Antimicrobial Resistance of Bacterial Agents of the Upper Respiratory Tract of School Children in Buea, Cameroon
The study was aimed at determining bacterial agents of the upper
respiratory tract and the susceptibility patterns of isolates to
antibiotics. In total, 200 throat swabs were obtained from students
attending different boarding schools within the Buea Municipality and
screened to obtain the prevalence of respiratory pathogens and to
understand the antibiotic susceptibility patterns of isolates using
standard microbiological procedure and the disc-diffusion test. Of the
200 samples screened, 112 (56%) had positive cultures with the dominant
bacterial pathogens being Haemophilus influenzae (20%), followed by
Streptococcus pneumoniae (15%), Klebsiella pneumoniae (11%), and
Staphylococcus aureus (10%). Although 56% of the isolates were
recovered from females compared to 44% from males, the difference was
not statistically significant (p>0.05). Sixty-seven percent of the
pathogens were isolated from the age-group of 10-13 years, 19.6% from
the age-group of 14-17 years, and 12.5% from the age-group of 18-21
years. Antibiotic susceptibility testing revealed that gentamicin (92%)
and cefuroxime (88.4%) were the most effective antibiotics against the
isolates. Generally, susceptibility ranged from 0% to 92% depending on
the antibiotic and the species of microorganism. Penicillin had the
highest (100%) resistance to all the isolates. The findings revealed
that students living in boarding schools in the Buea Municipality were
at risk of acquiring upper respiratory tract infections from their
peers since the upper respiratory tract of more than 50% of the
students was colonized with respiratory pathogens. Although
multidrug-resistant strains of organisms were identified, gentamicin
and cefuroxime are recommended as the first-line antibiotics of choice
against the pathogens. There is, therefore, a need for surveillance of
nasopharyngeal carriage of resistant strains of these organisms,
especially H. influenzae in unhealthy school children since the vaccine
is yet to be introduced in Cameroon. The findings have clinical and
epidemiological significance
Environmental factors affecting malaria parasite prevalence in rural Bolifamba, South- West Cameroon
The impact of some environmental factors on malaria parasite prevalence was investigated in rural Bolifamba, Cameroon. The study population comprised 1454 subjects aged 0 – 65 years. Malaria parasite prevalence was higher in the rainy (50.1%) than in the dry season (44.2%) with a significant difference (P=0.001) in mean parasite density between seasons. Individuals 15 years (37.4%). Malaria parasite prevalence (P=0.001) and parasite density (P=0.03) were higher in the individuals of wooden plank houses than those of cement brick houses. Inhabitants of houses surrounded by bushes or garbage heaps and swamps or stagnant water showed higher malaria parasite prevalence and densities compared with those from cleaner surroundings. Anopheles gambiae (63.8%) and A. funestus (32.8%) were associated with perennial transmission of malaria. Our data indicates that poor environmental sanitation and housing conditions may be significant risk factors for malaria parasite burden in Bolifamba. The impact of low erythrocyte density in human blood on the fitness and energetic reserves of the African malaria vector Anopheles gambiae
Background
Anaemia is a common health problem in the developing world. This condition is characterized by a reduction in erythrocyte density, primarily from malnutrition and/or
infectious diseases such as malaria. As red blood cells are the primary source of protein for haematophagous mosquitoes, any reduction could impede the ability of mosquito vectors to transmit malaria by influencing their fitness or that of the parasites they transmit. The aim of this study was to determine the impact of differences in the density of red blood cells in human blood on malaria vector (Anopheles gambiae sensu stricto) fitness. The hypotheses tested are that mosquito vector energetic reserves and fitness are negatively influenced by reductions in the red cell density of host human blood meals commensurate with those expected from severe anaemia.
Methods
Mosquitoes (An. gambiae s.s.) were offered blood meals of different packed cell volume(PCV) of human blood consistent with those arising from severe anaemia (15%) and normalPCV (50%). Associations between mosquito energetic reserves (lipid, glucose and glycogen)and fitness measures (reproduction and survival) and blood meal PCV were investigated.
