A férfi és női testek medikalizációja a XIX. századtól napjainkig
The thesis focuses on the phenomenon of medicalization, especially as applied to the male and female body. I examine the normalizing effect of expansive medical control from the 19th century to date. Medicalization is essentially understood as a normalizing process, which, due to the position of power occupied by biomedicine, constructs and reconstructs moral imperatives (norms). The concept itself was elaborated in its various aspects by authors such as Peter Conrad, Ivan Illich, Irving Kenneth Zola, or Michel Foucault in the 1960’s and 1970’s. In the course of about two hundred years covered by the thesis, the discourse about the male and female gender and sex had a profound effect on the framework in which health and illness were conceptualized in connection with a wide range of life events. The case-centered analysis of the areas dealt with in the thesis (such as sexual health, or the changes that accompany aging) offers an opportunity to discuss the criticism of medicalization within a solid conceptual framework

hogy amennyiben a részletes családi anamnézis, a beteg életkora, esetleges megelőző daganatos betegsége és nem utolsósorban a daganatjellemzők alapján felmerül, gondoljon patogén BRCA-mutáció jelenlétére, mérlegelje a teszt indokoltságát, illetve vesse fel a beteg számára a lehetőséget.

SUMMARY – Germinal BRCA status infl uences patient care both in early and advanced/metastatic breast cancer. Ideally, the patient should make the decision on the type of surgery or the avoidance of radiotherapy being aware of the BRCA status; based on the most recent clinical studies, this knowledge may infl uence the type of chemotherapy in the neoadjuvant, adjuvant, or metastatic setting or may raise the use of emerging
targeted therapies. DNA-targeting cytostatic agents, mostly platinum agents and PARP inhibitors that act by inducing synthetic lethality, provide specifi c therapies in BRCA-mutant cases. The optimum place and sequence of these specifi c agents in treatment, however, are not known yet. International guidelines promote BRCA testing for the specifi cation of treatment strategy in all HER2-negative advanced/metastatic breast cancer cases (NCCN) or at least in all cases when, based on certain predictors, the presence of mutations
is likely (ESMO). Recently, the methods employed for BRCA testing have improved immensely and are widely available through the services of various providers. For the identifi cation of the mutation, sequencing of the whole genes is needed, which can be achieved faster and more cost-effi ciently using next-generation sequencing (NGS) platforms compared to previous methods. It i s the responsibility of the physician to consider
the possibility of BRCA mutations and to raise the issue of BRCA testing to the patient if the family history, the age, previous malignant disease(s) of the patient, or the cancer features are suggestive of genetic risk.

Dohányzó várandósok leszokásának orvosi támogatása = Medical support of cessation for pregnant smokers
A dohányzĂł nĹ‘k Ă©letĂĽk során a legnagyobb valĂłszĂnűsĂ©ggel a várandĂłsság során szoknak le a dohánytermĂ©kek használatárĂłl. Ez az idĹ‘szak az egĂ©szsĂ©gĂĽgy számára kiválĂł esĂ©ly, amely során kettĹ‘s – anyai Ă©s magzati – egĂ©szsĂ©gnyeresĂ©g Ă©rhetĹ‘ el a leszokási kĂsĂ©rlet szakszerű támogatásával. A dohányzás felfĂĽggesztĂ©se kiemelt jelentĹ‘sĂ©gű a várandĂłsság alatt a magzat Ă©s az anya Ă©rdekĂ©ben egyaránt. Ă–sszefoglalĂł kĂ©ziratunk irodalmi áttekintĂ©s, melynek cĂ©lja, hogy aktuális Ă©s praktikus kiegĂ©szĂtĹ‘ informáciĂłkkal szolgáljon a várandĂłs dohányzĂłk leszokását támogatĂł szakemberek számára. A jelen áttekintĂ©s a PubMed adatbázisban elĂ©rhetĹ‘ publikáciĂłkon, valamint hazai Ă©s nemzetközi irányelveken Ă©s összefoglalĂłkon alapul, melyek gyakorlati jelentĹ‘sĂ©gĂĽk alapján kerĂĽltek beválogatásra. A várandĂłsság alatti dohányzás szĂĽlĂ©szeti, neonatolĂłgiai Ă©s pediátriai szövĹ‘dmĂ©nyeit tanácsos pozitĂvan átkeretezni, inkább a leszokás elĹ‘nyeirĹ‘l beszĂ©lni. A minimálintervenciĂłt ajánlott kiegĂ©szĂteni a gesztáciĂłs hĂ©tnek megfelelĹ‘ tanácsadással, szakellátásra irányĂtással, illetve a várandĂłs nĹ‘ dohányzĂł partnerĂ©t cĂ©lzĂł leszokástámogatással. VárandĂłsok számára elsĹ‘dlegesen a gyĂłgyszermentes leszokás javasolhatĂł, ha azonban ez nem lehetsĂ©ges vagy eredmĂ©nytelen, indokolt lehet a nikotinpĂłtlĂł kĂ©szĂtmĂ©nyek használata. Az általános nikotinpĂłtlĂł kezelĂ©stĹ‘l alapvetĹ‘en három pontban tĂ©r el a várandĂłsok nikotinpĂłtlĂł terápiája. Érdemes inkább az orális kĂ©szĂtmĂ©nyeket használni a transdermalis nikotinbevitel helyett; Ăłvatosabb titrálási idĹ‘szak ajánlott; Ă©s a kezelĂ©s idĹ‘tartamát tanácsos lerövidĂteni. A várandĂłs dohányzĂłk leszokástámogatása során a magatartás-orvoslási mĂłdszerek intenzĂv alkalmazása javasolt. Nagy hangsĂşly helyezĹ‘dik a stresszkezelĂ©sre, az Ă©rzelemszabályozásra, a viselkedĂ©sbeli Ă©s biolĂłgiai visszajelzĂ©sekre, az önjutalmazásra Ă©s a kĂĽlsĹ‘ ösztönzĹ‘k használatára. Ezek a mĂłdszerek a kognitĂv viselkedĂ©sterápiás modellen Ă©s a motiváciĂłs interjĂşs technikákon alapulnak, amelyekre több gyakorlati pĂ©ldát is bemutatunk az összefoglalĂł közlemĂ©nyben. Orv Hetil. 2023; 164(30): 1194–1203. | Female smokers are most likely to quit using tobacco products during pregnancy. This period is an excellent chance
for the health sector to achieve dual – maternal and fetal – health benefits with the professional support of the cessation attempt. In our review, we collected the practicalities of this specific cessation support. This review is based on
publications available in the PubMed database as well as domestic and international guidelines and summaries, which
were selected based on their practical importance. Quitting smoking during pregnancy is really important for both
the foetus and the mother. Reframing the obstetric, neonatal and pediatric complications of smoking during pregnancy positively, and focusing on the benefits of quitting are recommended. Minimal intervention is advised to be
complemented with counselling relating to the gestational age, referral to specialised care service and cessation support for the pregnant mother’s partner. Non-pharmacological support is the primary recommended therapy for
pregnant smokers. If it is not possible, or is unsatisfactory, the use of nicotine replacement therapy may be reasonable.
Nicotine replacement therapy for pregnant smokers differs from the general nicotine replacement treatments in the
following: use of oral formulations over transdermal nicotine intake; more cautious titration period; shorter treatment duration. Using behaviour interventions is also advised to support smoking cessation during pregnancy. The
emphasis is on stress management, emotion regulation, behavioural and biological feedback, self-reward and use of
external incentives. These methods are based on the cognitive behavioural therapy model and motivational interviewing techniques. Practical examples are presented in this summary
