    The subject. The impact of digital transformation on the foundations of the constitutional system, namely the possibility of its influence on their form, content and implementation system, has been studied in this article. The authors put forward a hypothesis about the formation of the principle of the digital state, which underlies the interaction of society and the state in the conditions of building a digital economy in Russia. Active implementation of this principle was observed during the COVID-19 pandemic. Also, the factors that can give an obvious and non-obvious impetus to the transformation of the constitutional identity of the foundations of the constitutional system are identified.The main functions of the constitutional principles are to preserve the stability of the foundations of the constitutional system and constitutional identity.     Chronic thromboembolic pulmonary hypertension after an acute pulmonary embolism: fundamental concepts of diagnosis and review of current treatment options

    Chronic thromboembolic pulmonary hypertension (CTEPH) is a severe and underdiagnosed disorder that leads to right ventricular failure and, subsequently, to death. The article discusses the fundamental concepts of monitoring patients after a pulmonary embolism (PE) in order to timely detect CTEPH. The pathogenetic processes, risk factors and diagnostic criteria of this complication are described. The role of early diagnosis of CTEPH, which may be important in relation to further outcomes, is emphasized. A modern strategy for monitoring patients after PE is presented in detail, where echocardiography plays a key role. The stepwise diagnostic algorithm for CTEPH includes a comprehensive analysis of the patient’s condition: assessment of clinical status, identification of N-terminal pro-brain natriuretic peptide levels, cardiopulmonary exercise testing, imaging diagnostic procedures (ventilation/perfusion lung scanning, pulmonary angiography) and, finally, right heart catheterization. The article provides an overview of modern treatment options. CTEPH is a unique type of pulmonary hypertension because it is potentially curable with surgery β€” pulmonary thromboendarterectomy. For patients with inoperable or persistent/ recurrent CTEPH, medication therapy is recommended. Currently, the only drug with a high class of evidence for this group of patients is riociguat. A positive effect on exercise tolerance, functional class, and hemodynamic parameters has been shown in the CHEST-1,2 studies. The favorable safety profile of the drug was also demonstrated in the long-term follow-up in routine clinical practice (EXPERT registry). The presence of various options in the management of patients and development of a multimodal therapy makes it possible to provide high-quality care to patients with CTEPH, and, namely, fast and accurate diagnosis plays a key role in timely treatment

    The Nature of Regulatory Autoantibody Changes Among Pregnant Women with Intrauterine Growth Restriction

    Β© 2016, Springer Science+Business Media New York.Anenzyme-linked immunosorbent assay known as ELI-TEST was applied in 388 pregnant women in the gestational complication risk group who were 11– 4Β weeks into pregnancy for the purpose of determining the serum content of Ig G class/that bind to double-stranded DNA, Ξ²2-glycoprotein I (Ξ²2 GP), total phospholipids (TFL), HCG vasculopathy markers (ANCA), collagen (Coll), PAPP-A, and insulin (Ins). After standardizing groups, 80 pregnant women with intrauterine growth restriction (IUGR) were selected for analysis. Characteristic changes in the levels of autoantibodies accompanying IUGR of varying severity were identified which not only gives a more accurate understanding of the pathogenesis of the disease but also enables one to define IUGR risk groups in the early stages of pregnancy

    Organizational factors for manufacturing defects reduction in small batch production

    The study is devoted to the defects organizational causes analysis in the machine building products production process. The companies with the small batch production were taken into consideration. Factors related to the time norms fulfilling percentage, the work rhythm, the consolidation of operations at one workplace, the new technologies introduction and equipment failures were chosen. A preliminary factors selection according to independence criteria allowed us to build a reliable regression model for the defects rate calculating. The main factors influencing the defects rate in small batch production are the time norms fulfilling percentage, the rate of consolidation of operations at one workplace, and the number of new technological processes introduced. This allowed us to offer recommendations on the machining sites work organization. The final operations and productswhich are made from expensive metals should be carried out at workplaces with a low rate of operations consolidation. Also, it is important that the wage system should not contribute to over-fulfillment of norms. The maintenance and repair system should prevent malfunctions during operations fulfilment. It was found that including autoantibodies to insulin, PAPP-A, and collagen into the analysis significantly improved the sensitivity and specificity of the prognosis and could be employed to form an intrauterine growth restriction (IUGR) risk group

    Comparative analysis of cutting fluids

    The article contains the results of seven working tests of cutting fluids for metal-cutting equipment. The results of the analysis were evaluated by experts, including process engineers, chemical engineers, foremen and production machine operators in the following areas: turning, drilling, milling, sawing, grinding, threading, boring works. The analysis made it possible to determine the scope of application of cutting fluids depending on the volume and rhythm of the work, the operating conditions of the cutting fluids, the requirements of the design and technological documentation taking into account the final cost of the cutting fluids used. The problem of determining an accelerated system of practical tests for cutting fluids to evaluate the effectiveness of using a particular product before conducting the production tests on machine equipment is considered. For this, criteria for evaluating the cutting fluids are proposed using the weighting method to ensure informed decision-making in the choice of a cutting fluid for admission to full-fledged production tests.     Study of the mechanism of action of p-chloromercuribenzoate on endonuclease from the bacterium Serratia marcescens

    The mechanism of action of p-chloromercuribenzoate (PCMB) on Serratia marcescens nuclease was investigated. The analysis showed that PCMB forms complexes with DNA. Binding of C7H5O2Hg+ to DNA changes the secondary structure of the DNA. These changes alter the enzymatic activity of S. marcescens nuclease, which was previously found to be sensitive to the secondary structure of the substrates. The nuclease activity was either suppressed or stimulated in the presence of PCMB depending on the C7H5O2Hg+ to nucleotide equivalent ratio. Binding of C7H5O2Hg+ to DNA did not form an abortive enzyme-substrate complex. Binding of Mg2+ to the C7H5O2Hg-DNA complex caused appropriate changes in secondary structure of the substrate. Since Mg2+ and C7H5O2Hg+, though differing in the type of metal cation, are similar in their mechanisms of influence on enzymatic activity of S. marcescens nuclease, the identity of other metal-containing effectors in their mechanism of action on Serratia marcescens nuclease is assumed
