Since the beginning of 1950’s the migration of population from countryside to cities constitutes a subject as an important part of demographic changing on its own. Since the second half of 1970’s, especially domestic migration from the small and medium size cities to big cities, by turning into family migration became dense in the three big metropolises in west. (Ãstanbul, Ankara, Ãzmir). In 1990’s both the migration from east and Southeast Anatolia to other regions and the obligatory migration inside the region have been added to elements which effect movement of population. While this changing, which occurs in the dynamics of domestic migration during the time, does not exist in most of present researches, one of the other absence is social sex viewpoint. In the studies about migration, the lack of social sex viewpoint is not peculiar only to Turkey but also it seems limited in international literature. Present studies show there are important differences between women and men in the point of migration causes, participation to migration process, experiences during this process and effects of migration, attitudes and reactions of immigrants. In the core of these differences there are, collaboration inside the family between woman and man and parallel to this, social roles of woman and man, which described by customs and traditions. Women’ life relating to migration is generally closely related to their situations in family as a wife, a mother or a young girl about to marry. The relations between them and both the places they left and they just came is formed in this base. Consequently, in a migration process, which includes a social and place changing, identity of sex has an important role as much as socio-economic class, culture, ethnic or national identity. G.A.P. Region which is in the Southeast of our developing country has a village-city appearance by the effects of both many side social, physiological, economic and urban destruction which created by terror and fight and generally the feudal structure in the region, extreme poverty, lack of top and underground structure and the political and economic choices which does not mind about neighborhood values. This work has aimed at examine to migration which is one of the most important social fact in the world we live in the context of woman and urbanization and to expose the appearance of women in extent of G.A.P. region in domestic immigration…

Model following control with discrete time SMC for time-delayed bilateral control systems

This paper proposes a new algorithm based on model following control to recover the uncompensated slave disturbance on time delayed motion control systems having contact with environment. In the previous works, a modified Communication Disturbance Observer (CDOB) was shown to be successful in ensuring position tracking in free motion under varying time delay. However, experiments show that due to the imperfections in slave plant Disturbance Observer (DOB) when there is rapid change of external force on the slave side, as in the case of environment contact, position tracking is degraded. This paper first analyzes the effect of environment contact for motion control systems with disturbance observers. Following this analysis, a model following controller scheme is proposed to restore the ideal motion on the slave system. A virtual plant is introduced which accepts the current from the master side and determines what the position output would be if there was no environment. Based on the error bet ween actual system and model system, a discrete time sliding mode controller is designed which enforces the real slave system to track the virtual slave output. In other words, convergence of slave position to the master position is achieved even though there is contact with environment. Experimental verification of the proposed control scheme also shows the improvement in slave position tracking under contact forces

The systemic vasculitis of the small-medium arteries, arterioles, venules and rarely large arteries that involves respiratory system and kidneys was defined as Granulomatosis with Polyangiitis (GPA) disease by Wegener in 1936. Intracranial aneurysms and subarachnoid haemorrhage (SAH) are extremely rare complications of GPA and our case will be the 2nd case treated with clipping aneurysm and the 11th case with subarachnoid haemorrhage in the literature. A 43-year-old man presented to the emergency room with a severe headache and was admitted for further evaluation. He had GPA diagnosis 14 years ago with cytoplasmic anti-neutrophil cytoplasmic antibody (C-ANCA) and PR3-ANCA positive laboratory tests and kidney biopsy. SAH was seen on cranial computed tomography (CT) images. Then cerebral digital substraction angiography (DSA) performed and right middle cerebral artery aneurysm exposed. Aneurysm was clipped without any complication. Intracranial aneurysms and SAH are extremely rare complications of GPA. GPA related aneurysmal SAH is an exceptional condition in neurovascular pathology. Monitoring patients with GPA for SAH must be remembered and kept in mind as a diagnosis

Supratentorial hemangioblastoma without von Hippel-Lindau syndrome in an adult: A rare case report

