    Recent seismic activites in Tokai,Tonankai and Nankai regions,hightened the desire for more disaster prevention activities.The reduction of damage in earthquake disaster can be assured by hard and soft prevention measures.Soft measure,namely maintains the cooperation between municipalities and local residents or independent voluntarily disaster prevention organization(self-support,or government supported) to increase the public awareness about the importance of disaster prevention measures and required activities.The main obejective of this research is fact finding and evaluating the the actual condition of the independent disaster prevention organizations of cities,towns and a village and their potencials for information contribution,interaction and colaboration during a major disaster using questionnaire survey.In this paper we present our research and discussion on cities,towns and a village in centeral region of Kanagawa Prefecture and accumulate the results with serveys of fiscal year 2006 in westen region

    Neurophysiological Analysis of Intermanual Transfer in Motor Learning

    The purpose of this study was to examine the effect of motor training on motor imagery (MI), by comparing motor performance and motor cortex excitability changes with and without intermanual transfer of motor learning. Intermanual transfer was investigated in terms of excitability changes in the motor cortex and motor performance from right hand training to left hand performance. Participants were assigned to a transfer training group and a control group. We recorded motor evoked potentials (MEPs) induced by transcranial magnetic stimulation (TMS), applied to the left extensor carpi radialis (ECR) both with and without intermanual transfer. The results showed that after learning by the right hand, MEPs decreased during left hand MI. MEPs during MI were significantly decreased by unilateral training in the transfer training group. Since intermanual transfer plays an important role in stabilizing performance by the contralateral side, this result suggests that unilateral training decreases MEPs during MI on the contralateral side. In the control group, without right hand training, MEPs significantly increased after left hand training during MI. In the trained side, we found increased excitability in the agonist muscle area of the primary motor cortex. However, in the untrained side, excitability decreased in the homonymous muscle area of the primary motor cortex. This constitutes an increase in inhibitory effects and suggests that excitability changes in the respective neural circuit contribute to skilled performance by the ipsilateral and contralateral sides in the same motor task

    ABSTRACT Based on the Berg-Barrett method, Scanning Energy Dispersive X-ray Diffraction Microscopy (SEDXDM) has been developed for nondestructively evaluating both the surface and subsurface of poly-erystallized materials. The SEDXDM includes an X-ray source generating continuous X-ray spectrum as a key component to form a highly resolved 2-D horizontal cross-sectional digital image. This article presents the evaluation of the fatigue damage (e.g., slip line and slip band caused by fatigue deformation) of the coarse-grained aluminum, which is made by means of annealing and has been repeatedly bent to generate the stress and make the slope of the stress created in the thickness direction of the plate specimen, under fully reversed anti-plane bending condition with the SEDXDM

    Dietary intake, mental status, physical activity, and lifestyle affecting bowel movement frequency and stool texture in young Japanese women

    Constipation is a symptom-based disorder, and its definition is mainly subjective. Patients are more concerned with ease ofpassage and consistency rather than frequency of bowel movement. Studies on bowel movement frequency and stool texture inthe general population are sparse, especially in young women. In this cross-sectional study, data obtained from self-administered questionnaires, including age, height, body weight, lifestyle, food habits, anxiety, depressive status, frequency of bowel movements,stool texture, and defecation-related symptoms were analyzed in 245 female Japanese university students. An establishedsemiquantitative questionnaire available for clinical investigation (FFQg) was used to obtain a detailed assessment of food intake and physical activity levels. Of the participants, 21.4% had bowel movements ?3 times per week and 33.3% had hard or lumpy stools ?25% and loose (mushy) or watery stools <25% of bowel movements. There was a positive association between infrequent bowel movements and hard or lumpy stools. These two situations both caused similar symptoms such as a sensation of incomplete evacuation and straining. There was no association of bowel movement frequency and stool texture with any specificnutrients and foods, dietary intake, mental status, or physical activity. Several lifestyle factors such as regular bowel movements and hesitation with evacuation were associated with bowel movement frequency and stool texture.     Daily Feeding of Fructooligosaccharide or Glucomannan Delays Onset of Senescence in SAMP8 Mice

    We hypothesized that daily intake of nondigestible saccharides delays senescence onset through the improvement of intestinal microflora. Here, we raised senescence accelerated mice prone 8 (SAMP8) on the AIN93 diet (CONT), with sucrose being substituted for 5% of fructooligosaccharide (FOS) or 5% of glucomannan (GM), 15 mice per group. Ten SAMR1 were raised as reference of normal aging with control diet. Grading of senescence was conducted using the method developed by Hosokawa, and body weight, dietary intake, and drinking water intake were measured on alternate days. Following 38 weeks of these diets we evaluated learning and memory abilities using a passive avoidance apparatus and investigated effects on the intestinal microflora, measured oxidative stress markers, and inflammatory cytokines. Continuous intake of FOS and GM significantly enhanced learning and memory ability and decelerated senescence development when compared with the CONT group. Bifidobacterium levels were significantly increased in FOS and GM-fed mice. Urinary 8OHdG, 15-isoprostane, serum TNF-α, and IL-6 were also lower in FOS-fed mice, while IL-10 in FOS and GM groups was higher than in CONT group. These findings suggest that daily intake of nondigestible saccharides delays the onset of senescence via improvement of intestinal microflora

    Randomised, multicentre prospective trial of transarterial chemoembolisation (TACE) plus sorafenib as compared with TACE alone in patients with hepatocellular carcinoma: TACTICS trial.

