    Non-Judicial Hanging in Guilan Province, Iran between 2011 and 2013

    Introduction: Hanging is one of the most commonly used way to commit suicide in many countries. This method used in suicide is considered a problem in Iran too, but no clear data exists regarding hanging in different regions or the country as a whole. Because of the epidemiologic differences in non-judicial hanging in different regions, this study aimed to assess it in Gilan province, Iran between 2011 and 2013. Methods: In this cross-sectional study, profiles of hanging cases registered in Poorsina hospital in Gilan, Iran between 2011 and 2013 were evaluated. Age, sex, marital status, place of residency, level of education, occupation, history of suicide, history of clinical illness, season of suicide and hanging outcome were evaluated. Results: 59 cases of hanging (mean age 31.4 ± 13.1 years and 83.0% male) were evaluated. 12 (20.34%) suffered from psychological disorders, and 9 (15.2%) confessed to substance abuse. 7 (11.9%) had a history of suicide attempts by hanging. Hanging was significantly higher in men (p<0.001), people with an education level of less than high school diploma (p=0.02) and the unemployed (p<0.05) patients. In the end, 20 (33.9%) of these attempts resulted in death. Conclusion: The results of this study showed that in 2 years, 59 cases committed suicide by hanging themselves, 33.9% of which finally died.     Toxoplasmosis in Patients with Cardiac Disorders: a Systematic Review and Meta-Analysis

    Toxoplasmosis is a common and serious infection caused by an obligatory intracellular protozoan, Toxoplasma gondii. This study investigated the possible association between heart failure and toxoplasmosis. We searched for toxoplasmosis and heart failure patients in English databases including PubMed, Scopus, ISI Web of Sciences, Science Direct, EMBASE, and Google Scholar up to June 2018. A total of 6 studies and 1,795 participants, comprising 934 cases and 861 controls, had acceptable criteria for entering the study. Im­munoglobulin G (IgG) antibodies against T. gondii were found in 53% (22 to 83) of patients with heart diseases and 26% (11 to 42) of healthy controls. In comparison, immunoglobulin M (IgM) antibodies were found in 0.5% (0.1 to 1) in patients with heart diseases and 0.3% (0 to 0.7) of healthy controls. The patients suffering from cardiac disorders were more significantly correlated to anti-T. gondii IgG (OR: 3.53; 95% CI, 2.27 to 5.47; P = 0.014) and IgM (OR: 1.80; 95% CI, 0.31 to 10.4; P = 0.028) seropositivity than healthy controls. Despite limitations such as the low number of studies, our research showed a high association between toxoplasmosis and cardiac disorders. Therefore, toxoplasmosis may be a risk factor in cardiac patients, and more studies are being done

    Cytotoxic and Immunomodulatory Activity of Curcumin and Chitosan on Experimental Toxoplasmosis

    Background: Toxoplasma gondii is a pathogenic parasite with worldwide distribution. We investigated curcumin and chitosan in combination on the viability of T. gondii tachyzoites in silico, in vitro and in vivo. Methods: A 3D model was employed in Urmia University of Medical Sciences, Urmia, Iran in 2021 to study the interaction between curcumin and dihydrofolate reductase (DHFR). Ramachandran root-mean-square deviation and VERIFY3D validated the model. Cytotoxicity of curcumin and chitosan was evaluated by MTT viability assay. BALB/c mice infected with 104 Toxoplasma organisms were treated with curcumin, chitosan, and the combination of curcumin+chitosan. Serum levels of inducible NO synthetase (iNOs), interferon gamma (IFN-γ), interleukin (IL)-5, glutamate oxaloacetic transaminases (SGOT), and glutamic pyruvate transaminase (SGPT) were determined. Result: Curcumin-DHFR and curcumin-DHPS (dihydropteroate synthase) interactions and calculated enzyme energy indicated an excellent affinity for curcumin with DHFR, but not DHPS. MTT results of concurrent treatments demonstrated IC50 rates of 0.1, 0.05, and 0.01 mg/ml at 24, 48, and 72h, respectively. IFN-γ, IL-5 and iNOs levels in curcumin+chitosan treated mice were 1.71, 0.51, and 1.51 IU/L, while those of SGOT and SGPT were 76 and 84 IU/L, respectively. Conclusion: The combination of curcumin and chitosan increased survival time of infected mice by seven days. Curcumin and chitosan in combination regulated the immune system and reduced liver damage, potentially forming the basis of a new treatment for toxoplasmosis

    The effectiveness of emotion-focused therapy on body image among women with eating disorders during COVID-19 pandemic lockdown