    Get PDF
Hemangioblastomas (HBLs) are highly vascular and cystic benign neoplasms. They form very small part of intracranial tumours and are often localized in the posterior fossa. Although most of them are sporadic, a significant group is accompanied with von Hippel-Lindau (VHL) syndrome. This case report presents a 57-year-old woman treated with total resection using micro-surgical technique and was diagnosed as HBL based on histopathologic findings. Contrast-enhanced cranial MRI of the patient with the complaints of spasms in the right side of body showed a right paracentral mass that caused midline shift. In literature, the previously reported cases of supratentorial HBL unaccompanied with VHL syndrome were searched in PUBMED, compiled and presented. It should be borne in mind that rare HBLs manifesting with various neurological symptoms may occur in the supratentorial region, and may not accompany with VHL syndrome

    Get PDF
This short report presents a case that developed rapid recurrence of a malignant meningioma. The meningioma was located on the right temporal lobe and total removal (Simpson grade-II) was performed. Radiotherapy was not given and the lesion recurred within four months. The MIB-1 (Ki-67) index was 30 % and the tumor fulfilled all the criteria of anaplasia. After the second surgery, patient was transferred to the Radiation Oncology for radiotherapy. Should we questioned the extent of surgery? Neurosurgeons should be careful and close follow-up the patients with malignant meningioma

Introduction: The role of inflammation in cancer has been defined, and now, inflammation is accepted as one of the hallmarks of cancer development. The aim of this study was to evaluate the difference regarding preoperative neutrophil to lymphocyte (NLR) and platelet to lymphocyte ratios (PLR) in patients with meningioma between patients and healthy controls and between grade-I and grade-II meningiomas. Methods: Retrospective analysis of preoperative neutrophil, lymphocyte, monocyte, and platelet counts and NLR, and PLR were evaluated in 61 patients underwent meningioma surgery. Results: Neutrophil count was significantly increased while lymphocyte count significantly decreased patients compared to controls. Similar findings were obtained in grade-II meningiomas compared with grade-I meningiomas. NLR were significantly higher in both grade-I and grade-II meningiomas than controls. Conclusion: We for the first time provided that higher NLR may be associated with grading of meningioma and be a predictive factor for progression of meningiomas. The use of medication against neutrophil-related inflammation may be helpful for patients with higher grade of meningioma decreasing peritumoral edema before and after surgery

Multiple myeloma and other plasma cell disorders are characterized by production of a large number of plasma cells in the bone marrow. On the other hand, plasmacytoma results from proliferation of abnormal plasma cells in the soft tissue or skeletal system. Neurological complications are frequently observed in these diseases. The most commonly known complications among those complications are spine fractures, spinal cord compressions, and peripheral neuropathies. Although neurological involvements are common in plasmacytomas, extramedullary spinal epidural localizations have been reported very rarely. In this case report, we aimed to present a plasmacytoma case that presented with acute onset of upper extremity monoparesis. A 40-year-old woman was admitted to our clinic with complaints of sudden weakness and numbness in her left arm following neck and left arm pain. Emergency cervical magnetic resonance imaging (MRI) revealed an epidural mass and the patient underwent emergency surgery. The patient showed improvement post-operatively and the pathology was reported as plasmacytoma. Following hematology consultation, systemic chemotherapy was initiated and radiotherapy was planned after wound healing

Background: In this study, the patients who were operated in two clinics due to traumatic cranial epidural hematoma (EDH) were assessed retrospectively and the factors that increase the costs were tried to be revealed through conducting cost analyses. Methods: The patients who were operated between 2010 and 2016 with the diagnosis of EDH were assessed in terms of age, sex, trauma etiology, Glasgow coma scale (GCS) at admission, the period from trauma to hospital arrival, trauma-related injury in other organs, the localization of hematoma, the size of hematoma, length of stay in the intensive care unit (ICU), length of antibiotherapy administration, number of consultations conducted, total cost of in-hospital treatments of the patients and prognosis. Results: Distribution of GCS were, between 13-15 in 18 (36%) patients, 9-13 in 23 (46%) patients and 3-8 in 9 (18%) patients. The reasons for emergency department admissions were fall from high in 29 (58%) patients, assault in 11 (22%) patients and motor vehicle accident in 10 (20%) patients. The average cost per ICU stay was 2838 (range=34320571 (range=343-20571 ). The average cost per surgical treatment was 314 $. ICU care was approximately 9 times more expensive than surgical treatment costs. The mortality rate of the study cohort was 14% (7 patients). Conclusion: The prolonged period of stay in the ICU, antibiotherapy and repeat head CTs increase the costs for patients who are surgically treated for EDH