    OBJECTIVE:This trial compared the efficacy and safety of transarterial chemoembolisation (TACE) plus sorafenib with TACE alone using a newly established TACE-specific endpoint and pre-treatment of sorafenib before initial TACE. DESIGN:Patients with unresectable hepatocellular carcinoma (HCC) were randomised to TACE plus sorafenib (n=80) or TACE alone (n=76). Patients in the combination group received sorafenib 400 mg once daily for 2-3 weeks before TACE, followed by 800 mg once daily during on-demand conventional TACE sessions until time to untreatable (unTACEable) progression (TTUP), defined as untreatable tumour progression, transient deterioration to Child-Pugh C or appearance of vascular invasion/extrahepatic spread. Co-primary endpoints were progression-free survival (PFS), which is not a conventional one but defined as TTUP, or time to any cause of death plus overall survival (OS). Multiplicity was adjusted by gatekeeping hierarchical testing. RESULTS:Median PFS was significantly longer in the TACE plus sorafenib than in the TACE alone group (25.2 vs 13.5 months; p=0.006). OS was not analysed because only 73.6% of OS events were reached. Median TTUP (26.7 vs 20.6 months; p=0.02) was also significantly longer in the TACE plus sorafenib group. OS at 1 year and 2 years in TACE plus sorafenib group and TACE alone group were 96.2% and 82.7% and 77.2% and 64.6%, respectively. There were no unexpected toxicities. CONCLUSION:TACE plus sorafenib significantly improved PFS over TACE alone in patients with unresectable HCC. Adverse events were consistent with those of previous TACE combination trials.     Quantitative assessment of harmonic power doppler myocardial perfusion imaging with intravenous levovist™ in patients with myocardial infarction: comparison with myocardial viability evaluated by coronary flow reserve and coronary flow pattern of infarct-related artery

    BACKGROUND: Myocardial contrast echocardiography and coronary flow velocity pattern with a rapid diastolic deceleration time after percutaneous coronary intervention has been reported to be useful in assessing microvascular damage in patients with acute myocardial infarction. AIM: To evaluate myocardial contrast echocardiography with harmonic power Doppler imaging, coronary flow velocity reserve and coronary artery flow pattern in predicting functional recovery by using transthoracic echocardiography. METHODS: Thirty patients with anterior acute myocardial infarction underwent myocardial contrast echocardiography at rest and during hyperemia and were quantitatively analyzed by the peak color pixel intensity ratio of the risk area to the control area (PIR). Coronary flow pattern was measured using transthoracic echocardiography in the distal portion of left anterior descending artery within 24 hours after recanalization and we assessed deceleration time of diastolic flow velocity. Coronary flow velocity reserve was calculated two weeks after acute myocardial infarction. Left ventricular end-diastolic volumes and ejection fraction by angiography were computed. RESULTS: Pts were divided into 2 groups according to the deceleration time of coronary artery flow pattern (Group A; 20 pts with deceleration time ≧ 600 msec, Group B; 10 pts with deceleration time < 600 msec). In acute phase, there were no significant differences in left ventricular end-diastolic volume and ejection fraction (Left ventricular end-diastolic volume 112 ± 33 vs. 146 ± 38 ml, ejection fraction 50 ± 7 vs. 45 ± 9 %; group A vs. B). However, left ventricular end-diastolic volume in Group B was significantly larger than that in Group A (192 ± 39 vs. 114 ± 30 ml, p < 0.01), and ejection fraction in Group B was significantly lower than that in Group A (39 ± 9 vs. 52 ± 7%, p < 0.01) at 6 months. PIR and coronary flow velocity reserve of Group A were higher than Group B (PIR, at rest: 0.668 ± 0.178 vs. 0.248 ± 0.015, p < 0.0001: during hyperemia 0.725 ± 0.194 vs. 0.295 ± 0.107, p < 0.0001; coronary flow velocity reserve, 2.60 ± 0.80 vs. 1.31 ± 0.29, p = 0.0002, respectively). CONCLUSION: The preserved microvasculature detecting by myocardial contrast echocardiography and coronary flow velocity reserve is related to functional recovery after acute myocardial infarction