    BACKGROUND: It was argued that the coronavirus disease 2019 (COVID-19) lockdown could affect eating disorders (EDs) by triggering EDs behaviors and exacerbating existing symptoms. This study was carried out to examine the effectiveness of emotion-focused therapy (EFT) on body image of women with EDs during the COVID-19 pandemic lockdown. METHODS: This was a quasi-experimental study with pretest-posttest and a control group. The statistical population included all people with physical image disorder from September to October 2021. Thirty-four were selected using a purposeful sampling method. They were randomly assigned to experimental (n = 17) and control (n = 17) groups. The experimental group underwent eight sessions of 120-minute online treatment, one session per week. The control group did not receive any intervention until the end of the experimental group. Measurement tool was Cash Multidimensional Body-Self Relations Questionnaire (MBSRQ). The experimental group received eight sessions of EFT; however, the control group did not receive such training. The research data were examined through multivariate analysis of covariance (MANCOVA) in SPSS software. RESULTS: The intervention yielded more substantial outcomes concerning the variables mentioned in the experimental group, surpassing the impact observed in the control group. Based on the scores from the appearance assessment scales (F = 0.39, P < 0.001), appearance orientation (F = 196.71, P < 0.001), and satisfaction with body area (F = 0.25, P = 0.61), there was a significant difference between the two groups. According to the eta-squared (η2), the appearance orientation scale had the most significant impact on the effectiveness of EFT. This means that only 0.87% of the difference in appearance orientation scores after the test could be attributed to the therapy's effectiveness. CONCLUSION: Given the results of the current study and the research background, EFT can be recommended as an efficient interventional method to improve women’s body image

    The Role of Voluntary Function in Predicting Addiction Potential: A Survey on Iranian Red Crescent Societies

    Background and Aim:Considering Voluntary Function, the purpose of the present study was to predictthe addiction potential among some members of the Iranian Red Crescent Society (IRCS).Materials and Methods:The research method was descriptive-correlational. The statistical population of the present study was all members of the Iranian Red Crescent Society (IRCS). The sampling method was multi-stage cluster sampling, in which 620 active volunteers of the IRCS from 31 province and 175 cities of Iran (48.7% female and 50.1 Male mean age 23.27±3.32, range 14–31 years)were selected for this research.The research data was collected using theIranian Addiction Potential Scale (IAPS) and Voluntary Function Inventory (VFI). Results:Findings proved that there was a negative significant correlation between the AP and all measurements of VF such as protective enhancement, understanding, career, values, and motives; meaning that the more time youth spent on participating in voluntary activities, the less likely they sought to resort to misusing AP. Findings of the multiple regression has proved that volunteerism could predict 15% of changes in the AP as a criterion variable.Conclusion:Voluntary function can increase happiness, mental health, expand interpersonal relationships and social networking, self-esteem and social skills in individuals. These skills can reduce the high-risk behaviors, including addiction. Therefore, it is necessary to pay attention to this valuable factors in preventive programs

    Study of type and frequency of Alfa-thalassemia mutations in a cohort of 3,823 patients from Isfahan Province, Iran

    Introduction: Alpha-thalassemia (α-thalassemia) is caused by a range of mutations in the α-globin gene resulting in the complete reduction or absence of α-globin chain production. Material and methods: This study assessed the presence of α-thalassemia in 3,823 patients referred to Al-Zahra Hospital, Isfahan, Iran during a 10-year period (from 2012 to 2022). These patients experienced anaemia for more than ten years but had not the full indication for β-thalassemia or iron deficiency. Results: Based on the present assessment, 3,483 cases out of 3,823 suspicious cases had an α-Thalassemia-involved mutation (91.1%). According to the results, the most common detected mutation in the α-thalassemia carriers of Isfahan province was –α3.7 with a frequency of 81.58% (3,119 individuals), followed by α5nt (–TGAGG) (3.71% in total or 39.01% between 364 patients), polyadenylation signal mutations (polyA2) (14.28% between 364 patients), αcodon 19 (GCG4GC–, a2) (11.53%), –α3.7/–α3.7 (11.53%), –α20.5 (7.69%), Hb Constant Spring [Hb CS, a142, Stop →Gln; HBA2: c.427T4C] (5.7%), α4.2 (5.49) and – –MED (4.67%). Conclusion: The results of this investigation may be valuable for designing a program for carrier screening, premarital genetic counselling, and prenatal diagnosis in the Isfahan province

    High variations of the thebaine concentrations in Iranian poppy (Papaver bracteatum Lindl.) from various regions in Iran

