    O Estresse Ocupacional e a Síndrome de Burnout no Ambiente de Trabalho: Suas Influências no Comportamento dos Professores da Rede Privada do Ensino Médio de João Pessoa

    O propósito da presente pesquisa foi investigar a ocorrência de estresse ocupacional e síndrome de burnout em professores da rede de ensino privado da cidade de João Pessoa. Participaram da pesquisa nove professores, de ambos os sexos, que lecionam em diversas escolas e estas foram escolhidos aleatoriamente. Na coleta dos dados foi empregado um quadro baseado no Maslach Burnout Inventory (MBI), composto por 29 questões que buscavam identificar a presença do burnout a partir de três dimensões: comportamental, cognitiva e física, e 17 perguntas que analisaram o perfil dos pesquisados. Os resultados obtidos demonstram que: 30,76% comportamentais, 38,45% físicos e 30,76% cognitivos, sendo assim podemos afirmar que a maioria dos professores apresentou diversos sintomas do estresse ocupacional e em apenas 11,11% foram apresentadas características da síndrome de burnou

    Rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART): Study protocol for a randomized controlled trial

    Background: Acute respiratory distress syndrome (ARDS) is associated with high in-hospital mortality. Alveolar recruitment followed by ventilation at optimal titrated PEEP may reduce ventilator-induced lung injury and improve oxygenation in patients with ARDS, but the effects on mortality and other clinical outcomes remain unknown. This article reports the rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART). Methods/Design: ART is a pragmatic, multicenter, randomized (concealed), controlled trial, which aims to determine if maximum stepwise alveolar recruitment associated with PEEP titration is able to increase 28-day survival in patients with ARDS compared to conventional treatment (ARDSNet strategy). We will enroll adult patients with ARDS of less than 72 h duration. The intervention group will receive an alveolar recruitment maneuver, with stepwise increases of PEEP achieving 45 cmH(2)O and peak pressure of 60 cmH2O, followed by ventilation with optimal PEEP titrated according to the static compliance of the respiratory system. In the control group, mechanical ventilation will follow a conventional protocol (ARDSNet). In both groups, we will use controlled volume mode with low tidal volumes (4 to 6 mL/kg of predicted body weight) and targeting plateau pressure <= 30 cmH2O. The primary outcome is 28-day survival, and the secondary outcomes are: length of ICU stay; length of hospital stay; pneumothorax requiring chest tube during first 7 days; barotrauma during first 7 days; mechanical ventilation-free days from days 1 to 28; ICU, in-hospital, and 6-month survival. ART is an event-guided trial planned to last until 520 events (deaths within 28 days) are observed. These events allow detection of a hazard ratio of 0.75, with 90% power and two-tailed type I error of 5%. All analysis will follow the intention-to-treat principle. Discussion: If the ART strategy with maximum recruitment and PEEP titration improves 28-day survival, this will represent a notable advance to the care of ARDS patients.     OBJETIVO: investigar o nível de consciência dos pais em relação à importância da triagem auditiva neonatal, bem como verificar os motivos do não comparecimento ao retorno agendado após a primeira avaliação. MÉTODO: a casuística foi constituída por 31 mães e/ou responsáveis dos recém-nascidos da unidade neonatal do Hospital Universitário de Maringá, os quais não compareceram ao reteste agendado após a alta hospitalar. RESULTADOS: os motivos apresentados pelas mães e/ou responsáveis para o não comparecimento ao retorno agendado envolveram atitudes que puderam sinalizar pouca importância atribuída às questões relacionadas à audição e a Triagem Auditiva Neonatal, pois sugiram motivos irrelevantes como o esquecimento do retorno, mãe pensou que o bebê não deixaria ser submetido ao exame e a perda de horário para nova avaliação. Tal fato, provavelmente reflete a falta de conscientização por parte das mães dos recém-nascidos quanto à importância da Triagem Auditiva Neonatal. CONCLUSÃO: existe a necessidade de aumentar a conscientização geral em relação à Triagem Auditiva Neonatal, por parte dos familiares e dos profissionais que atuam diretamente com os recém-nascidos, os quais contribuirão para a agilidade do processo diagnóstico, garantindo melhores perspectiva ao futuro de crianças portadoras de deficiência auditiva

    Nursing orientation given to children in antineoplastic chemotherapy treatment

    A qualitative descriptive study carried out in a public pediatric hospital using semi-structured interviews with nine nursing professionals. It aims to identify orientation given by the nursing staff to school-aged children regarding antineoplastic chemotherapy and the aspects which difficult and facilitate such orientation. The results point to the fact that children receive little information about the side effects of their treatment, for all other information is directed to parents. The child’s relationship with their peers and family members facilitates orientation and such relationship serves as a mediator for better acceptance of treatment on the child’s behalf. The fact that the children do not understand the language used by nursing professionals during orientation was pointed out as a hindering factor. In conclusion, professionals recognize that children are capable of understanding the given orientation, but find it difficult to communicate with them using accessible language, in accordance with the child’s development. doi: doi: 10.5216/ree.v16i2.27009

Maringá, PR, Brasil.Background: The objective of this study was to investigate the association between KIR genes and the immunopathogenesis of leprosy. Methods: The types of KIR and HLA genes were evaluated by PCR-SSOP-Luminex in 408 patients with leprosy and 413 healthy individuals. Statistical analysis was performed using the Chi-square or Fisher’s exact test and stepwise multivariate analysis. Results: There was a higher frequency of activating KIR genes (KIR2DS1, 2DS2 and 3DS1) together with their HLA ligands in the tuberculoid (TT) group as compared to the lepromatous leprosy (LL) group. KIR2DL2/2DL2-C1 was more frequent in the patient, TT and LL groups than in the control group. Borderline patients presented a higher frequency of inhibitory pairs when compared to the control group, and a higher frequency of activating pairs as compared to the LL group. Multivariate analysis confirmed the associations and demonstrated that being a female is a protective factor against the development of the disease per se and the more severe clinical form. Conclusions: This study showed that activating and inhibitory KIR genes may influence the development of leprosy – in particular, activating genes may protect against the more aggressive form of the disease – thereby demonstrating the role of NK cells in the immunopathology of the disease
