Leishmania (Viannia) naiffi : rare enough to be neglected?
In the Brazilian Amazon, American tegumentary leishmaniasis (ATL) is endemic and presents a wide spectrum of clinical manifestations due, in part, to the circulation of at least seven Leishmaniaspecies. Few reports of Leishmania (Viannia) naiffiinfection suggest that its occurrence is uncommon and the reported cases present a benign clinical course and a good response to treatment. This study aimed to strengthen the clinical and epidemiological importance of L. (V.) naiffiin the Amazon Region (Manaus, state of Amazonas) and to report therapeutic failure in patients infected with this species. Thirty Leishmania spp samples isolated from cutaneous lesions were characterised by multilocus enzyme electrophoresis. As expected, the most common species was Leishmania (V.) guyanensis (20 cases). However, a relevant number ofL. (V.) naiffi patients (8 cases) was observed, thus demonstrating that this species is not uncommon in the region. No patient infected withL. (V.) naiffievolved to spontaneous cure until the start of treatment, which indicated that this species may not have a self-limiting nature. In addition, two of the patients experienced a poor response to antimonial or pentamidine therapy. Thus, either ATL cases due to L. (V.) naifficannot be as uncommon as previously thought or this species is currently expanding in this region

Entretanto, até o momento ainda não existe uma vacina, dificultando o controle da doença. Ademais, ainda não se conhece quais são as condições que contribuem para que indivíduos infectados evoluam sem desenvolver sinais clínicos da doença, apresentem insucesso terapêutico, ou que venham a desenvolver a forma mucosa da leishmaniose. O propósito desta tese foi avaliar a evolução pós-tratamento de casos de infecção por Leishmania naiffi (Ln) e L. braziliensis (Lb) sob dois aspectos diferentes: 1) descrever uma série de casos de LTA causados por Ln selecionados dentre trinta pacientes atendidos na Fundação de Medicina Tropical Dr. Heitor Vieira Dourado (FMT-HVD), Manaus, no período de 2011 a 2013. 2) avaliar se a assinatura imunológica, em termos de subclasses de imunoglobulina e perfil de diferenciação celular e memória, é similar entre pacientes de Lb com até 26 anos de cura após tratamento com dois diferentes esquemas terapêuticos com antimônio. Foram estudados 43 pacientes curados de leishmaniose cutânea (LCC), sendo estes divididos em dois grupos: pacientes de LCC acompanhados desde a fase ativa até três anos (a) após cura da infecção por Lb (n= 23, LCC<3a) e pacientes LCC com até 26 anos de cura da infecção por Lb (n= 12, LCC 12-26a). Além disso, 30 isolados de Leishmania de pacientes de leishmaniose cutânea (LC) proveniente da FMT-HVD foram avaliados
Células mononucleares do sangue periférico (CMSP) foram analisadas após estímulo in vitro com antígenos de Lb. A fenotipagem das subpopulações de linfócitos T foi realizada através da citometria de fluxo quanto à ativação, imunosenescência e memória imunológica. A produção de IgG e suas subclasses anti-Leishmania foram avaliadas por ELISA. Os isolados de Leishmania foram caracterizados quanto à espécie pela técnica de isoenzimas. A presença do Leishmania RNA vírus (LRV) na Ln foi avaliada por sequenciamento. Após a cura clínica foi observada uma redução dos níveis de IgG1 e IgG3 anti-Lb (p<0.0001). O tempo de cura clínica neste estudo correlacionou-se negativamente com os valores de Índice de ELISA (IE) obtidos a partir de amostras de LCC<3a, para IgG (r= -0.79; p<0.0001), IgG1 (r=-0.64, p<0.001) e IgG3 (r= -0.4; p<0.05). As CMSP dos compartimentos de memória dos LCC 12-26a apresentaram aumento da ativação das células TME e TMC (p<0.05) frente aos antígenos de Lb, apesar de não ter sido observado expansão destes compartimentos. Foi observada uma frequência de 26,7% da Ln entre os isolados de LC. Ademais, dois pacientes de Ln evoluíram com falha terapêutica após o tratamento com antimonial ou pentamidina. Em um caso clínico foi descrita uma possível associação entre o LRV presente na Ln e o insucesso terapêutico por pentamidina
Os resultados acima indicam que: (1) após longo tempo de cura ocorre uma modulação da reposta imune de memória capaz de manter suas células ativadas, mas não expandir seus compartimentos frente a uma nova estimulação antigênica específica. Além disso, a IgG1 e IgG3 surge como uma importante ferramenta a ser utilizada no acompanhamento clínico dos pacientes. (2) Os casos de LTA por Ln podem não ser tão raros e nem ter um caráter benigno como é descrito na literatura. patients progressed with treatment failure after treatment with antimony or pentamidine.
In a clinical case was described a possible association between the LRV present in
Ln
and
therapeutic failure by pentamidine. The above results indicate that: (1)
after long time healing
there is a modulation of the immune response of memory able to maintain its activated cells but
not expand its compartments front of new specific antigen stimulation. Moreover, IgG1 and
IgG3 appear as an important tool to be used in
clinical patient monitoring. (2) cases of LTA by
Ln
may not be as rare and even have a benign character as described in the literature. Later
studies in the Amazon region may provide more information about ATL caused by
L. naiff