    This paper aims to contribute reliable information to promote the pharmacological utilization of Papaver bracteatum by analyzing itswild population in Iran. For this purpose, 27 Iranian poppy specimens from 7 provinces of Iran were analyzed. The highest thebaine concentrations were detected in the capsules, whereas they were relatively lower in the flowering stems. Maximal concentration (more than 53 mg/g d.w.) was present in the P. bracteatum plant capsules growing in the Zanus region of Mazandaran province. Surprisingly, the thebaine concentration varies drastically excessively among the tested specimens, ranging from 5 to more than 53 mg/g d.w. Up to now, it is not known whether these drastic differences are due to clonal variations or the differences in the growth conditions, e.g., due to different stress levels of the plants. There is no doubt that the Iranian poppy represents an important alternative source for thebaine, yet, further studies and research are required to promote the farming of this auspicious pharmaceutical plant. In this context, it is of special interest to elucidate the effects of potential genotypes, the impact of the growing conditions as well as seasonal effects

    The association between acylcarnitine and amino acids profile and metabolic syndrome and its components in Iranian adults: Data from STEPs 2016

    BackgroundEvidence, albeit with conflicting results, has suggested that cardiometabolic risk factors, including obesity, type 2 diabetes (T2D), dyslipidemia, and hypertension, are highly associated with changes in metabolic signature, especially plasma amino acids and acylcarnitines levels. Here, we aimed to evaluate the association of circulating levels of amino acids and acylcarnitines with metabolic syndrome (MetS) and its components in Iranian adults.MethodsThis cross-sectional study was performed on 1192 participants from the large–scale cross-sectional study of Surveillance of Risk Factors of non-communicable diseases (NCDs) in Iran (STEP 2016). The circulating levels of amino acids and acylcarnitines were measured using liquid chromatography-tandem mass spectrometry (LC-MS/MS) in individuals with MetS (n=529) and without MetS (n=663).ResultsThe higher plasma levels of branched-chain amino acids (Val, Leu), aromatic amino acids (Phe, Tyr), Pro, Ala, Glu, and the ratio of Asp to Asn were significantly associated with MetS, whereas lower circulating levels of Gly, Ser, His, Asn, and citrulline were significantly associated with MetS. As for plasma levels of free carnitine and acylcarnitines, higher levels of short-chain acylcarnitines (C2, C3, C4DC), free carnitine (C0), and long-chain acylcarnitines (C16, C18OH) were significantly associated with MetS. Principal component analysis (PCA) showed that factor 3 (Tyr, Leu, Val, Met, Trp, Phe, Thr) [OR:1.165, 95% CI: 1.121-1.210, P&lt;0.001], factor 7 (C0, C3, C4) [OR:1.257, 95% CI: 1.150-1.374, P&lt;0.001], factor 8 (Gly, Ser) [OR:0.718, 95% CI: 0.651-0.793, P&lt; 0.001], factor 9 (Ala, Pro, C4DC) [OR:1.883, 95% CI: 1.669-2.124, P&lt;0.001], factor 10 (Glu, Asp, C18:2OH) [OR:1.132, 95% CI: 1.032-1.242, P= 0.009], factor 11 (citrulline, ornithine) [OR:0.862, 95% CI: 0.778-0.955, P= 0.004] and 13 (C18OH, C18:1 OH) [OR: 1.242, 95% CI: 1.042-1.480, P= 0.016] were independently correlated with metabolic syndrome.ConclusionChange in amino acid, and acylcarnitines profiles were seen in patients with MetS. Moreover, the alteration in the circulating levels of amino acids and acylcarnitines is along with an increase in MetS component number. It also seems that amino acid and acylcarnitines profiles can provide valuable information on evaluating and monitoring MetS risk. However, further studies are needed to establish this concept

    The unfinished agenda of communicable diseases among children and adolescents before the COVID-19 pandemic, 1990-2019: a systematic analysis of the Global Burden of Disease Study 2019

