    The Benefits of Combined Anti-platelet Treatment in Carotid Artery Stenting

    AbstractObjectiveTo assess the benefits of a combined anti-platelet regime of aspirin and clopidogrel in carotid artery stenting.MethodsA randomised controlled trial was performed comparing aspirin and 24-h heparin with aspirin and clopidogrel for patients undergoing carotid artery stenting. Outcome measures included 30-day bleeding and neurological complications and 30-day stenosis rates.ResultsBleeding complications (groin haematoma or excessive bleeding at the groin site) occurred in 17% of the heparin and 9% of the clopidogrel group (p=0.35; n.s). The neurological complication rate in the 24-h heparin group was 25% compared to 0% in the clopidogrel group (p=0.02). The 30-day 50–100% stenosis rates were 26% in the heparin group and 5% in the clopidogrel group (p=0.10; n.s).ConclusionsThe dual anti-platelet regime has a significant impact on reducing adverse neurological outcomes without an additional increase in bleeding complications. This study was terminated prematurely due to an unacceptable level of complications in the heparin arm of the trial

    Exchange Rate Misalignment, Volatility and Import Flows in Malaysia

    This paper investigates the effect of real exchange rate misalignment and volatility on Malaysian import flows during 1991:Q1 to 2003:Q4. A measure of the quantitative proxy of the real exchange rate misalignment is constructed using the Natural Real Exchange Rate (NATREX) equilibrium model, whereas the volatility of real exchange rate is generated from the GARCH model. This paper differs from existing literature as the effects of exchange rate misalignment significantly hastened the level of Malaysian imports for period of the study. The empirical results also show that the exchange rate volatility has merely promoted the Malaysian imports during the crisis period. This suggests that the exchange rate misalignment and volatility are important determinants in inspiring Malaysian import flows, especially during the 1997 Asian financial crisis

    Using a generalized Weyl formalism, we show how stationary, axisymmetric solutions of the four-dimensional vacuum Einstein equation can be turned into static, axisymmetric solutions of five-dimensional dilaton gravity coupled to a two-form gauge field. This procedure is then used to obtain new solutions of the latter theory describing pairs of extremal magnetic black holes with opposite charges, known as black diholes. These diholes are kept in static equilibrium by membrane-like conical singularities stretching along two different directions.     Economic Viability of Tectona grandis sole Cropping and Intercropping for 20 years Planting Project

    Economic viability of Tectona grandis sole cropping and intercropping for 20 years planting project were carried at the Boonrich plantation in Lahad Datu, Sabah. The study was conducted to evaluate the economic viability and to compare between T. grandis sole cropping and intercropping of T. grandis with Salacca zalaca. The data of height, diameter, cost and prices were analysed. The parameter that used to measure the economic viability of the project were net present value (NPV), internal rate of return (IRR), benefit cost ratio (BCR), land equivalent ratio (LER), sensitivity analysis (SA) and payback period. The analysis was carrying out by using Microsoft excel. From the study, the NPV of intercropping is RM27,063.70 (USD8,841.54), the LER is 1.37, 23.68% of IRR and have a shorter payback period (13 years) compare to the sole cropping. As a conclusion, the intercropping can give a higher profit to the company, not only that it has a shorter payback period than the sole cropping. In addition, it also showed a higher yield as the LER is more than one (1.37). Therefore, T. grandis intercrop with S. zalaca will gain extra benefit and greater output than the sole cropping

    Continuously Guided Atomic Interferometry Using a Single-Zone Optical Excitation: Theoretical Analysis

    In an atomic interferometer, the phase shift due to rotation is proportional to the area enclosed by the split components of the atom. However, this model is unclear for an atomic interferometer demonstrated recently by Shahriar et al., for which the atom simply passes through a single-zone optical beam, consisting of a pair of bichromatic counter-propagating beams. During the passage, the atomic wave packets in two distinct internal states couple to each other continuously. The two internal states trace out a complicated trajectory, guided by the optical beams, with the amplitude and spread of each wavepacket varying continuously. Yet, at the end of the single-zone excitation, there is an interference with fringe amplitudes that can reach a visibility close to unity. For such a situation, it is not clear how one would define the area of the interferometer, and therefore, what the rotation sensitivity of such an interferometer would be. In this paper we analyze this interferometer in order to determine its rotation sensitivity, and thereby determine its effective area. In many ways, the continuous interferometer (CI) can be thought of as a limiting version of the Borde-Chu Interferometer (BCI). We identify a quality factor that can be used to compare the performance of these interferometers. Under conditions of practical interest, we show that the rotation sensitivity of the CI can be comparable to that of the BCI. The relative simplicity of the CI (e.g., elimination of the task of precise angular alignment of the three zones) then makes it a potentially better candidate for practical atom interferometry for rotation sensing.Comment: 34 page

    Objective: To investigate the allele and genotype frequencies of NFKBI -94 ins/de! ATTG (rs28720239) polymorphism and to evaluate the association between the polymorphism and colorectal cancer (CRC) risk in Malaysian population. Methods: Genomic DNA was extracted from the peripheral blood samples of 474 study subjects, which consisted of 237 histopathologically confirmed CRC patients and an equal number of cancer-free controls. The NFKBI -94 ins/de! ATTG (rs28720239) polymorphism was genotyped using polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) method and confirmed by DNA sequencing. The association between the polymorphic genotypes and CRC risk was evaluated by deriving odds ratios (ORs) and 95% confidence intervals (Cls) using unconditional logistic regression analysis. Results: The frequencies of wildtype (del/del), heterozygous ( del/ins) and variant (ins/ins) genotypes in CRC patients were 31.7%, 53.6% and 14.8%, respectively, while those in cancer-free controls were 35.0%, 58.2% and 6.8%, respectively. The frequency of the variant genotype was significantly higher in cases compared to controls (P < 0.01 ). Evaluation of the risk association of the polymorphic genotypes revealed that the variant genotype could contribute to a significantly increased risk of CRC (OR= 2.42, 95% CI= 1.24-4.73, P < 0.01 ). Conclusions: The variant allele of NFKBI -94 ins/de! ATTG (rs28362491) polymorphism is associated with higher risk of sporadic CRC in Malaysian population

    MEF2 is an in vivo immune-metabolic switch

    Infections disturb metabolic homeostasis in many contexts, but the underlying connections are not completely understood. To address this, we use paired genetic and computational screens in Drosophila to identify transcriptional regulators of immunity and pathology and their associated target genes and physiologies. We show that Mef2 is required in the fat body for anabolic function and the immune response. Using genetic and biochemical approaches, we find that MEF2 is phosphorylated at a conserved site in healthy flies and promotes expression of lipogenic and glycogenic enzymes. Upon infection, this phosphorylation is lost, and the activity of MEF2 changes—MEF2 now associates with the TATA binding protein to bind a distinct TATA box sequence and promote antimicrobial peptide expression. The loss of phosphorylated MEF2 contributes to loss of anabolic enzyme expression in Gram-negative bacterial infection. MEF2 is thus a critical transcriptional switch in the adult fat body between metabolism and immunity
