    Characterization of antimicrobial resistance pattern and molecular analysis among extended spectrum β-lactamase-producing Escherichia coli

    Background: Infection is a serious problem in medicine and appropriate antibiotic therapy is very important. Because of broad spectrum activity and low toxicity of β-lactam antibiotics, they are the most commonly used drugs. But, bacterial resistance to β-lactam antibiotics, has been considered as the global healthcare concern. The aim of study was to evaluate the antimicrobial resistance pattern and molecular characterization among ESBL-producing Escherichia coli isolated from patients with diarrhea admitted to a hospital in Ilam, Iran. Methods: Totally, fifty E. coli isolates were investigated. Confirmatory tests for phenotypic detection of ESBLs were performed. Molecular identification of the blaTEM and blaSHV genes was carried out by PCR method. To identify genetic relatedness among isolates, Randomly Amplified Polymorphic DNA (RAPD) analysis was performed. Results: The antibiotic susceptibility results showed that the most effective antibiotic was imipenem and minimum effect was related to gentamicin. Thirty-one isolates (62) were ESBL-producing organisms according to phenotypic method. The distribution of blaTEM and blaSHV genes among ESBL-producing isolates were 20 (64.5) and 6 (19.3), respectively. RAPD-PCR typing among isolates gave us eight different types. Twelve isolates were clustered in genotype A and all of them were ESBL-producer. Conclusion: The present study showed noticeable incidence of ESBL-producing E. coli isolated from outpatients and hospitalized patients with diarrhea. Therefore, it seems that constant supervision is crucial to monitor the ESBL-producing microorganisms in hospitals and community. © 2016 The Authors

    Estimation of the parasitic infection prevalence in children with Helicobacter pylori infection in Ilam city (2012-2013)

    Background: Helicobacter pylori is a common cause of chronic infection in human beings. The infection has universal prevalence and contracts all age groups. Probably, these bacteria are the cause of the most common chronic bacterial infection in man and have infected approximately half of the world population. The urease of these bacteria degrades the urea in stomach’s mucosa to ammoniac which results pH increment of the stomach lumen. This may allow the pathogenic intestinal protozoa to take the opportunity to cross through stomach’s decreased pH situation and cause the disease. Objectives: The current study aimed to evaluate the prevalence of parasitic infections (such as giardiasis) in children with Helicobacter pylori infection in Ilam city. Patients and Methods: Following the sample collection during 12 months from children in Ilam (Ilam, Iran), Helicobacter pylori infection was determined based on stool antigen analysis (HPSA) by enzyme-linked immunosorbent assay (ELISA) method in children who had recurrent abdominal pain. Stool specimens were examined by the direct examination and spontaneous sedimentation method to detect both trophozoite and cyst of parasites. Results: In this study 37 children with H. pylori infection were evaluated, and the patients with positive results for Giardia lamblia, and Entamoebahistolytica/dispar were found 29.7, and 10.8 respectively. Conclusions: The results of the current study suggest that H. pylori infection may provide favorable conditions for Giardiasis infection, but this presumption needs to be investigated further with more samples. © 2014, Pediatric Infections Research Center

    In vivo Antibacterial and Wound Healing Activities of Roman Chamomile (Chamaemelum nobile)

    BACKGROUND: Today considerable number of drugs are produced from plants. Several plants with antibacterial and healing applications are used in medicine such as Roman chamomile (Chamaemelum nobile L.). Wound infection is one of the most prevalent infections among infectious diseases around the world. Due to appearance of drug resistance, researchers are now paying attention to medicinal plants. Therefore, this study was designed to investigate the antimicrobial and wound healing properties of C. nobile against Pseudomonas aeruginosa using in vivo conditions. METHODS: Ethanolic extract of C. nobile was provided using standard method. The 5 C. nobile ointment was prepared by dissolving lyophilized extract in eucerin. Forty five male rats were obtained from Ilam university. After anesthetization and wound creation, wounds were infected by P. aeruginosa. The rats were divided into three groups, group I was treated with C. nobile ointment, group II was treated with tetracycline ointment and the third group was treated with base gel as control group. RESULTS: Antibacterial and wound healing activities of C. nobile ointment were more than tetracycline ointment significantly. Our results indicated that extract of C. nobile had effective antibacterial activity and accelerated the progression of wound healing. CONCLUSION: Our study indicated that antibacterial and wound healing activities of C. nobile ointment were notable. C. nobile therapy in combination with antibiotics can also be useful because medicinal plants contents operate in synergy with antibiotics. These results revealed the value of plant extracts to control antibiotic resistant bacteria in wound infections