    BACKGROUND: Communicable disease control has long been a focus of global health policy. There have been substantial reductions in the burden and mortality of communicable diseases among children younger than 5 years, but we know less about this burden in older children and adolescents, and it is unclear whether current programmes and policies remain aligned with targets for intervention. This knowledge is especially important for policy and programmes in the context of the COVID-19 pandemic. We aimed to use the Global Burden of Disease (GBD) Study 2019 to systematically characterise the burden of communicable diseases across childhood and adolescence. METHODS: In this systematic analysis of the GBD study from 1990 to 2019, all communicable diseases and their manifestations as modelled within GBD 2019 were included, categorised as 16 subgroups of common diseases or presentations. Data were reported for absolute count, prevalence, and incidence across measures of cause-specific mortality (deaths and years of life lost), disability (years lived with disability [YLDs]), and disease burden (disability-adjusted life-years [DALYs]) for children and adolescents aged 0-24 years. Data were reported across the Socio-demographic Index (SDI) and across time (1990-2019), and for 204 countries and territories. For HIV, we reported the mortality-to-incidence ratio (MIR) as a measure of health system performance. FINDINGS: In 2019, there were 3·0 million deaths and 30·0 million years of healthy life lost to disability (as measured by YLDs), corresponding to 288·4 million DALYs from communicable diseases among children and adolescents globally (57·3% of total communicable disease burden across all ages). Over time, there has been a shift in communicable disease burden from young children to older children and adolescents (largely driven by the considerable reductions in children younger than 5 years and slower progress elsewhere), although children younger than 5 years still accounted for most of the communicable disease burden in 2019. Disease burden and mortality were predominantly in low-SDI settings, with high and high-middle SDI settings also having an appreciable burden of communicable disease morbidity (4·0 million YLDs in 2019 alone). Three cause groups (enteric infections, lower-respiratory-tract infections, and malaria) accounted for 59·8% of the global communicable disease burden in children and adolescents, with tuberculosis and HIV both emerging as important causes during adolescence. HIV was the only cause for which disease burden increased over time, particularly in children and adolescents older than 5 years, and especially in females. Excess MIRs for HIV were observed for males aged 15-19 years in low-SDI settings. INTERPRETATION: Our analysis supports continued policy focus on enteric infections and lower-respiratory-tract infections, with orientation to children younger than 5 years in settings of low socioeconomic development. However, efforts should also be targeted to other conditions, particularly HIV, given its increased burden in older children and adolescents. Older children and adolescents also experience a large burden of communicable disease, further highlighting the need for efforts to extend beyond the first 5 years of life. Our analysis also identified substantial morbidity caused by communicable diseases affecting child and adolescent health across the world.     Adolescent transport and unintentional injuries: a systematic analysis using the Global Burden of Disease Study 2019

    Background: Globally, transport and unintentional injuries persist as leading preventable causes of mortality and morbidity for adolescents. We sought to report comprehensive trends in injury-related mortality and morbidity for adolescents aged 10–24 years during the past three decades. Methods: Using the Global Burden of Disease, Injuries, and Risk Factors 2019 Study, we analysed mortality and disability-adjusted life-years (DALYs) attributed to transport and unintentional injuries for adolescents in 204 countries. Burden is reported in absolute numbers and age-standardised rates per 100 000 population by sex, age group (10–14, 15–19, and 20–24 years), and sociodemographic index (SDI) with 95% uncertainty intervals (UIs). We report percentage changes in deaths and DALYs between 1990 and 2019. Findings: In 2019, 369 061 deaths (of which 214 337 [58%] were transport related) and 31·1 million DALYs (of which 16·2 million [52%] were transport related) among adolescents aged 10–24 years were caused by transport and unintentional injuries combined. If compared with other causes, transport and unintentional injuries combined accounted for 25% of deaths and 14% of DALYs in 2019, and showed little improvement from 1990 when such injuries accounted for 26% of adolescent deaths and 17% of adolescent DALYs. Throughout adolescence, transport and unintentional injury fatality rates increased by age group. The unintentional injury burden was higher among males than females for all injury types, except for injuries related to fire, heat, and hot substances, or to adverse effects of medical treatment. From 1990 to 2019, global mortality rates declined by 34·4% (from 17·5 to 11·5 per 100 000) for transport injuries, and by 47·7% (from 15·9 to 8·3 per 100 000) for unintentional injuries. However, in low-SDI nations the absolute number of deaths increased (by 80·5% to 42 774 for transport injuries and by 39·4% to 31 961 for unintentional injuries). In the high-SDI quintile in 2010–19, the rate per 100 000 of transport injury DALYs was reduced by 16·7%, from 838 in 2010 to 699 in 2019. This was a substantially slower pace of reduction compared with the 48·5% reduction between 1990 and 2010, from 1626 per 100 000 in 1990 to 838 per 100 000 in 2010. Between 2010 and 2019, the rate of unintentional injury DALYs per 100 000 also remained largely unchanged in high-SDI countries (555 in 2010 vs 554 in 2019; 0·2% reduction). The number and rate of adolescent deaths and DALYs owing to environmental heat and cold exposure increased for the high-SDI quintile during 2010–19. Interpretation: As other causes of mortality are addressed, inadequate progress in reducing transport and unintentional injury mortality as a proportion of adolescent deaths becomes apparent. The relative shift in the burden of injury from high-SDI countries to low and low–middle-SDI countries necessitates focused action, including global donor, government, and industry investment in injury prevention. The persisting burden of DALYs related to transport and unintentional injuries indicates a need to prioritise innovative measures for the primary prevention of adolescent injury.