Results
The amount of protein that malaria vectors acquired from blood feeding (indexed by
haematin excretion) was significantly reduced at low blood PCV. However, mosquitoes
feeding on blood of low PCV had the same oviposition rates as those feeding on blood of normal PCV, and showed an increase in egg production of around 15%. The long-term survival of An. gambiae s.s was reduced after feeding on low PCV blood, but PCV had no significant impact on the proportion of mosquitoes surviving through the minimal period required to develop and transmit malaria parasites (estimated as 14 days post-blood feeding). The impact of blood PCV on the energetic reserves of mosquitoes was relatively minor.
Conclusions
These results suggest that feeding on human hosts whose PCV has been depleted due to severe anaemia does not significantly reduce the fitness or transmission potential of malaria vectors, and indicates that mosquitoes may be able exploit resources for reproduction more
efficiently from blood of low rather than normal PCV

Confirmed malaria cases among children under five with fever and history of fever in rural western Tanzania
The World Health Organization recommends that malaria treatment should begin with parasitological diagnosis. This will help to control misuse of anti-malarial drugs in areas with low transmission. The present study was conducted to assess the prevalence of parasitologically confirmed malaria among children under five years of age presenting with fever or history of fever in rural western Tanzania. A finger prick blood sample was obtained from each child, and thin and thick blood smears were prepared, stained with 10% Giemsa and examined under the light microscope. A structured questionnaire was used to collect each patient's demographic information, reasons for coming to the health center; and a physical examination was carried out on all patients. Fever was defined as axillary temperature ≥ 37.5°C. A total of 300 children with fever or a history of fever (1 or 2 weeks) were recruited, in which 54.3% (163/300, 95%CI, 48.7-59.9) were boys. A total of 76 (76/300, 25.3%, 95%CI, 22.8 - 27.8) of the children had fever. Based on a parasitological diagnosis of malaria, only 12% (36/300, 95%CI, 8.3-15.7) of the children had P. falciparum infection. Of the children with P. falciparum infection, 52.7% (19/36, 95%CI, 47.1-58.3) had fever and the remaining had no fever. The geometrical mean of the parasites was 708.62 (95%CI, 477.96-1050.62) parasites/μl and 25% (9/36, 95%CI, 10.9 -- 39.1) of the children with positive P. falciparum had ≥ 1001 parasites/μl. On Univariate (OR = 2.13, 95%CI, 1.02-4.43, P = 0.044) and multivariate (OR = 2.15, 95%CI, 1.03-4.49) analysis, only children above one year of age were associated with malaria infections. Only a small proportion of the children under the age of five with fever had malaria, and with a proportion of children having non-malaria fever. Improvement of malaria diagnostic and other causes of febrile illness may provide effective measure in management of febrile illness in malaria endemic areas

Complex Interactions between Soil-Transmitted Helminths and Malaria in Pregnant Women on the Thai-Burmese Border
Intestinal worms, particularly hookworm and whipworm, can cause anaemia, which is harmful for pregnant women. The WHO recommends deworming in pregnancy in areas where hookworm infections are frequent. Some studies indicate that coinfection with worms and malaria adversely affects pregnancy whereas other studies have shown that coinfection with worms might reduce the severity of malaria. On the Thai-Burmese border malaria in pregnancy has been an important cause of maternal death. We examined the relationship between intestinal helminth infections in pregnant women and their malaria risk in our antenatal care units. In total 70% of pregnant women had worm infections, mostly hookworm, but also roundworm and whipworm; hookworm was associated with mild anaemia although ova counts were not high. Women infected with hookworm had more malaria and their babies had a lower birth weight than women without hookworm. In contrast women with roundworm infections had the lowest rates of malaria in pregnancy. Deworming eliminates all worms. In this area it is unclear whether mass deworming would be beneficial
