Auswertung des endoskopischen Langzeitmanagements mittels Argon-Plasma-Koagulation bei Patienten mit gastrointestinalen Angiodysplasien
In dieser retrospektiven Studie wurde ĂĽber einen Zeitraum von 2008 bis 2017 ein
Patientenkollektiv von 48 PatientenInnen mit insgesamt 108 Untersuchungen
aufgenommen. Aufnahmekriterium waren mit APC therapierte
behandlungsbedĂĽrftige Angiodysplasien, sowohl isoliert (49 %), als auch im
Rahmen anderer Vorerkrankungen (GAVE-Syndrom (42,3 %), PHG (5,9 %),
Heyde-Syndrom 2,9 %). Die Stichprobe bestand aus 32 männlichen Patienten
und 16 weiblichen Patientinnen. Es wurden sowohl der Stellenwert der APC-Therapie im Rahmen gastrointestinaler Angiodysplasien untersucht, als auch
Risikofaktoren und gemeinsame Merkmale des betroffenen Patientenkollektivs
herausgearbeitet und mittels SPSS statistisch ausgewertet.
In Zusammenschau der erhobenen Daten zeigte sich das männliche Geschlecht
in dieser Studie als nachteilig fĂĽr den Krankheitsverlauf. Darauf verweist das
Durchschnittsalter, das beim männlichen Geschlecht - zwar nicht signifikant, aber
dennoch auffällig - geringer war als das Behandlungsalter der Frauen (69,52 bei
Frauen vs. 65,68 Jahre bei Männern). Auch in der Darstellung der
Blutungssituation waren die Männer benachteiligt, da sie von der prognostisch
ungünstigeren akuten Blutungssituation (Forrest I) häufiger betroffen waren als
die Frauen (Männer 67 % vs. Frauen 33 %). Die günstigere Variante (Forrest III)
lag hingegen beim weiblichen Geschlecht häufiger vor (Frauen 68,8 % vs.
Männer 53,8 %). Männliche Patienten wiesen zudem eine geringere
Therapieansprechrate auf. Diese wurde anhand des Hämoglobinwertes
untersucht. Beim männlichen Geschlecht zeigte sich nicht nur ein geringerer
Ausgangs-Hb-Wert (bei Aufnahme 7,95 g/dl bei den Männern vs. 8,46 g/dl bei
den Frauen), sondern im Vergleich zu den Frauen auch im Verlauf ein geringerer
Hb-Wert-Anstieg (bei Entlassung 9,16 g/dl bei den Männern vs. 9,85 g/dl bei den
Frauen). Zudem erhielten Männer mit im Durchschnitt drei
Erythrozytenkonzentraten mehr Transfusionen als die Frauen mit 2,59
Erythrozytenkonzentraten. Insgesamt kann mit einem durchschnittlichen Hb-Wert-Anstieg von 1,3 Hb-Punkten im Verlauf des Aufenthaltes in der Universitätsklinik Tübingen jedoch ein Erfolg der durchgeführten APC-Therapien
untermauert werden.
Weiterhin konnte der wichtige Zusammenhang zwischen dem Vorliegen
kardiovaskulärer Risikofaktoren und Angiodysplasien dargestellt werden. So
konnten in 33,7 % ein chronisch fortgesetzter Nikotinkonsum und in 70 % der
Fälle, als häufigste Komorbidität, eine arterielle Hypertonie identifiziert werden.
Hierzu passen auch die vorgestellten Studienergebnisse, die einen möglichen
positiven Zusammenhang therapiebedĂĽrftiger Angiodysplasien mit der Einnahme
von ASS implizieren: In unserer Stichprobe lag zwischen den APC-Wiederholungssitzungen mit Einnahme von ASS ein Zeitraum von 237,9 Tagen vs. 117,4 Tagen ohne Einnahme von ASS.
Insgesamt lieĂź sich so erstaunlicherweise ein eher protektiver Einfluss
antithrombogener Medikamente beobachten. Die durchschnittliche Dauer
zwischen APC-Wiederholungssitzungen war mit Einnahme der Medikamente mit
Einfluss auf die Hämostase mit im Durchschnitt 166,54 Tagen länger als ohne
Einnahme antithrombogener Medikamente (125,69 Tage).
