    Diagnosis of Overt Disseminated Intravascular Coagulation: A Comparative Study Using Criteria from the International Society Versus the Korean Society on Thrombosis and Hemostasis

    PURPOSE: Since 1993, Koreans have used diagnostic criteria set by the Korean Society on Thrombosis and Hemostasis (KSTH) in the diagnosis of overt disseminated intravascular coagulation (DIC). In 2001, the Scientific and Standardization Committee (SCC) of the International Society on Thrombosis and Hemostasis (ISTH) proposed new diagnostic criteria for the diagnosis of overt DIC. We wanted to compare the use of the ISTH versus KSTH criteria in the diagnosis of overt DIC. MATERIALS AND METHODS: We enrolled 131 patients over the age of 15 years, who had been admitted and diagnosed as having DIC from May 2000 to April 2005 at the Youngdong Severance Hospital, Seoul, Korea. Of the 131 patients, there were 71 males and 60 females, with a median age of 61 years. Hemostatic tests, including platelet counts, PT, aPTT, fibrinogen level and D-dimer, were evaluated based on the respective scoring systems. To assess the concordance between the two diagnostic systems, we used the Student's t-test and the K-coefficient. RESULTS: There were 79 patients compatible with the ISTH criteria and 63 patients with the KSTH criteria. Sixty-one patients were compatible with both diagnostic systems. The grade of agreement, or concordance rate, was 84.7% and the K-coefficient, or interrater reliability, was as low as 0.6 without significance. However, if we scored 1 point for a fibrinogen level of 100-150 mg/dL, and 2 points for a level below 100 mg/dL, for the ISTH criteria, then 63 patients were compatible with both diagnostic systems, and the concordance rate increased to 85.5% and the K-coefficient to 0.71 with significance. CONCLUSION: To achieve good agreement between the ISTH and KSTH diagnostic systems for overt DIC, we highly recommend changing the plasma fibrinogen cut-off value in the ISTH criteria from 100mg/dL to 150 mg/dL and scoring up to 2 points for a level below 100 mg/dL.ope

    A Modeling and Design Methodology of Double Exponential Pulse Generator for Simulation-Based Conducted Disturbance Immunity Testing

    In this paper, an equivalent circuit model of a double-exponential pulse generator is proposed for use as a time-domain noise source in high-altitude electromagnetic pulse (HEMP) conducted disturbance immunity testing. The analytic relationship between the proposed equivalent circuit model and the source pulse requirements expressed by the test standards is derived. Based on this relationship, a design methodology for the equivalent circuit model is proposed to extract the circuit components that satisfy the source pulse requirements, particularly in the form of source impedance and pulse waveform requirements. The proposed design methodology is applied to design an equivalent circuit model of the double exponential pulse generator with various test modes in the conducted disturbance immunity test. The designed double exponential pulse generator is applied to a simulation-based conductive disturbance immunity testing platform based on the International Electrotechnical Commission (IEC) 61000-4-24 standard to validate the effectiveness of the proposed equivalent circuit model and design methodology

    THERMAL HYDRAULIC ISSUES OF CONTAINMENT FILTERED VENTING SYSTEM FOR A LONG OPERATING TIME

    This study investigated the thermal hydraulic issues in the Containment Filtered Venting System (CFVS) for a long operating time using the MELCOR computer code. The modeling of the CFVS, including the models for pool scrubbing and the filter, was added to the input file for the OPR-1000, and a Station Blackout (SBO) was chosen as an accident scenario. Although depressurization in the containment building as a primary objective of the CFVS was successful, the decontamination feature by scrubbing and filtering in the CFVS for a long operating time could fail by the continuous evaporation of the scrubbing solution. After the operation of the CFVS, the atmosphere temperature in the CFVS became slightly above the water saturation temperature owing to the release of an amount of steam with high temperature from the containment building to the scrubbing solution. Reduced pipe diameters at the inlet and outlet of the CFVS vessel mitigated the evaporation of scrubbing water by controlling the amount of high-temperature steam and the water saturation temperature

    DS-ARP: A New Detection Scheme for ARP Spoofing Attacks Based on Routing Trace for Ubiquitous Environments

    Despite the convenience, ubiquitous computing suffers from many threats and security risks. Security considerations in the ubiquitous network are required to create enriched and more secure ubiquitous environments. The address resolution protocol (ARP) is a protocol used to identify the IP address and the physical address of the associated network card. ARP is designed to work without problems in general environments. However, since it does not include security measures against malicious attacks, in its design, an attacker can impersonate another host using ARP spoofing or access important information. In this paper, we propose a new detection scheme for ARP spoofing attacks using a routing trace, which can be used to protect the internal network. Tracing routing can find the change of network movement path. The proposed scheme provides high constancy and compatibility because it does not alter the ARP protocol. In addition, it is simple and stable, as it does not use a complex algorithm or impose extra load on the computer system

    Phosphorylation of Histone H2A.X in Peripheral Blood Mononuclear Cells May Be a Useful Marker for Monitoring Cardiometabolic Risk in Nondiabetic Individuals

    Phosphorylation of H2A.X (serine 139) in the histone H2A family located in the downstream of the DNA damage kinase signaling cascade is an important indicator of DNA damage. Recently, phosphorylation of H2A.X was proposed as a sensitive biomarker of aging. This study investigated if phosphorylation of H2A.X in peripheral blood mononuclear cells (PBMCs) is associated with cardiometabolic risk in nondiabetic individuals. Basic parameters and oxidative stress/inflammatory markers were measured in nondiabetic healthy Koreans (n = 119). Phosphorylation of H2A.X was measured randomly among the study subjects using a flow cytometer. According to the number of metabolic syndrome risk factor (MetS-RF), the study subjects were subdivided into "super healthy" (MetS โˆ’ RF = 0, n = 71) and "MetS-risk" (MetS โˆ’ RF โ‰ฅ 1, n = 48) groups. Phosphorylation of H2A.X in PBMCs (percentages and mean fluorescence intensity) was significantly higher in the MetS-risk group than in the super healthy group after adjusting for age, sex, cigarette smoking, and alcohol consumption. Phosphorylated H2A.X was positively correlated with the number of MetS-RF as well as waist circumference, blood pressures, triglyceride, Hb A1C , oxidized LDL, high sensitivity C-reactive protein, tumor necrosis factor-alpha, and alanine aminotransferase after the adjustment. The present study suggested that phosphorylated H2A.X in circulating PBMCs measured by flow cytometer may be a useful marker for monitoring cardiometabolic risk in nondiabetic individuals

    Wideband Channel Measurements and Modeling for In-House Power Line Communication

    This paper reports the results of extensive wideband channel measurements conducted on in-house outlets at 10~30MHz. Two kinds of measurements were performed: impulse response measurements and noise measurements. Impulse responses were obtained in two different time scales: 50 impulse responses taken for 1 minute to see short term variation and for 6 milliseconds to see very short term variation. Noise measurements were carried out to measure background noise, appliance noise and periodic noise. These measurements at a specific outlet are repeated in every other hour over 24 hours. The measured data were carefully reduced and analyzed to extract relevant information to obtain time-varying channel and noise characteristic for in-house power line communication