clínica da leishmaniose tegumentar americana. No entanto, pouco se sabe sobre a resposta
imune (RI) duradoura responsável pelo controle da infecção por Leishmania em
indivíduos curados de leishmaniose cutânea (LC). É provável que o perfil da RI efetora
e/ou reguladora desencadeada nesses indivíduos contribua para o desenvolvimento de
estratégias vacinais promissoras. O T-bet, GATA-3 e Foxp3 são fatores transcricionais
(FT), envolvidos na diferenciação de células T, apontados como moduladores da
expressão de genes que codificam citocinas efetoras e reguladoras. Baseado nisso, nosso
objetivo foi avaliar se os níveis de expressão gênica de citocinas do tipo 1 (IFNG, IL12A,
IL18), tipo 2 (IL4, IL5), reguladoras (IL10) e, respectivamente, dos FT TBET, GATA3 e
FOXP3, em resposta aos antígenos de Leishmania estavam associados ao tempo de cura
clínica da LC. Foram estudados: pacientes de LC curados há menos de dois anos
(LCC2a; n=10), ou que evoluíram para a
cura espontânea (CE; n=05). O grupo controle foi constituído de indivíduos saudáveis,
sem histórico de leishmaniose (IS; n=08). O RNAm foi extraído das células
mononucleares de sangue periférico (CMSP) estimuladas in vitro com antígenos de
L. braziliensis. Para a amplificação gênica, foram utilizadas placas de 96 poços contendo
simultaneamente todos os iniciadores dos genes alvo, além dos genes constitutivos. O
número de cópias de genes foi quantificado pela da técnica de PCR em tempo real. Os
resultados demonstraram que não houve alteração nos níveis de expressão de TBET,
GATA3, FOXP3 nos grupos de LCC estudados, mas os CE apresentaram níveis
significativamente mais elevados para todos os FT analisados. Os LCC>2a tiveram
redução na expressão de IL12A em comparação aos LCC<2a (p<0,01), mas não houve
alteração em relação ao IFNG. Embora o número de cópias de IL18, IL4 e IL10 fosse
mais baixo nos LCC>2a em relação aos LCC<2a, esta diferença não foi significativa.
Além dos FT, os CE apresentaram expressão estatisticamente mais elevada de IFNG,
IL12A, IL4, IL5, e IL10. Os resultados acima sugerem que os pacientes que curaram
espontaneamente apresentaram uma maior capacidade de expressar citocinas do tipo 1,
tipo 2 e reguladoras comparadas aos curados que receberam o tratamento. A redução da
expressão de genes de citocinas efetoras e inibitórias com o aumento do tempo de cura
sugere que este padrão esteja associado com uma resposta protetora bem modulada.The profile of Type 1 and Type 2 cytokines in addition to the expression of transcriptional
regulators are determinant for the clinical cure of American tegumentary leishmaniasis
(ATL). Despite of all investigation in the field, little is known about the progression of the
immune response (RI) after the clinical cure (CL), when the parasite burden and excess
inflammation is kept in check by a well regulated immune response. It is probable that the
RI profile involving the expression of effector and regulatory molecules contributes to
ensure the successful strategy for the immune response against the parasite. T-bet,
GATA-3 and Foxp3 are transcriptional factors (TF), involved in differentiation of T cells,
these molecules are modulators of gene expression that control effector and regulatory
cytokines. Based on that, our aim was to associate the levels of cytokines gene expression
of type 1 (IFNG, IL12A, IL18), type 2 ( IL4, IL5) and regulatory molecules (IL10, TBET,
GATA3 and FOXP3) in response to Leishmania antigen, to clinical time of cure in CL.
Were investigated patients of CL with less than two years of cure (CCL<2y; n=11), more
than two years of cure (CCL>2y; n=10), or that evolved to spontaneous healing without
intervention (SH; n=05). The control group was composed by healthy individuals, with no
history of leishmaniasis (HI; n=08). Total RNA was extracted from peripheral blood
mononuclear cells (PBMC) stimulated in vitro with L. braziliensis antigens. The extracted
RNA was used to quantify specific gene expression by Real Time PCR using customized
assay kits. The experiments showed that there is no change in transcription for TBET,
GATA3 or FOXP3 at studied CCL groups, however SH patients have significantly higher
gene copy number for all analyzed TF. LCC>2a patients showed reduced expression of
IL12A in comparison with LCC<2a (p<0,01), but yet the expression of IFNG was
identical in both groups. There is a trend for lower gene copy numbers of IL18, IL4 e IL10
in LCC>2a in comparison with LCC<2a patients, but this difference was not statistically
relevant. Besides increased expression of transcriptional regulators, CE patients showed
significant increase in gene copy numbers for IFNG, IL12A, IL4, IL5, e IL10. Taken
together, our results suggest that patients with spontaneous cure of leishmaniases present
a stronger Type 1, Type 2 and regulatory responses when compared to patients that
developed progressive disease and received treatment. With increased time of cure, we
observed a reduction on gene expression of effector and inhibitory cytokines. This
phenomena may represent a pattern of well modulated protective immune response
against the parasite

Generally, patients with L. naiffi cutaneous leishmaniasis exhibit a good therapeutic response to either pentavalent antimonials or pentamidine. However, after pentamidine treatment, this patient's infection evolved to therapeutic failure. To understand this clinical outcome, we investigated the presence of the Leishmania RNA virus (LRV) in parasites isolated from the cutaneous lesion; herein, we discuss the possible association between a poor response to pentamidine therapy and the presence of the LRV