Als limitierend stellt sich in unserer Studie das Fehlen einer Kontrollgruppe und
eine relativ kleine Stichprobe dar. Dennoch konnten in dieser Arbeit einige
interessante Punkte herausgearbeitet werden, die zu weiterer Forschung
anregen sollten
Endogenous oxytocin response to film scenes of attachment and loss is pronounced in schizophrenia
Oxytocin (OXT) is critically involved in the regulation of attachment and interpersonal function. In this study, emotional childrens movies were used to stimulate OXT secretion in patients with schizophrenia and healthy controls (HCs). Furthermore, associations of OXT levels with measures of attachment style (Psychosis Attachment Measure), childhood adversity (Childhood Trauma Questionnaire) and symptom severity [Positive and Negative Syndrome Scale (PANSS)] were considered. In 35 patients with schizophrenia and 35 matched HCs, radioimmunoassay with sample extraction was used to determine OXT plasma levels before and after viewing of movie scenes portraying emotional bonding and loss and compared to a non-emotional condition. Statistical analysis indicated lower baseline OXT levels in female patients than in all other groups. OXT reactivity during emotional movies was significantly higher in patients when compared to HCs. OXT reactivity during the control movie related to PANSS `general psychopathology. No significant associations appeared between baseline or induced OXT levels and other PANSS subscales, attachment style or childhood adversity in patients. Our findings suggest differences of baseline OXT and a higher OXT reactivity toward strong emotional stimuli in patients with schizophrenia, suggesting a role of OXT as a gender- and context-dependent modulator of socio-emotional function
Peripheral oxytocin is inversely correlated with cognitive, but not emotional empathy in schizophrenia
Endogenous oxytocin has been associated with different aspects of social cognition in healthy subjects and patients with schizophrenia. In this pilot study, we investigated the relationship between plasma oxytocin and oxytocin level changes induced by empathy-eliciting, attachment-related movie scenes with correlates of cognitive and emotional empathy in patients and healthy controls. The Multifaceted Empathy Test (MET) and the Interpersonal Reactivity Index (IRI) were administered to patients with schizophrenia (N = 35, 12 females) and healthy controls (N = 35, 12 females) to estimate dimensions of cognitive and emotional empathy. Peripheral basal oxytocin concentrations and oxytocin responses to movie-based emotional stimuli were assessed using radioimmunoassay with sample extraction. In patients, induced oxytocin level changes were inversely correlated with MET cognitive empathy regarding negative emotional states. Controlling for non-social cognition and age revealed a significant negative association between basal oxytocin levels and MET cognitive empathy for positive emotions. In healthy subjects, oxytocin reactivity was inversely correlated with the IRI subscale "fantasy". Oxytocin was not related to any measure of emotional empathy. A hyper-reactive oxytocin system might be linked to impaired cognitive empathy as a part of a dysfunctional regulative circuit of attachment-related emotions and interpersonal stressors or threats by attribution of meaning. Healthy adults with a disposition to identify with fictional characters showed lower oxytocin reactivity, possibly indicating familiarity with movie-based stimuli. The oxytocinergic system may be involved in maladaptive coping mechanisms in the framework of impaired mentalizing and associated dysfunctional responses to interpersonal challenges in schizophrenia

Post-traumatic stress disorder and beyond: an overview of rodent stress models.
Post-traumatic stress disorder (PTSD) is a psychiatric disorder of high prevalence and major socioeconomic impact. Patients suffering from PTSD typically present intrusion and avoidance symptoms and alterations in arousal, mood and cognition that last for more than 1 month. Animal models are an indispensable tool to investigate underlying pathophysiological pathways and, in particular, the complex interplay of neuroendocrine, genetic and environmental factors that may be responsible for PTSD induction. Since the 1960s, numerous stress paradigms in rodents have been developed, based largely on Seligman's seminal formulation of 'learned helplessness' in canines. Rodent stress models make use of physiological or psychological stressors such as foot shock, underwater trauma, social defeat, early life stress or predator-based stress. Apart from the brief exposure to an acute stressor, chronic stress models combining a succession of different stressors for a period of several weeks have also been developed. Chronic stress models in rats and mice may elicit characteristic PTSD-like symptoms alongside, more broadly, depressive-like behaviours. In this review, the major existing rodent models of PTSD are reviewed in terms of validity, advantages and limitations; moreover, significant results and implications for future research-such as the role of FKBP5, a mediator of the glucocorticoid stress response and promising target for therapeutic interventions-are discussed
