all these analyses have reconfirmed that the Itokawa particles returned by the Hayabusa spacecraft are very weakly shocked equilibrated LL chondrites, which matches the results of the preliminary examination team

    Changes in serum antibody titers after vaccination for COVID-19 and evaluation of post-vaccination health conditions

     Introduction: The coronavirus disease 2019 (COVID-19) vaccine has preventive effects and high immunogenicity, but the outcomes of vaccination have not been widely reported. The goal of this study was to examine serum antibody titers before and after vaccination and to evaluate post-vaccination health conditions. Methods: The subjects were 2,304 medical workers (mean age 41 years) at Kawasaki Gakuen who agreed to participate in the study and underwent COVID-19 vaccination, beginning in March 2021. Serum IgG antibody titers for SARS-CoV-2 spike protein were measured before the first vaccination and 4 weeks after the second vaccination. Health conditions were observed for 4 weeks after the second vaccination. Results: The rates of seroconversion, seroprotection, and change in geometric mean antibody titer at 4 weeks after the second vaccination were 99.9%, 99.9%, and 2,685.5 (95% CI 587.8-5,319.2), respectively, suggesting high immunogenicity. After the first vaccination, pain, enlargement, and reddening occurred at the local injection site, and systemic side effects included fatigue, headache, physical pain, chill, nausea, and fever. After the second vaccination, the incidence of pain decreased, but those of other events increased. There were no serious side effects requiring hospitalization. In logistic regression analysis, sex, age, fever,chill, and lymph node enlargement after the second vaccination were associated with a change in antibody titer. Conclusions: Serum antibody titers suggested high immunogenicity of the COVID-19 vaccine and a health condition survey confirmed the safety of the vaccine. Systemic side effects may serve as an index of immunization (acquisition of antibody) by the vaccine

The study was a quantitative research methods such as surveys of descriptive inferential research with cross sectional approach. Number of samples in this research is 285 sample in inpatient and 140 in emergency room. The instrument used a questionnaire. Analysis of data using multiple linear regression. This study show that there is the influence of therapeutic communication nurse to satisfaction of outpatients and Emergency room in RSUD Yogyakarta, and orientation phase is a phase that most influence on patient satisfaction. The most influential to therapeutic communication is termination stage

    A dehydrated space-weathered skin cloaking the hydrated interior of Ryugu

    Without a protective atmosphere, space-exposed surfaces of airless Solar System bodies gradually experience an alteration in composition, structure and optical properties through a collective process called space weathering. The return of samples from near-Earth asteroid (162173) Ryugu by Hayabusa2 provides the first opportunity for laboratory study of space-weathering signatures on the most abundant type of inner solar system body: a C-type asteroid, composed of materials largely unchanged since the formation of the Solar System. Weathered Ryugu grains show areas of surface amorphization and partial melting of phyllosilicates, in which reduction from Fe3+ to Fe2+ and dehydration developed. Space weathering probably contributed to dehydration by dehydroxylation of Ryugu surface phyllosilicates that had already lost interlayer water molecules and to weakening of the 2.7 µm hydroxyl (–OH) band in reflectance spectra. For C-type asteroids in general, this indicates that a weak 2.7 µm band can signify space-weathering-induced surface dehydration, rather than bulk volatile loss

    Pollen food allergy syndrome caused by Japanese radish: A case report

    Pollen food allergy syndrome (PFAS) is a food allergy that manifests as hypersensitivity symptoms of the oropharyngeal mucosa on ingesting specific foods, and findings resemble herpetic gingivostomatitis. Few reports of PFAS caused by consuming radishes are found in the literature. A 31-year-old man presented to our department with stomatitis and pharyngeal pain. He had no history of allergies. Herpetic gingivostomatitis was suspected. He was admitted to the emergency room a few days later complaining of oral and epigastric pain. Symptoms were similar to those reported previously. He reported frequently consuming raw Japanese radish (Raphans sativus L.) which gave rise to his symptoms. Japanese radish was suspected as the allergen. The skin-prick test confirmed the diagnosis of PFAS. PFAS can be diagnosed easily once the food-causing symptoms are identified. Upon encountering widespread erosion in the oral cavity, it is essential to consider PFAS as the possible cause

    A Case of Ameloblastoma Removed Using a Three-dimensional Transparent Plastic Jaw Model Which Can Visualize Internal Jawbone Structures

    Construction of three-dimensional (3D) models of various organs from CT images using 3D printers has recently become possible. In oral surgery, surgical simulation using a 3D printed jaw model is being widely used. With the conventional use of plaster for jaw modeling, the structures in the jawbone cannot be clearly visualized. Here, we report a case of a mandibular ameloblastoma extirpated using a 3D transparent plastic jaw model which can visualize the structures in the jawbone and assist in recognition of the anatomical position of a tumor. The patient was a 23-year-old woman who visited our hospital in 2016 with a complaint of swelling of the right mandible. Following the diagnosis of ameloblastoma, the tumor was removed. However, after 1 year and 7 months, tumor recurrence was suspected under the mandibular canal. Using a 3D jaw model for visualizing the inside of the jawbone, the tumor was removed with curettage of the surrounding tissue with minimal surgical invasion under general anesthesia. No recurrence has been observed 8 months post-surgery. Therefore, the use of the 3D jaw model facilitated recognition of the anatomical position of the tumor, which resulted in safe and reliable tumor removal
