    The Production of Health and the Valuation of Medical Inputs in Wage-Amenity Models

    Using a hedonic wage-amenity model, this paper examines the valuation of medical inputs into the production of health. The data used in this study include the incomes, demographics and measures of human capital for households in eastern North Carolina with county level medical input supply. These data allow an estimate of the marginal value of medical care inputs such as the physician to population ratio and the availability of specialized services in an area of the country where the lack of available medical care has been of particular concern to policy makers. Our results indicate that while health care inputs are not a significant determinant of earnings overall, they are important in counties that have been designated as medically underserved. In underserved counties each additional physician per 10,000 individuals in the county decreases earnings by about 11.6%. This suggests that physicians act as an amenity and workers are willing to accept lower wages to locate in counties with a higher physician to population ratio

    Remote State Preparation

    Quantum teleportation uses prior entanglement and forward classical communication to transmit one instance of an unknown quantum state. Remote state preparation (RSP) has the same goal, but the sender knows classically what state is to be transmitted. We show that the asymptotic classical communication cost of RSP is one bit per qubit - half that of teleportation - and becomes even less when transmitting part of a known entangled state.     The quantum capacity is properly defined without encodings

    We show that no source encoding is needed in the definition of the capacity of a quantum channel for carrying quantum information. This allows us to use the coherent information maximized over all sources and and block sizes, but not encodings, to bound the quantum capacity. We perform an explicit calculation of this maximum coherent information for the quantum erasure channel and apply the bound in order find the erasure channel's capacity without relying on an unproven assumption as in an earlier paper.Comment: 19 pages revtex with two eps figures.     Purification of Noisy Entanglement and Faithful Teleportation via Noisy Channels

    Two separated observers, by applying local operations to a supply of not-too-impure entangled states ({\em e.g.} singlets shared through a noisy channel), can prepare a smaller number of entangled pairs of arbitrarily high purity ({\em e.g.} near-perfect singlets). These can then be used to faithfully teleport unknown quantum states from one observer to the other, thereby achieving faithful transfrom one observer to the other, thereby achieving faithful transmission of quantum information through a noisy channel. We give upper and lower bounds on the yield D(M)D(M) of pure singlets (Ψ\ket{\Psi^-}) distillable from mixed states MM, showing D(M)>0D(M)>0 if \bra{\Psi^-}M\ket{\Psi^-}>\half.Comment: 4 pages (revtex) plus 1 figure (postscript).     Selective Expansion of Cross-Reactive Cd8+ Memory T Cells by Viral Variants

    The role of memory T cells during the immune response against random antigenic variants has not been resolved. Here, we show by simultaneous staining with two tetrameric major histocompatibility complex (MHC)–peptide molecules, that the polyclonal CD8+ T cell response against a series of natural variants of the influenza A nucleoprotein epitope is completely dominated by infrequent cross-reactive T cells that expand from an original memory population. Based on both biochemical and functional criteria, these cross-reactive cytotoxic T cells productively recognize both the parental and the mutant epitope in vitro and in vivo. These results provide direct evidence that the repertoire of antigen-specific T cells used during an infection critically depends on prior antigen encounters, and indicate that polyclonal memory T cell populations can provide protection against a range of antigenic variants

    A systematic review of parent and family functioning in pediatric solid organ transplant populations

    The process of pediatric solid organ transplantation (SOT) places new and increased stressors on patients and family members. Measures of family functioning may predict psychological and health outcomes for pediatric patients and their families, and provide opportunity for targeted intervention. This systematic review investigated parent and family functioning and factors associated with poorer functioning in the pediatric SOT population. Thirty‐seven studies were identified and reviewed. Studies featured a range of organ populations (eg, heart, liver, kidney, lung, intestine) at various stages in the transplant process. Findings highlighted that parents of pediatric SOT populations commonly report increased stress and mental health symptoms, including posttraumatic stress disorder. Pediatric SOT is also associated with increased family stress and burden throughout the transplant process. Measures of parent and family functioning were associated with several important health‐related factors, such as medication adherence, readiness for discharge, and number of hospitalizations.     Quantum Channel Capacity of Very Noisy Channels

    We present a family of additive quantum error-correcting codes whose capacities exceeds that of quantum random coding (hashing) for very noisy channels. These codes provide non-zero capacity in a depolarizing channel for fidelity parameters ff when f>.80944f> .80944. Random coding has non-zero capacity only for f>.81071f>.81071; by analogy to the classical Shannon coding limit, this value had previously been conjectured to be a lower bound. We use the method introduced by Shor and Smolin of concatenating a non-random (cat) code within a random code to obtain good codes. The cat code with block size five is shown to be optimal for single concatenation. The best known multiple-concatenated code we found has a block size of 25. We derive a general relation between the capacity attainable by these concatenation schemes and the coherent information of the inner code states.Comment: 31 pages including epsf postscript figures.     Quantum Nonlocality without Entanglement

    We exhibit an orthogonal set of product states of two three-state particles that nevertheless cannot be reliably distinguished by a pair of separated observers ignorant of which of the states has been presented to them, even if the observers are allowed any sequence of local operations and classical communication between the separate observers. It is proved that there is a finite gap between the mutual information obtainable by a joint measurement on these states and a measurement in which only local actions are permitted. This result implies the existence of separable superoperators that cannot be implemented locally. A set of states are found involving three two-state particles which also appear to be nonmeasurable locally.     Links between dissipation, intermittency, and helicity in the GOY model revisited

    High-resolution simulations within the GOY shell model are used to study various scaling relations for turbulence. A power-law relation between the second-order intermittency correction and the crossover from the inertial to the dissipation range is confirmed. Evidence is found for the intermediate viscous dissipation range proposed by Frisch and Vergassola. It is emphasized that insufficient dissipation-range resolution systematically drives the energy spectrum towards statistical-mechanical equipartition. In fully resolved simulations the inertial-range scaling exponents depend on both model parameters; in particular, there is no evidence that the conservation of a helicity-like quantity leads to universal exponents.Comment: 24 pages, 13 figures; submitted to Physica
